OTHER PROGRAM SERVICES 4: Public Outreach - Includes responding to public information requests and social media outreach to inspire more to register as donors. OTHER PROGRAM SERVICES 5: Website & Registry Management - Includes the management of the DonateLifeNC.org website content and the online NC Donor Registry portal. The website had 105,899 visits in the fiscal year ended June 30, 2014, with 81,277 unique visitors. We had 5,538 total organ donor registrations during this period through the website with 3,434 online registrations and 2,104 paper form uploads from donor drives. OTHER PROGRAM SERVICES 6: Team NC - A team that participated in the 2014 Transplant Games of America in Houston, Texas. Donate Life NC served as their fiscal agent to manage revenue and reimburse team members for their travel expenses to Houston to compete in the games.
Form 990, Part VI, Line 4: Description of Significant Changes to Organizational Documents
The Bylaws were amended to outline the duties of the Board of Directors. The Board is responsible for the overall policy and direction of Donate Life NC and delegates day-to-day operations to the Executive Director. The specific powers of the Board of Directors include, but are not limited, to: [1] Defining the mission, goals and objectives of the organization and assigning priorities among the goals and objectives when needed; [2] Selecting the Executive Director and annually reviewing his/her performance; [3] Approving major personnel policies; [4] Reviewing and approving the organizations budget and associated financial statements; [5] Raising the financial resources required to meet Donate Life NCs goals and objectives, as coordinated by the Executive Director, and establishing general fund raising principles; [6] Conducting a biennial review and evaluation of Donate Life NCs performance of the goals and objectives of highest priority.
Form 990, Part VI, Line 6: Explanation of Classes of Members or Shareholder
Regular Membership status may be granted to organ procurement agencies, tissue banks, eye banks, and transplant centers that are chartered and operating in North Carolina. Associate Membership status may be granted to any organization with a common interest in public education about donation. Ad-hoc Membership status may be granted to individuals, organizations and governmental agencies and departments that have an interest in donation and advance care planning. Ad-hoc Members may participate in work groups and subcommittees and attend the Annual Meeting. Ad hoc members may serve on the Donate Life NC Board of Directors.

Prior to filing, the Form 990 will be reviewed by the Executive Director and Treasurer, and then distributed to the full Board for review and approval before filing.

The policy is monitored by the Board Secretary and reviewed and signed annually by all staff and board members with significant decision making authority. It is each signator's responsibility to disclose any potential conflict as soon as it is known, or reasonably should be known.
