Finally, UVM Medical Center provided a community wide survey - with online responses and paper responses - to receive input from the community as a whole. FINANCIAL COUNSELORS ACTIVELY EDUCATE ALL INPATIENT, OBSERVATION AND INVASIVE SERVICE OUTPATIENTS OF OUR PROGRAM, PRIOR TO OR CONCURRENT WITH THE PATIENTS STAY, SUBSEQUENTLY AIDING IN THE APPLICATION PROCESS FOR STATE AID AND UVM MEDICAL CENTER'S FINANCIAL ASSISTANCE POLICY. PATIENTS ARE CONSISTENTLY NOTIFIED/EDUCATED ON THE UVM MEDICAL CENTER FINANCIAL ASSISTANCE POLICY BOTH IN WRITTEN AND VERBAL FORMATS. AT THE TIME OF SERVICE, A PLAIN LANGUAGE SUMMARY OF THE POLICY IS PROVIDED, ALONG WITH THE FULL POLICY AND APPLICATION UPON REQUEST. INHOUSE PATIENTS ARE FURTHER EDUCATED AND PROVIDED ASSISTANCE IN THE APPLICATION PROCESS AT THE BEDSIDE BY FINANCIAL ADVOCATES WHO FOLLOW THE PATIENT UNTIL DISCHARGE AND/OR THE APPLICATION IS COMPLETED. SCHEDULE H, PART V, LINE 20D THE PATIENT ASSISTANCE PROGRAM (CHARITY) HAS BOTH AN INCOME AND ASSETS BASED TEST UP TO 400% OF THE FEDERAL POVERTY GUIDELINES (FPG); ALONG WITH AN APPEALS PROGRAM FOR PATIENTS WHO TEST HIGHER THAN 400% OF THE FPG. BASED UPON THE PATIENT'S FPG LEVEL, A PATIENT MAY BE GRANTED FULL OR PARTIAL ASSISTANCE AT THE TIME OF APPLICATION APPROVAL. FULL ASSISTANCE EQUALS A 100% WRITE-OFF OF CHARGES FOR A 6 MONTH TIMEFRAME. PARTIAL ASSISTANCE EQUALS A SCALED DISCOUNT OFF CHARGES OR BALANCE BASED UPON THE AVERAGE AMOUNT GENERALLY BILLED TO INSURED PATIENTS (AGB). THE DISCOUNT % IS APPLIED TO THE BALANCE OF EACH ENCOUNTER FOR UP TO A 6 MONTH WINDOW OF SERVICES PROVIDED. BETWEEN 55% AND 85% DISCOUNTS ARE PROVIDED BASED UPON THE PATIENT'S FPG LEVEL. AT THE CLOSE OF THE 6 MONTH GRANT, PATIENTS MUST APPLY FOR ASSISTANCE IF ONGOING NEED OR HARDSHIP REMAINS. THE AVERAGE GENERALLY BILLED IS A HOSPITAL AND PHYSICIAN BLENDED CALCULATION ON THE PREVIOUS YEAR'S FULLY ADJUDICATED COLLECTION RATES FOR MEDICARE, MEDICARE ADVANTAGE, AND COMMERCIAL INSURANCES. THE COLLECTION RATE IS CALCULATED USING TOTAL PAYMENTS DIVIDED BY GROSS CHARGES. FOR UVMMG, THE AGB CALCULATED TO 45% AND RESULTED IN A MINIMUM CHARITY GRANT OF 55%.
