| FORM 990 PART I, LINE 1 | THE GOAL OF FOUNTAIN GATE LIFE AND WELLNESS IS TO PROMOTE PHYSICAL, EMOTIONAL, AND SPIRITUAL HEALTH AND WHOLENESS IN OUR COMMUNITY. THE PRIMARY MEANS BY WHICH WE ACCOMPLISH OUR MISSION ARE AS FOLLOWS: 1. COUNSELING. WE PROVIDE COUNSELING, MARRIAGE AND FAMILY THERAPY, AND SOCIAL WORK SERVICES USING STATE-LICENSED, MASTERS-LEVEL PROFESSIONALS. THESE SERVICES ARE PROVIDED TO INDIVIDUALS (ADULTS, ADOLESCENTS, AND CHILDREN), COUPLES, AND FAMILIES, AND COVER A WIDE RANGE OF ISSUES AND PATHOLOGIES, SUCH AS DEPRESSION, GRIEF AND LOSS, TRAUMA AND ABUSE (PHYSICAL, EMOTIONAL, AND SEXUAL), ANXIETY AND STRESS, IDENTITY ISSUES, CAREER AND LIFE TRANSITIONS, RELATIONSHIP CONFLICTS, PREMARITAL COUNSELING, MARRIAGE AND FAMILY, BLENDED FAMILIES, CHILD RAISING, MEN'S AND WOMEN'S ISSUES, AND SPIRITUAL CONCERNS. 2. GROUPS. WE PROVIDE THERAPY GROUPS AS A WAY FOR INDIVIDUALS, COUPLES, AND FAMILIES TO MEET WITH OTHERS WHO ARE EXPERIENCING SIMILAR PROBLEMS, AND TO LEARN NEW WAYS OF COPING AND RELATING UNDER THE GUIDANCE OF OUR PROFESSIONAL STAFF. GROUPS ARE OFFERED ON A PERIODIC BASIS AND FOCUS ON A VARIETY OF THEMES SUCH AS PARENTING, MANAGING ANXIETY, SETTING BOUNDARIES IN INTERPERSONAL RELATIONSHIPS, AND WORKING THROUGH GRIEF AND LOSS. 3. CREATIVE WORKSHOPS. THESE SMALL-GROUP EXPERIENCES ENGAGE THE PARTICIPANTS IN A CREATIVE PROCESS THAT CAN BRING INSIGHT AND PERSPECTIVE TO PERSONS WHO MAY BE STRUGGLING WITH A PARTICULAR PROBLEM. WORKSHOPS TYPICALLY INCLUDE CREATIVE ACTIVITIES (STRUCTURED AND UNSTRUCTURED), REFLECTION TIME, AND GROUP DISCUSSION. CREATIVE ACTIVITIES MAY INCLUDE MEDIA SUCH AS VISUAL ART, CLAY, PHOTOGRAPHY, MOVEMENT, MUSIC, AND WRITING EXERCISES. 4. FARM AND GARDEN. IN 2014 FOUNTAIN GATE CONTINUED GATHERING COMMUNITY SUPPORT FOR A PROPOSED 2.5 ACRE COMMUNITY FARM AND GARDEN PROJECT THAT WILL INCLUDE INDIVIDUAL GARDEN PLOTS, LARGER FARM PLOTS FOR RAISING FOOD FOR CHARITIES, CONTEMPLATIVE GARDENS, WALKWAYS, AND OTHER EDUCATIONAL, THERAPEUTIC AND RECREATIONAL AREAS. STAKEHOLDERS INCLUDE THE CITY OF KENNESAW AND THE MASTER GARDENER VOLUNTEERS OF COBB COUNTY, AS WELL A GROWING BASE OF ENTHUSIASTIC MEMBERS OF THE GENERAL COMMUNITY. IN 2014, FOUNTAIN GATE CONTINUED TO OPERATE A PROTOTYPE VERSION OF THE PROJECT(FOUNTAIN GATE GARDENS) WHICH INCLUDED A 26 BED COMMUNITY GARDEN AND A 5,000 SQ FT "PLANT A ROW FOR THE HUNGRY" CHARITABLE GARDEN. FOUNTAIN GATE'S COUNSELING SERVICES ARE PROVIDED ON A SLIDING SCALE OF HOURLY FEES THAT ARE BASED ON HOUSEHOLD INCOME AND FAMILY SIZE. THE SCALE GOES AS LOW AS $10; TO-DATE 62% OF ALL SESSIONS HAVE BEEN BELOW $40. WHEN SPEAKING WITH POTENTIAL CLIENTS WE NEGOTIATE THEIR FEE BASED ON THEIR CIRCUMSTANCES, USING THE SLIDING SCALE TO DETERMINE THE MINIMUM ALLOWABLE FEE FOR A GIVEN CLIENT. BASED ON THE ADVICE OF NUMEROUS PEOPLE FROM THE PROFESSIONAL AND ACADEMIC REALMS WE DO NOT OFFER "FREE COUNSELING" BECAUSE OF ITS NEGATIVE THERAPEUTIC VALUE: CHARGING A MINIMAL FEE ($10) HELPS CLIENTS ENGAGE AND VALUE THE COUNSELING PROCESS AS WELL AS MAINTAIN A COMMITMENT TO CONSISTENT ATTENDANCE. NOTE THAT TYPICAL FEES FOR THE TYPES OF SERVICES WE OFFER RANGE FROM $75-$125 IN OUR COUNTY, AND UP TO $200 IN NEIGHBORING COUNTIES. |
