| Form 990, Part III, Line 4d: Other Program Services Description | OTHER PROGRAM SERVICES 4: Behavior Modification: This program is specially designed for clients with higher level behavioral challenges. The goal is to increase the individuals level of independence, and decrease their level of negative behaviors, while they are involved in work and other meaningful activities. The program requires a 1:3 ratio. Once a client is able to successfully meet the goals identified in his or her Behavior/Individual Service Plan, they are encouraged to transition to one of our less restrictive programs. OTHER PROGRAM SERVICES 5: The Work Activity Program focuses on small-group and individual employment which allows clients to meet their career goals and also receive a steady paycheck. Clients participate in paid group work sites where they learn and hone skills. With additional coaching, selected adults are supported during a one-year to one-and-a-half year timeframe to move on to individual job placements in the community. The result is increased independence and self-reliance. OTHER PROGRAM SERVICES 6: The Senior Program is open to those who are 50+ years and is programmatically tailored to meet the needs of older adults. The focus is on an environment filled with the discovery of recreational alternatives, the improvement of individual's leisure skills, and the enhancement of the overall retirement experience for our senior population. Senior Program participants are encouraged to become involved in our many site-based programs, such as art, horticulture, computers and swimming. Seniors are also offered the option of continuing to earn a paycheck in our employment-related programs. OTHER PROGRAM SERVICES 7: The SMSC TRANSITION PROGRAM offers a nonpublic school transition program for students 17 - 22 years old as they are aging-out of a lifetime of a public education system and structure. The program offers young adults with developmental disabilities, who may also have other physical challenges, a wide range of intensive, individualized interventions designed to provide needed assistance in areas of academic, social and vocational training that will allow a smooth transition to adulthood. |
