The community health needs assessment was conducted during FY 2013. The main model used was the Association for Community Health Improvement's (ACHI) Community Health Needs Assessment toolkit.The following qualitative data sets were studied: - Community Health Needs Assessment of Community Leaders - In some communities additional surveys included: Community Health Needs Assessment of Residents (Generalizable)The following quantitative data sets were studied: - 2011 County Health Profiles for Counties - Aging Profiles for Counties - Diversity Profiles for CountiesAsset mapping was conducted by reviewing the data and identifying the unmet needs from the various surveys and data sets. The process implemented in this work was based on the McKnight Foundation model - Mapping Community Capacity by John L McKnight and John P Kretzmann, Institute for Policy Research at Northwestern University.Each unmet need was researched to determine what resources were available in the community to address the needs. The Community Health Needs Assessment Collaborative performed the asset mapping and reviewed the findings. The group conducted an informal gap analysis to determine what needs remained after resources were thoroughly researched. Once gaps were determined the group proceeded to the prioritization process. The multi-voting methodology was implemented to determine what top priorities would be further developed into implementation strategies.

Sanford convened key health care leaders and other not-for-profit leaders in the Fargo Moorhead community to establish a Fargo Moorhead Community Health Needs Assessment Collaborative. The collaborative members included public health leaders, other community hospital organizations, and not-for-profit groups. A primary goal of this collaborative was to craft standardized tools, indicators and methodology that can be used by all group members when conducting assessments and also be used by all Sanford hospital facilities across the enterprise. After much discussion, it was determined that the Robert Wood Johnson Framework for county profiles would be the secondary data model.A subgroup of this collaborative met with researchers from the North Dakota State University Center for Social Research to develop a survey tool for our key stakeholder groups. The survey tool incorporated the University of North Dakota's Center for Rural Health community health needs assessment tool and the Fletcher Allen community health needs assessment tool. North Dakota State University and the University of North Dakota Center for Rural Health worked together to develop additional questions and to ensure that scientific methodology was incorporated in the design.The representatives of the communities assessed included city mayors, city council/commission members, physicians, nurses, school superintendents and school board members, parish nurses, representatives from the Native American community, Faith Community Leaders, as well as legal services, mentally and physically disabled, social services, non-profit organizations and financial services. The CHNA for all Sanford hospital facilities includes listings of the individuals who served on steering groups and those who provided input. Orientation and education is provided for staff members regarding cultural competency, expectations and the unique needs of a diverse population. Sanford also provides culturally consistent resources in the facilities (smudging ceremonies, native healers, etc.).Safety - Sanford is addressing this need by training employees regarding elderly abuse and neglect, patient safety and abuse reporting requirements. Parenting programs are available to new parents. Needs remain in the community and Sanford will share the concerns and the findings of the CHNA with community leadership.Prenatal Care - Over 20% of deliveries are to mothers who present to the medical center without having had prenatal care. Sanford will share these findings with public health agencies to develop a plan to engage patients in prenatal care early in pregnancy.

Sanford is committed to finding a cure for Type 1 Diabetes. A chronic disease self management program, Better Choices/Better Health is available free of charge to all patients with a chronic disease and can be accessed online at http://selfmanage.org/sanfordOutreach Specialty Care - Sanford is actively recruiting for additional providers and is making telemedicine available for neurology and dermatology.Keeping Young People in the Community - Sanford is addressing this need by sharing the results of the CHNA with community leaders.Cancer - Sanford is addressing this need by providing cancer screenings and digital mammography. The community has numerous options available. Sanford will share these concerns with public health.Economy - Although Sanford contributes substantially to the economic growth of the region and provides medical care to those who qualify for charity care through the Community Care Program and a discounted fee for those who qualify, there remain unmet needs beyond Sanford's scope. Sanford will share the survey results and unmet needs with community/city leaders.Elderly - Sanford supports the aging population in numerous ways. However there is a need for additional community services. Sanford will share these concerns with community leaders.Health Factors - Sanford does offer numerous programs to improve health factors. However there are needs in the community to reduce traffic deaths, teenage birth rates, etc. Sanford will share these concerns with community leadership.New Americans - Sanford has recently updated the policy so that children under 18 years of age are not able to translate for their parents. Sanford will share this concern with community leadership, social services and public health.Pollution/Environment - Sanford will share this information with community leaders.Safety - Sanford Tracy does not have the capacity to meet the safety needs of the community but will share these concerns with city leaders.Schools - Sanford will not directly address the school needs for the community but will share the concerns with community leaders and will communicate the concerns to Tracy school leadership.Traffic - Sanford will share these concerns with city leadership and will communicate the need for stop signs and speed bumps.Transportation - There is no transportation system available at this time. Residents must rely on volunteers to transport them out of the area. Sanford will not address this issue as it is out of Sanford' scope of work, however this information will be shared with city and county leadership.Youth - Sanford provides many services to meet the needs of youth. After much discussion, it was determined that the Robert Wood Johnson Framework for county profiles would be the secondary data model.A subgroup of this collaborative met with researchers from the North Dakota State University Center for Social Research to develop a survey tool for our key stakeholder groups. The survey tool incorporated the University of North Dakota's Center for Rural Health community health needs assessment tool and the Fletcher Allen community health needs assessment tool. North Dakota State University and the University of North Dakota Center for Rural Health worked together to develop additional questions and to ensure that scientific methodology was incorporated in the design.The representatives of the communities assessed included city mayors, city council/commission members, physicians, nurses, school superintendents and school board members, parish nurses, representatives from the Native American community, Faith Community Leaders, as well as legal services, mentally and physically disabled, social services, non-profit organizations and financial services.
