PART I, LINE 1B THE CLARK JOINTLY OWNS A RESIDENCE WITH THE DIRECTOR. IN ACCORDANCE WITH AN AGREEMENT DATED NOVEMBER 1, 1994, AND UPDATED DECEMBER 1, 2001, THE DIRECTOR AND THE CLARK SHARE ON A 50/50 BASIS THE COST OF MAINTAINING AND CARRYING THE PROPERTY, AS WELL AS AGREED UPON CAPITAL IMPROVEMENTS TO THE PROPERTY. ON THE RARE OCCASION WHEN THE DIRECTOR'S BUSINESS TRAVELS REQUIRE HIS SPOUSE TO ACCOMPANY HIM, THE CLARK WILL PAY FOR THE COST OF TRAVEL. THE CLARK PAYS THE ANNUAL DUES ON BEHALF OF TWO EMPLOYEES, INCLUDING THE DIRECTOR, REIMBURSING THEM FOR THE COST OF MEMBERSHIP IN THEIR RESPECTIVE ALUMNI CLUBS.
PART I, LINE 4B THE DIRECTOR PARTICIPATES IN A SUPPLEMENTAL NON-QUALIFIED RETIREMENT PLAN (SECTION 457B PLAN). DURING CALENDAR YEAR 2013, THE CLARK CONTRIBUTED $12,129 TO THE PLAN. THIS AMOUNT IS INCLUDED ON FORM 990 PART VII, COLUMN (F) AND ON SCHEDULE J, PART II, COLUMN B(III). IN JANUARY 2009, THE CLARK ESTABLISHED A SUPPLEMENTAL EXECUTIVE RETIREMENT PLAN FOR THE BENEFIT OF ITS DIRECTOR. THIS PROGRAM IS FUNDED BY THE CASH SURRENDER VALUE OF A PREVIOUSLY ISSUED LIFE INSURANCE POLICY ALONG WITH ON-GOING CONTRIBUTIONS.
