Return Reference Explanation
SCHEDULE D, PAGE 3, PART X THE SYSTEM IS A NOT-FOR-PROFIT CORPORATION THAT HAS BEEN RECOGNIZED AS TAX-EXEMPT PURSUANT TO SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE. THE SYSTEM APPLIES ACCOUNTING POLICIES THAT PRESCRIBE WHEN TO RECOGNIZE AND HOW TO MEASURE THE FINANCIAL STATEMENT EFFECTS OF INCOME TAX POSITIONS TAKEN OR EXPECTED TO BE TAKEN ON ITS INCOME TAX RETURNS. THESE RULES REQUIRE MANAGEMENT TO EVALUATE THE LIKELIHOOD THAT, UPON EXAMINATION BY THE RELEVANT TAXING JURISDICTIONS, THOSE INCOME TAX POSITIONS WOULD BE SUSTAINED. BASED ON THAT EVALUATION, THE SYSTEM ONLY RECOGNIZES THE MAXIMUM BENEFIT OF EACH INCOME TAX POSITION THAT IS MORE THAN 50% LIKELY OF BEING SUSTAINED. TO THE EXTENT THAT ALL OR A PORTION OF THE BENEFITS OF AN INCOME TAX POSITION ARE NOT RECOGNIZED, A LIABILITY WOULD BE RECOGNIZED FOR THE UNRECOGNIZED BENEFITS, ALONG WITH ANY INTEREST AND PENALTIES THAT WOULD RESULT FROM DISALLOWANCE OF THE POSITION. SHOULD ANY SUCH PENALTIES AND INTEREST BE INCURRED, THEY WOULD BE RECOGNIZED AS OPERATING EXPENSES. BASED ON THE RESULTS OF MANAGEMENT'S EVALUATION, NO LIABILITY IS RECOGNIZED IN THE ACCOMPANYING BALANCE SHEET FOR UNRECOGNIZED INCOME TAX POSITIONS. FURTHER, NO INTEREST OR PENALTIES HAVE BEEN ACCRUED OR CHARGED TO EXPENSE AS OF DECEMBER 31, 2014 AND 2013 OR FOR THE YEARS THEN ENDED. THE SYSTEM'S TAX RETURNS ARE SUBJECT TO POSSIBLE EXAMINATION BY THE TAXING AUTHORITIES. FOR FEDERAL INCOME TAX PURPOSES, THE TAX RETURNS ESSENTIALLY REMAIN OPEN FOR POSSIBLE EXAMINATION FOR A PERIOD OF THREE YEARS AFTER THE RESPECTIVE FILING DEADLINES OF THOSE RETURNS.

Form and Line Reference Explanation
PART I, LINE 7G - SUBSIDIZED HEALTH SERVICES EXPLANATION THIS SECTION INCLUDES 112,724 IN NET COSTS FOR PHYSICIAN CLINICS WHICH PROVIDE ACCESS TO PHYSICIAN SERVICES IN THE COMMUNITY REGARDLESS OF ABILITY TO PAY.
PART I, LINE 7, COLUMN (F) - EXCLUSIONS FROM PERCENT OF TOTAL EXPENSE IN DERIVING THE DENOMINATOR TO BE USED FOR COLUMN (F), THE FOLLOWING ADJUSTMENTS WERE MADE TO THE TOTAL EXPENSES REPORTED ON FORM 990, PART IX, LINE 25: FORM 990, PART IX, LINE 25 81,316,036 ADD: EXPENSES REPORTED IN PART VIII 728,211 DENOMINATOR FOR COLUMN (F) 82,044,247
PART I, LINE 7 - COSTING METHODOLOGY EXPLANATION THE DATA REPORTED IN THIS AREA IS REPORTED AS INSTRUCTED BY CATHOLIC HEALTH ASSOCIATION'S "A GUIDE FOR PLANNING AND REPORTING COMMUNITY BENEFITS, 2008". SEE ALSO THE DESCRIPTION FOR PART III, LINE 4.
PART III, LINE 2 - BAD DEBT EXPENSE METHODOLOGY AMOUNTS INCLUDED ON PART III LINE 2 REPRESENT THE AMOUNT OF CHARGES CONSIDERED UNCOLLECTIBLE AFTER REASONABLE ATTEMPTS TO COLLECT, AND WRITTEN OFF TO BAD DEBT EXPENSE. NEWTON MEDICAL CENTER IS THE SOLE PROVIDER OF 24 HOUR, 7 DAY A WEEK EMERGENT CARE IN NEWTON COUNTY, GEORGIA. IN ADDITION, MANY CITIZENS OF NEWTON COUNTY HAVE NO ACCESS TO PRIMARY CARE AS A RESULT OF THEIR INABILITY OR UNWILLINGNESS TO PAY FOR SERVICES. OTHERS HAVE NO ACCESS TO PRIMARY CARE PHYSICIANS DUE TO THEIR PARTICULAR INSURANCE PLAN. THIS IS ESPECIALLY TRUE OF MEDICAID RECIPIENTS. MANY OF THESE COMMUNITY MEMBERS SEEK CARE IN THE EMERGENCY ROOM AT NEWTON MEDICAL CENTER FOR BOTH EMERGENT AND NON-EMERGENT HEALTH CARE NEEDS. SOME OF THESE PATIENTS REQUIRE EXTENDED CARE AS INPATIENTS OR EXTENSIVE OBSERVATION. OUR EMERGENCY ROOM ALSO SERVES AS A HOLDING AREA FOR THE MENTALLY ILL, OFTEN KEEPING THESE CITIZENS FOR SEVERAL DAYS BEFORE THEY ARE PLACED IN A MORE APPROPRIATE SETTING. MANY OF THESE PATIENTS DO NOT MEET CRITERIA OR ARE NON-COMPLIANT IN COMPLETING THE PROCESS NEEDED TO SHOW ELIGIBILITY FOR CHARITY CARE. OTHERS ARE UNWILLING TO PAY, EVEN THOUGH THEY MAY HAVE THE MEANS TO DO SO. MANY INSURED PATIENTS HAVE HIGH DEDUCTIBLES AND ALSO FAIL TO PAY FOR THEIR CARE. IN ADDITION, NEWTON MEDICAL CENTER PROVIDES SERVICES TO PATIENTS ON AN ELECTIVE BASIS THAT ARE UNABLE TO MEET THEIR OUT OF POCKET EXPENSES UNDER THEIR INSURANCE PROGRAM, MEDICARE, OR OTHER THIRD PARTY PROVIDER. IN THESE CASES, THE HOSPITAL DOES WORK WITH THE PATIENTS TO MAKE PAYMENT ARRANGEMENTS, HOWEVER, MANY PATIENTS FAIL TO FOLLOW THROUGH OR ARE FINANCIALLY UNABLE TO COMPLETE THOSE ARRANGEMENTS. CARE IS PROVIDED DESPITE THE RISK THAT MANY PATIENTS WILL NOT MAKE AGREED UPON PAYMENTS. IN ALL OF THE CASES DESCRIBED ABOVE, THE RESULT IS THE HOSPITAL INCURRING BAD DEBT EXPENSE BY PROVIDING HEALTH CARE SERVICES THAT ARE NEEDED BY COMMUNITY MEMBERS. ACCORDINGLY, IT IS CLEAR THAT THE COMMUNITY BENEFITS FROM THE HOSPITAL INCURRING BAD DEBT EXPENSE FOR CARE PROVIDED.
BAD DEBT EXPENSE FOOTNOTE TO FINANCIAL STATEMENTS ACCOUNTS RECEIVABLE ARE REDUCED BY AN ALLOWANCE FOR DOUBTFUL ACCOUNTS. IN EVALUATING THE COLLECTABILITY OF ACCOUNTS RECEIVABLE, THE SYSTEM ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR PAYOR SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR DOUBTFUL ACCOUNTS AND PROVISION FOR BAD DEBTS. MANAGEMENT REGULARLY REVIEWS DATA ABOUT THESE MAJOR PAYOR SOURCES OF REVENUE IN EVALUATING THE SUFFICIENCY OF THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY COVERAGE, THE SYSTEM ANALYZES CONTRACTUALLY DUE AMOUNTS AND PROVIDES AN ALLOWANCE FOR DOUBTFUL ACCOUNTS AND A PROVISION FOR BAD DEBTS, IF NECESSARY (FOR EXAMPLE, FOR EXPECTED UNCOLLECTIBLES, DEDUCTIBLES AND COPAYMENTS ON ACCOUNTS FOR WHICH THE THIRD-PARTY PAYOR HAS NOT YET PAID, OR FOR PAYORS WHO ARE KNOWN TO BE HAVING FINANCIAL DIFFICULTIES THAT MAKE THE REALIZATION OF AMOUNTS DUE UNLIKELY). FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS (WHICH INCLUDES BOTH PATIENTS WITHOUT INSURANCE AND PATIENTS WITH DEDUCTIBLES AND COPAYMENT BALANCES DUE FOR WHICH THIRD-PARTY COVERAGE EXISTS FOR PART OF THE BILL), THE SYSTEM RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS PAST EXPERIENCE, WHICH INDICATES THAT MANY PATIENTS ARE UNABLE OR UNWILLING TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE STANDARD RATES (OR THE DISCOUNTED RATES IF NEGOTIATED) AND THE AMOUNTS ACTUALLY COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED IS CHARGED OFF AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. THE SYSTEM'S ALLOWANCE FOR DOUBTFUL ACCOUNTS FOR SELF-PAY PATIENTS DECREASED FROM 81% OF SELF-PAY ACCOUNTS RECEIVABLE AT DECEMBER 31, 2013, TO 79% OF SELF-PAY ACCOUNTS RECEIVABLE AT DECEMBER 31, 2014. IN ADDITION, THE SYSTEM'S SELF-PAY WRITE-OFFS DECREASED APPROXIMATELY 2,188,000 FROM APPROXIMATELY 13,999,000 FOR 2013 TO 11,811,000 FOR 2014. THE DECREASE IN SELF-PAY WRITE-OFFS IS DUE TO A DECREASE IN PATIENT VOLUME. THE SYSTEM HAS NOT CHANGED ITS CHARITY CARE OR UNINSURED DISCOUNT POLICIES DURING 2014 OR 2013.
PART III, LINE 8 - MEDICARE EXPLANATION MEDICARE ALLOWABLE COSTS ARE COMPUTED IN ACCORDANCE WITH COST REPORTING METHODOLOGIES UTILIZED ON THE MEDICARE COST REPORT AND IN ACCORDANCE WITH RELATED REGULATIONS. INDIRECT COSTS ARE ALLOCATED TO DIRECT SERVICE AREAS USING THE MOST APPROPRIATE STATISTICAL BASIS.
PART III, LINE 9B - COLLECTION PRACTICES EXPLANATION THE ORGANIZATION WRITES OFF PATIENT ACCOUNTS RECEIVABLE BALANCES FOR PATIENTS QUALIFYING FOR CHARITY CARE OR FINANCIAL ASSISTANCE AND DOES NOT MAKE FURTHER COLLECTION EFFORTS.
PART VI, LINE 2 - NEEDS ASSESSMENT LOCAL AND STATE DEMOGRAPHICS FROM THE NEWTON COUNTY CHAMBER OF COMMERCE. FINANCIAL RECORDS OF PATIENTS. FEEDBACK FROM HOSPITAL SPONSORED SCREENINGS AND SUPPORT GROUPS. IN 2013, THE ORGANIZATION CONDUCTED ITS FIRST FORMAL COMMUNITY HEALTH NEEDS ASSESSMENT.
PART VI, LINE 3 - PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE INFORMATION IS AVAILABLE VIA OUR WEBSITE AT: HTTP://WWW.PIEDMONT.ORG/PATIENT-TOOLS/PIEDMONT-FINANCIAL-ASSISTANCE. THERE IS ALSO A FINANCIAL ASSISTANCE BROCHURE AND OTHER INFORMATION FOUND WITHIN OUR PATIENT GUIDE. SIGNS ADVISING PATIENTS OF THE FINANCIAL ASSISTANCE POLICY ARE POSTED AND SUMMARIES ARE AVAILABLE IN ADMISSIONS AND IN THE EMERGENCY ROOM. PATIENTS THAT ARE POTENTIALLY ELIGIBLE FOR THE PROGRAMS ARE DIRECTED TO OUR FINANCIAL COUNSELORS FOR FURTHER SCREENING AND TO COMPLETE APPLICATION TO DETERMINE ELIGIBILITY. INPATIENTS WHO MAY BE ELIGIBLE FOR FINANCIAL ASSISTANCE ARE INFORMED OF THE FINANCIAL ASSISTANCE PROGRAMS AS PART OF THE INPATIENT DISCHARGE PROCESS. SOME OF THE STAFF ARE BI-LINGUAL AND ARE USED WITH NON-ENGLISH SPEAKING PATIENTS WHEN NEEDED. THE HOSPITAL USES MED- ASSIST TO WORK WITH UNINSURED PATIENTS TO ASSIST THEM IN DETERMINING IF THEY ARE ELIGIBLE FOR PROGRAMS SUCH AS DISABILITY, MEDICAID, AS WELL THE HOSPITAL'S FINANCIAL ASSISTANCE PROGRAM. ALL PATIENT ACCOUNTS, REGISTRATION, AND SCHEDULING STAFF ARE TRAINED TO UNDERSTAND THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY AND TO REFER PATIENTS WHO MAY BE ELIGIBLE TO OUR PATIENT COUNSELORS FOR FURTHER ASSISTANCE. ALL THIRD PARTIES THAT COLLECT PAYMENT ON BEHALF OF THE HOSPITAL HAVE BEEN MADE AWARE AND RECEIVED TRAINING TO IDENTIFY AND REFER PATIENTS FOR FINANCIAL ASSISTANCE. THEY ARE REQUIRED TO FOLLOW HOSPITAL POLICIES IN THESE MATTERS.
PART VI, LINE 4 - COMMUNITY INFORMATION NEWTON COUNTY HAS A HIGHER UNEMPLOYMENT RATE THAN THAT OF THE STATE OR NATION AT 8.4%. UNINSURED PATIENTS MAKE UP 21% OF NEWTON COUNTY. THE POPULATION OF THE COUNTY IS AT 102,446 AS OF 2013, WITH 54.1% BEING WHITE, 42.5% BLACK, WITH 3.4% HISPANIC OR OTHER. THE HISPANIC POPULATION HAS QUADRUPLED SINCE 2000. THE POPULATION IS AGING IN PLACE.
PART VI, LINE 5 - PROMOTION OF COMMUNITY HEALTH THE NEWTON COUNTY HOSPITAL AUTHORITY (AUTHORITY) REORGANIZED AND ESTABLISHED NEWTON HEALTH SYSTEM, INC. (SYSTEM). THE AUTHORITY IMPLEMENTED A REORGANIZATION PLAN WHEREBY ALL THE ASSETS, MANAGEMENT AND GOVERNANCE OF THE HOSPITAL WAS TRANSFERRED TO THE SYSTEM, A NOT-FOR-PROFIT CORPORATION, QUALIFIED AS AN ORGANIZATION DESCRIBED IN SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE. IN JANUARY OF 1996, THE AUTHORITY ENTERED INTO AN AGREEMENT TO LEASE THE REAL PROPERTY, REMAINING NET ASSETS AND OPERATIONS OF THE AUTHORITY TO THE SYSTEM. THE SYSTEM, LOCATED IN COVINGTON, GEORGIA, OPERATES NEWTON MEDICAL CENTER, A 101 BED NOT-FOR-PROFIT ACUTE CARE HOSPITAL. THE SYSTEM PROVIDES INPATIENT, OUTPATIENT, EMERGENCY CARE AND HOME HEALTH SERVICES FOR RESIDENTS OF NEWTON COUNTY AND THE SURROUNDING AREA. ADMITTING PHYSICIANS ARE PRIMARILY PRACTITIONERS IN THE LOCAL AREA. NEWTON MEDICAL CENTER (NMC) HAS AN OPEN MEDICAL STAFF WHERE QUALIFIED APPLICANTS ARE SCREENED THROUGH A STANDARD CREDENTIALS COMMITTEE PROCESS IN ORDER TO VERIFY APPROPRIATE EDUCATION AND TRAINING FOR THE PRIVILEGES THAT THEY SEEK. THE HOSPITAL IS GOVERNED BY A HEALTH SYSTEM BOARD OF DIRECTORS MADE UP OF MEMBERS FROM WITHIN THE COMMUNITY, INCLUDING MEMBERS OF THE HOSPITAL'S MEDICAL STAFF. NMC, AS A NOT-FOR-PROFIT, REINVESTS ANY EXCESS OR SURPLUS FUNDS TO PURSUE CAPITAL IMPROVEMENTS OR FUND THE FUNDED DEPRECIATION OF THE ORGANIZATION FOR FUTURE REPAIR, REPLACEMENT OR EXPANSION. THE HOSPITAL SPONSORS A FREE PHYSICIAN REFERRAL SERVICE, AS WELL AS SEVERAL AVENUES IN WHICH THE PUBLIC CAN ASK QUESTIONS ABOUT HEALTH SERVICES. THIS INCLUDES: FACEBOOK, TWITTER AND NUMEROUS FORMS LOCATED ON THE NMC WEBSITE. THE HOSPITAL RUNS SEVERAL ADS YEARLY TO PROMOTE SERVICES TO THE COMMUNITY. THE ORGANIZATION OFFERS SEVERAL FREE HEALTH SCREENINGS, AS WELL AS DISCOUNT PROGRAMS TO THE RESIDENTS OF THE COUNTY. NMC IS A PARTICIPANT IN MANY PUBLIC ACTIVITIES THROUGH PUBLIC AND PRIVATE ORGANIZATIONS, SUCH AS THE POLICE DEPARTMENT, PUBLIC SCHOOLS, BUSINESS HEALTH FAIRS AND HOSPITAL HEALTH FAIRS OPEN TO THE PUBLIC.
PART VI, LINE 7 - STATE FILING OF COMMUNITY BENEFIT REPORT GEORGIA
