Program Service Accomplishments Housing Coordinators/Support Workers provided 4,786 hours of service to 265 households with severe and persistent mental illness as listed below: 1) Housed 90 persons (77 households) in Supportive Housing (including Permanent Supportive Housing) in Extended Housing owned units, via properties located throughout Lake County (does not include persons housed in two group homes and one transitional group home but does include McNaughton and McKinley Grove Apartments). - Member of Painesville Crime Free Housing & Northeast Ohio Apartment Association 2) Housed 118 households via rental subsidies/vouchers to obtain or maintain housing throughout Lake County (including Permanent Supportive Housing). - HUD's Shelter + Care provided rental vouchers to 61 homeless and disabled households. Mental Health services provided from Beacon Health and Signature Health. - Lake County ADAMHS Board funded rental assistance to 45 households (part of Rental Subsidy Program) - Housed 12 Veteran households through HOME funding from the Lake County Board of Commissioners. Funds are targeted for veterans (Part of Rental Subsidy Program). 3) Provided 50 households with "no interest" loans. This loan program is for one-time rental assistance or security deposits to help persons/families obtain or maintain housing. - 4 Housing Loans were paid off. 4) number of households successfully transferring off of Extended Housing assistance: - 7 households were able to gain enough income to no longer need a Housing Subsidy. - 2 households transferred into another housing program within the Lake County continuum. - 5 households moved out of the County or State Homeless Outreach Workers (PATH Program) provided aggressive outreach to 298 homeless persons in Lake County. Of these: 1) 112 homeless persons were linked with mental health services and resources in the community, such as case management with other mental health agencies. - 80 engaged in mental health services - 30 households accessed housing and were no longer homeless - The Homeless Advisory Council donated $300 in utility deposits to help house 3 homeless persons and bought $500 worth of Laketran tickets for homeless persons working with the PATH program. Extended Housing owns & manages 20 properties throughout Lake County including: - Willoughby Place and Madison Place - Two group homes operated by Northcoast Behavioral Healthcare. - North Coast House-transitional residential care operated by Beacon Health, Inc. - McKinley Grove - permanent supportive housing with services on site. - The Extended Housing Wellness Center: Mission is to seek to provide skills and services from a variety of non-clinical perspectives that assist and reinforce in the Empowerments and Wellness of priority populations. In doing so, the Wellness Center reflects the best of a community that assists the development of these individuals. - BRIDGES Mental Health Consumer Empowerment Services began providing services in February 2014. - Beginning July of 2014, Extended Housing began providing residential care via North Coast House. Volunteer Projects: Extended Housing's properties benefited from over 300 Hours of service, primarily through businesses volunteering through Lake County's United Way Day of Caring and projects at the Extended Housing Wellness Center (St. Gabriel and downtown Painesville Organization). Extended Housing also received help from the Mentor High School Football Team and the Perry Wrestling Team.
