The Visiting Nurse Association of St. Luke's (VNA) is a member of the fully integrated St. Luke's University Health Network (Network). The VNA, established in 1919, has been a member of the Network since 1993. St. Luke's Home Health was awarded Home Care Elite designation four years in a row from 2011 to 2014. This recognition is awarded to home health organizations by OCS, a national home health benchmarking organization, and recognizes the top 25 percent of agencies based on performance metrics that include: quality of care, quality improvement, patient experience, process measure implementation and financial management. Home Health Compare publically reported data scores reveal that 85 percent of St. Luke's Home Health process and outcomes measures ranked at or above Pennsylvania and national scores in FY '14. The VNA annually provides free community outreach services and programs reaching more than 3,990 people. St. Luke's Home Health St. Luke's Home Health provides intermittent skilled nursing; physical, occupational and speech therapies; medical social work and home health aide services to home-bound patients. Services are provided under a plan of treatment established and approved by a physician. While all age groups are served, those 65 years of age or older comprise 65 percent of admissions. 67 percent of all patients admitted are referred to the VNA from a hospital. In FY '14, the VNA provided 15.5 visits on average to 5,886 individuals. Home Health accepts Medicare and other third-party payers. Patients without insurance may qualify for services on a sliding scale under the St. Luke's University Health Network Charity Care Policy. St. Luke's Hospice St. Luke's Hospice provides patients of any age, and their families, compassionate end-of-life care within their homes and at various long-term care facilities, nursing homes and assisted-living communities. In addition, St. Luke's Hospice operates an inpatient Hospice House. The focus is on care, not cure; and on quality and value of life, not the length of life. In FY '14, the VNA provided end-of-life care to 1,539 patients; including 507 patients at Hospice House where costs exceeded financial reimbursement levels by more than $1.58 million (excluding charitable contributions and earnings from endowment). St. Luke's Nurse-Family Partnership St. Luke's Nurse-Family Partnership is an evidence-based, nurse home visiting program to improve the health, well-being and economic self-sufficiency of low-income, first-time parents and their children. Care is provided in this voluntary prevention program by specially trained registered nurses beginning early in the mother's pregnancy and continuing until her child's second birthday. The Nurse-Family Partnership served 327 families residing in the Lehigh Valley (encompassing the cities of Allentown, Bethlehem and Easton and the surrounding rural areas) during FY '14. Parent Advocate in the Home (PATH) PATH provides health and supportive services to families with children age 3 years or younger. In FY '14, PATH provided 1,739 visits to 194 unduplicated patients. A visiting nurse assists families understand child growth and development, home safety, discipline, healthy eating, problem solving and parenting. This program focuses on early child development, nutrition, health and preparing the families and their children to be ready for school. Visiting Nurse Advocate For the County (VNAC) The Visiting Nurse Advocate for the county provides child health monitoring and child advocacy services to children living in troubled homes in Northampton and Lehigh counties, in Southeastern Pennsylvania. In FY '14, 8,299 hours of service were provided to 151 families.
