Form 990, Part VI, Section B, Line 11b	The board of directors assign the responsibility of the timely and accurate filing of the 990 report to the management team of the credit union. The completed report is reviewed by the credit union CPA.
Form 990, Part VI, Section B, Line 12c	Credit union policies are reviewed annually by the board of directors.
Form 990, Part VI, Section B, Line 15	The credit union CEO is evaluated annually by the board of directors. Industry comparative resources are reviewed and approved and budget ranges are considered. The Vice President is evaluated annually by the CEO utilizing the same considerations above. All reviews are documented, signed and kept in the employee's personnel file.
Form 990, Part VI, Section C, Line 19	The credit union conducts an annual business meeting of its members to review and discuss the financial statements and any other business of the credit union. All credit union branches have this information available for the members throughout the year as well. All governing documents and procedures reside at the main office and are available for review upon member request.
