| COMMUNITY BENEFIT STATEMENT | CORE FORM, PART III, STATMENT OF PROGRAM SERVICE ACCOMPLISHMENTS | St. Luke's Warren Hospital ("SL-Warren") is a Joint Commission-accredited, not-for-profit 214-licensed bed acute care hospital located in Phillipsburg, Warren County, New Jersey, providing care primarily to residents of Warren and Hunterdon counties in New Jersey, and Easton, Northampton County, Pennsylvania. The hospital annually provides care for more than 7,000 observations and admissions, 93,000 outpatients and 29,836 ED visits and is recognized by the IRS as an Internal Revenue Code Section 501(c)(3) tax-exempt organization. Pursuant to its charitable purposes, SL-Warren provides medically necessary healthcare services to all individuals in a non-discriminatory manner regardless of race, color, creed, sex, national origin, religion or ability to pay. Founded in 1923, SL-Warren was acquired by nationally recognized St. Luke's University Health Network in 2012. The Hospital provides patients with access to more than 231 physicians across 45 medical specialties. The Hospital has earned Primary Stroke Center Designation (7/10) from the New Jersey Department of Health & Senior Services; and received Top Performer on Key Quality Measures status from The Joint Commission (10/12) for the second consecutive year. SL-Warren continues to participate in the New Jersey Department of Health & Seniors Services public reporting initiative. Performance improvement teams work to continuously improve the processes of care provided to patients suffering a heart attack, patients with pneumonia or heart failure and those undergoing surgical procedures. The Hospital has significantly expanded medical expertise through the addition of the following services: - Direct access to gynecologic oncologists; - Direct access to highly trained cardiac team which includes experienced and skilled cardiologists, cardiac and vascular surgeons, interventional radiologists and electrophysiologists.
