RESIDENTS WERE INVITED TO PUBLIC FORUMS FOR A COMPLETE OVERVIEW OF THE PRIMARY AND SECONDARY DATA THAT WAS COLLECTED AS WELL AS A DISCUSSION ON THE FIVE NEEDS THAT WERE IDENTIFIED. COMMUNITY RESIDENTS ARE ALSO INVITED ON AN ANNUAL BASIS TO PUBLIC FORUMS TO DISCUSS THE IMPACT OF PROGRAM IMPLEMENTATION AND EVALUATIONS OF THE COLLABORATIVE HEALTH IMPROVEMENT PLAN. PMC ALSO CONSULTED WITH COMMUNITY PARTNERS BY INVITING THEM TO A SEPARATE FOCUS GROUP. THE SAME FOCUS GROUP QUESTIONS WERE ASKED TO THE COMMUNITY PARTNERS AND THEIR COMMENTS WERE VALUED BASED ON THEIR PROFESSIONAL EXPERTISE AND EXPERIENCE IN WORKING WITH THE COMMUNITY SURROUNDING THE HOSPITAL. COMMUNITY PARTNERS ARE ALSO INVITED TO ASSIST IN THE PLANNING PROCESS OF THE COLLABORATIVE HEALTH IMPROVEMENT PLAN BY SUGGESTING POTENTIAL PROJECTS THAT THE HOSPITAL SHOULD IMPLEMENT AS WELL AS ATTENDING BI-ANNUAL MEETINGS ON EACH OF THE FIVE ISSUES. THE CHNA STEERING COMMITTEE INCLUDES HEALTH OFFICIALS AND HEALTH DIRECTORS FROM SIX MUNICIPALITIES IN THE SERVICE AREA, A REPRESENTATIVE FROM THE LOCAL FEDERALLY QUALIFIED HEALTH CENTER, A PRINCIPAL FROM THE UNION CITY BOARD OF EDUCATION, AND THE DIRECTOR OF HUDSON REGIONAL HEALTH COMMISSION, THE COUNTY'S ENVIRONMENTAL HEALTH AGENCY (CEHA) AND LOCAL INFORMATION NETWORK COMMUNICATION SYSTEMS AGENCY (LINCS). RESIDENTS WERE INVITED TO PUBLIC FORUMS FOR A COMPLETE OVERVIEW OF THE PRIMARY AND SECONDARY DATA THAT WAS COLLECTED AS WELL AS A DISCUSSION ON THE FIVE NEEDS THAT WERE IDENTIFIED. COMMUNITY RESIDENTS ARE ALSO INVITED ON AN ANNUAL BASIS TO PUBLIC FORUMS TO DISCUSS THE IMPACT OF PROGRAM IMPLEMENTATION AND EVALUATIONS OF THE COLLABORATIVE HEALTH IMPROVEMENT PLAN. PMC ALSO CONSULTED WITH COMMUNITY PARTNERS BY INVITING THEM TO A SEPARATE FOCUS GROUP. THE SAME FOCUS GROUP QUESTIONS WERE ASKED TO THE COMMUNITY PARTNERS AND THEIR COMMENTS WERE VALUED BASED ON THEIR PROFESSIONAL EXPERTISE AND EXPERIENCE IN WORKING WITH THE COMMUNITY SURROUNDING THE HOSPITAL.
