A CONFLICT OF INTEREST DISCLOSURE FORM ASKS FOR A LIST OF OTHER COMPANIES/ ORGANIZATIONS OF WHICH THE BOARD OR EXECUTIVE TEAM MEMBER IS AN OFFICER OR BOARD MEMBER AND IS FURNISHED ANNUALLY TO EACH DIRECTOR, OFFICER, AND MEMBER OF THE EXECUTIVE STAFF OF THE ORGANIZATION. Form 990, Part VI, Line 15b: Compensation Review and Approval Process for Officers and Key Employees
IT IS THE POLICY OF LACHC TO REVIEW ALL SALARIES ANNUALLY. THE ANNUAL CPCA SURVEY PROVIDES SALARY & BENEFITS FOR OVER 100 TYPICAL COMMUNITY CLINIC POSITIONS, INCLUDING THE CEO, CFO & CHIEF MEDICAL OFFICER.
