MESA COUNTY HEALTH DEPARTMENT STARTED THE CHNA PROCESS DECIDED ON FOUR CRUCIAL STEPS TO ASSESS NEEDS, DETERMINE CAUSES, AND REACH CONSENSUS ON POSSIBLE INTERVENTIONS TO ACHIEVE DESIRED OUTCOMES. THE FOLLOWING FIVE PREDISPOSING, REINFORCING AND ENABLING FACTORS WERE IDENTIFIED AS HAVING A POTENTIAL IMPACT ON HEALTH OUTCOMES IN MESA COUNTY AND WILL BECOME THE EMPHASIS OF FUTURE PUBLIC HEALTH IMPROVEMENT EFFORTS: - PARENTING, SOCIAL & EMOTIONAL WELLBEING, ACCESS TO HEALTHCARE, BUILT ENVIRONMENT AND BUILDING A SENSE OF COMMUNITY.PRIORITY AREAS WERE SELECTED BY ANALYZING THE ROOT CAUSES OF MORBIDITIES AND MORTALITIES IN MESA COUNTY. THE MESA COUNTY HEALTH DEPARTMENT UNDERSTOOD THAT IN ORDER TO IMPACT HEALTH OUTCOMES, HEALTH OFFICIALS MUST LOOK BEYOND PROGRAMS, POLICIES, AND EVEN INDIVIDUAL BEHAVIORS AND FOCUS ON ADDRESSING THE CONTRIBUTING FACTORS AND DETERMINANTS OF POOR HEALTH. THE PRIORITY AREAS, RECOMMENDATIONS, AND STRATEGIES FOR IMPLEMENTATION RECOGNIZE THE INTERCONNECTEDNESS OF INDIVIDUAL BEHAVIORS AND THE SYSTEMS AND ENVIRONMENTS IN WHICH PEOPLE GROW AND LEARN. SM SELECTED THE FOLLOWING CONTRIBUTING HEALTH FACTORS OF MENTAL HEALTH, OBESITY AND TOBACCO CESSATION.MENTAL HEALTH - MESA COUNTY REPORTS THAT ADULTS WHO REPORT THEIR HEALTH WAS NOT GOOD IS ABOVE THE STATE AVERAGE AT 3.9 DAYS VERSUS 3.1 DAYS. THIS INDICATOR LINKS PHYSICAL AND BEHAVIORAL UNHEALTHY WHICH INTERFERES WITH OVERALL HEALTH. WORSE YET IS MESA COUNTY'S RATE OF SUICIDE: 34.4 DEATHS PER 100,000 PERSONS VERSUS THE STATE AVERAGE IF 17.4 PERSONS. SUICIDE IS A SERIOUS PUBLIC HEALTH PROBLEM THAT CAN HAVE LASTING HARMFUL EFFECTS ON INDIVIDUALS, FAMILIES, AND COMMUNITIES. WHILE ITS CAUSES ARE COMPLEX AND DETERMINED BY MULTIPLE FACTORS, THE GOAL OF SUICIDE PREVENTION IS SIMPLE: REDUCE FACTORS THAT INCREASE RISK (I.E. RISK FACTORS) AND INCREASE FACTORS THAT PROMOTE RESILIENCE (I.E. PROTECTIVE FACTORS). IDEALLY, PREVENTION ADDRESSES ALL LEVELS OF INFLUENCE: INDIVIDUAL, RELATIONSHIP, COMMUNITY, AND SOCIETAL. SM HAS BEEN A STRONG SUPPORTER OF ADDRESSING MENTAL HEALTH IN MESA COUNTY AND WORKS WITH MIND SPRINGS HEALTH, THE LOCAL BEHAVIORAL HEALTH ORGANIZATION AS A REFERRAL SOURCE FOR ITS MANY COMMUNITY-BASED PROGRAMS, SERVICES AND TRAININGS. FOR THOSE USING SM, THE HOSPITAL FUNDED AN INPATIENT MENTAL HEALTH SERVICE AND NOW SUPPORTS MIND SPRINGS HEALTH, THE LOCAL BEHAVIORAL HEALTH ORGANIZATION. MIND SPRINGS HEALTH HAS FULL-TIME THERAPISTS AT ST. MARY'S FAMILY PRACTICE AND AT THE MARILLAC CLINIC WHICH ARE ORGANIZATIONS FINANCIALLY SUPPORTED BY SM. THESE THERAPISTS ARE TRAINED IN INTEGRATED CARE MODELS OF TREATMENT AND PROVIDE BRIEF, SOLUTION-FOCUSED BEHAVIORAL INTERVENTION TO PATIENTS. FURTHER, THERAPISTS ARE READILY ABLE TO EASILY CONSULT WITH MEDICAL PROVIDERS REGARDING MENTAL/BEHAVIORAL HEALTH ISSUES. THE HOSPITAL ALSO EMPLOYS PSYCHIATRIC PHYSICIANS FOR BOTH ADULTS AND CHILDREN WHICH HELPS INCREASE COMMUNITY ACCESS TO THIS IMPORTANT SERVICE. IN 2014, SM PROVIDED FINANCIAL SUPPORT TO THE SUICIDE PREVENTION FOUNDATION, MESA COUNTY COUNSELING AND EDUCATION CENTER, AND WESTERN COLORADO SUICIDE PREVENTION. THESE ORGANIZATIONS PROVIDE FREE OR LOW COST BEHAVIORAL HEALTH SERVICES TO THE COMMUNITY AND INCREASE ACCESS TO THOSE WITHOUT COVERAGE FOR THE SERVICE. OBESITY - THE RATE OF OBESITY IS RISING IN MESA COUNTY. IN 2014, 22% OF ADULTS WERE CONSIDERED OBESE VERSUS THE STATE RATE OF 20% (COUNTY HEALTH RANKINGS, 2014). AS A PARTNER WITH HEALTH MESA COUNTY, OBESITY IS BEING ADDRESSED AS A PROMOTION OF HEALTH THROUGH WELL-CONSTRUCTED AND INCLUSIVE NEIGHBORHOODS, TRAILS, SIDEWALKS AND OPEN SPACES. HEALTHY MESA COUNTY HAS A COMMUNITY-LEVEL WEBSITE WHERE CITIZENS CAN FIND ACTIVITIES AND RESOURCES SUCH AS BIKING TRAIL MAPS THAT SUPPORT INCREASED PHYSICAL ACTIVITY. TOBACCO CESSATIONTHE RATE OF TOBACCO USE IN MESA COUNTY IS 20% AS COMPARED TO THE STATE RATE OF 17% (COUNTY HEALTH RANKINGS, 2014). IN PARTNERSHIP WITH MESA COUNTY HEALTH DEPARTMENT AND THE SCL HEALTH SYSTEM, A HOSPITAL LEVEL APPROACH HAS BEEN UNTAKEN AS FIRST STEPS. THIS INCLUDES UPDATING THE HOSPITAL TOBACCO USE POLICY, ASSURING THERE IS DESIGNATED SMOKING AREAS ON THE HOSPITAL CAMPUS AWAY FROM PUBLIC ACCESS. AS A HOSPITAL THAT SERVES ITS COMMUNITY, SEVERAL NEW INTERVENTIONS WILL ASSURE PATIENTS WHO SMOKE ARE SCREENED AND PROVIDED APPROPRIATE INTERVENTIONS TO QUIT. ELECTRONIC REFERRAL TO THE QUIT LINE--SM AND SCL HEALTH ARE WORKING IN PARTNERSHIP WITH THE QUIT LINE TO ESTABLISH ELECTRONIC REFERRALS FOR PATIENTS READY TO QUIT. RATES OF PATIENT USE--SM IS WORKING WITH SYSTEM HOSPITALS TO OBTAIN REGULAR REPORTS ON RATES OF PATIENT SMOKERS, THOSE REFERRED AND THOSE WHO HAVE COMPLETED THE QUIT LINE INTERVENTION.ESTABLISHMENT OF AN INTERNAL TOBACCO TEAM-SM HAS ASSEMBLED A TEAM OVERSEEING TOBACCO CESSATION MADE UP OF SENIOR LEADERSHIP, HEALTHCARE PROFESSIONALS AND OTHER ADMINISTRATORS. PART III, LINE 9B: AN INTEGRAL COMPONENT OF OUR MISSION IS TO BE GOOD FINANCIAL STEWARDS. THIS REQUIRES US TO DETERMINE WHICH PATIENTS ARE IN NEED OF CHARITY CARE AND WHICH ARE ABLE TO CONTRIBUTE SOME PAYMENT FOR CARE RECEIVED. IN ADDITION, THE PATIENTS RECEIVE INFORMATION ABOUT THE FINANCIAL ASSISTANCE POLICY WITH THEIR PATIENT BILLS. PATIENTS ARE CONTACTED MULTIPLE TIMES ABOUT UNPAID BALANCES PRIOR TO INITIATING ANY COLLECTION ACTION. THE FINANCIAL ASSITANCE POLICY AND APPLICATION ARE POSTED ON THE HOSPITAL'S WEBSITE. THE POLICYY AND APLLICATION ARE ALSO AVAILBLE UPON REQUEST. THE ORGANIZATION HAS A FINANCIAL ASSISTANCE PROGRAM THAT PROVIDES PATIENTS OPPORTUNITIES TO APPLY FOR FREE OR DISCOUNTED CARE OR TO BE ENROLLED IN A GOVERNMENT SPONSORED MEDICAL CARE PROGRAM. THE HOSPITAL IS LOCATED IN GRAND JUNCTION, CO THE LARGEST CITY BETWEEN SALT LAKE CITY, UTAH AND DENVER, COLORADO; THEREFORE, IT SERVES AS A REGIONAL HUB FOR SERVICES AND MEDICAL TECHNOLOGIES. MESA COUNTY LIES ON THE WESTERN BORDER OF COLORADO AND COVERS 3,309 SQUARE MILES. ACCORDING TO THE COLORADO STATE DEMOGRAPHY OFFICE THE ESTIMATED 2014 POPULATION OF MESA COUNTY IS 148,255 (US CENSUS BUREAU 2014 ESTIMATE). MESA COUNTY HAS EXPERIENCED STEADY POPULATION GROWTH RATES AND IS EXPECTING THIS TREND TO CONTINUE. HISTORICALLY, THE AREA HAS GROWN AT 2%, ANNUALLY, AND IS PROJECTED TO CONTINUE TO GROW AT 2.3%, ANNUALLY, THROUGH 2018. THESE PROJECTIONS WERE PROVIDED PRIOR TO AND DURING THE ECONOMIC DOWNTURN. THESE FEDERAL PROGRAMS ARE FINANCIALLY SUPPORTED BY ST. MARY'S. ORIGINALLY SPONSORED BY MESA COUNTY, ST. MARY'S ASSUMED SPONSORSHIP AND SUPPORT OF THESE PROGRAMS DURING THE ECONOMIC DOWNTURN OF THE 1980'S WHEN THE COUNTY SUFFERED SEVERE BUDGET CONSTRAINTS FROM THE LOSS OF ENERGY-RELATED REVENUES. ST. MARY'S HAS CONTINUED TO SPONSOR AND SUPPORT THESE PROGRAMS BECAUSE OF THE UNIQUE SERVICES THEY PROVIDE TO THE SENIORS IN NEED IN MESA COUNTY. ST. MARY'S SENIOR COMPANION PROGRAM HELPS OLDER ADULTS REMAIN HEALTHY, INDEPENDENT, AND IN THEIR OWN HOMES. THE NEED FOR THIS PROGRAM GROWS OUT OF THE PROBLEMS FACED BY SENIORS WHEN THEY OR THEIR PARTNER DEVELOPS MOBILITY ISSUES AND NEED HELP WITH EVERYDAY TASKS. VOLUNTEER COMPANIONS SERVED NEARLY 30,000 HOURS HELPING 350 SENIORS WITH DAILY ACTIVITIES, SUCH AS GROCERY SHOPPING, BILL PAYING, AND TRANSPORTATION TO MEDICAL APPOINTMENTS. WITHOUT HELP OF SENIOR COMPANIONS, MANY OLDER ADULTS WOULD BE FORCED TO LEAVE THEIR HOMES FOR ASSISTED LIVING FACILITIES OR NURSING HOMES. MANY WOULD NEED TO RELY ON PUBLIC SUPPORT SUCH AS MEDICAID TO PAY FOR THESE SERVICES. BY PROVIDING LOW-COST, PERSONAL INTERACTION WITH AT-RISK SENIORS, THE PROGRAM HELPS ENSURE SENIORS ARE ABLE TO GET GROCERY SHOPPING, MEDICAL APPOINTMENT AND ERRANDS COMPLETED. THE PROGRAM BENEFITS MORE THAN ST. MARY'S BECAUSE THE GOAL OF THE PROGRAM IS TO KEEPSENIORS LIVING INDEPENDENTLY OUTSIDE OF ASSISTED LIVING AND TO REDUCE THE RISK OF ILLNESS OR ACCIDENT THAT WOULD REQUIRE A HOSPITAL STAY. IN ADDITION TO MAJOR SUPPORT PROVIDED BY ST. MARY'S, THE COMMUNITY PROVIDES ADDITIONAL FINANCIAL SUPPORT THROUGH CONTRIBUTIONS TO ENHANCE THE SERVICES PROVIDED.ST. MARY'S FOSTER GRANDPARENT PROGRAM PROVIDES TWO TYPES OF SUPPORT IN THE COMMUNITY: 1) ASSISTANCE FOR AT-RISK YOUTH, AND 2) A SOURCE OF INCOME FOR LOW-INCOME SENIORS. QUALIFIED SENIOR VOLUNTEERS RECEIVE A TAX-FREE HOURLY STIPEND. PROVIDING NEARLY 60,000 HOURS OF INVALUABLE TUTORING, MENTORING AND ONE-ON-ONE EDUCATIONAL SUPPORT FOR 1,500+ AT-RISK CHILDREN, THE PROGRAM WORKS IN COLLABORATION WITH A BROAD RANGE OF COMMUNITY AGENCIES INCLUDING: MESA COUNTY SCHOOL DISTRICT; DELTA COUNTY SCHOOL DISTRICT; ROCKY MOUNTAIN HEAD START, MIGRANT HEAD START, KIDS OF THE KINGDOM, CHILD QUEST, LIL'MAVERICKS, AND COLLBRAN JOB CORPS, JUST TO NAME A FEW. WITHOUT THE PROGRAM, MORE YOUTH WOULD LIKELY DROP OUT OF SCHOOL AND REQUIRE PUBLICLY SUPPORTED INTERVENTION, AND SENIORS WOULD RELY MORE HEAVILY ON PUBLICLY AND PRIVATELY SUPPORTED FINANCIAL ASSISTANCE PROGRAMS. THE PRIMARY BENEFICIARIES OF THIS PROGRAM ARE THE PARTICIPATING SENIORS AND STUDENTS AND COMMUNITY AND GOVERNMENT-SUPPORT PROGRAMS, AS THE BENEFITS ACHIEVED ARE OUTSIDE THE REALM OF ST. MARY'S HOSPITAL'S SERVICES. IN ADDITION TO MAJOR SUPPORT PROVIDED BY ST. MARY'S, THE COMMUNITY PROVIDES ADDITIONAL FINANCIAL SUPPORT THROUGH CONTRIBUTIONS TO ENHANCE THE SERVICES PROVIDED.GRAY GOURMET IS A NUTRITIONAL MEAL PROGRAM FOR SENIORS SPONSORED BY ST. MARY'S HOSPITAL. WITH NINE COMMUNITY DINING SITES, HOME-DELIVERED MEALS THROUGH 18 DELIVERY ROUTES, AND OTHER NUTRITION-RELATED SERVICES, GRAY GOURMET IS DEDICATED TO THE GOOD HEALTH OF MESA COUNTY SENIORS. MEALS ARE PREPARED FRESH DAILY. SENIORS CURRENTLY RECEIVE HOT, NUTRIENT-DENSE MEALS IN FRIENDLY COMPANY AT NINE MEAL SITES. HOMEBOUND SENIORS 60 YEARS OF AGE AND OLDER CAN ARRANGE FOR HOME-DELIVERY SERVICE. FOR WEEKENDS, FROZEN MEALS ARE AVAILABLE FOR THOSE ENROLLED IN THE HOME DELIVERY SERVICE. IN 2013, NEARLY 110,000 MEALS WERE SERVED. GRAY GOURMET'S REGISTERED DIETITIAN ASSURES MEALS MEET RECOMMENDED DIETARY STANDARDS. PERSONALIZED DIETARY COUNSELING IS OFFERED TO ALL PARTICIPANTS, AND NUTRITION EDUCATION IS AVAILABLE TO GROUPS. BOTH ARE FREE TO PARTICIPANTS. WITHOUT THE NUTRITIONAL SUPPORT PROVIDED BY GRAY GOURMET, MORE SENIORS WOULD BE PLACED IN ASSISTED LIVING FACILITIES, RELYING ON PUBLIC SUPPORT SUCH AS MEDICAID TO PAY FOR THESE SERVICES. THESE PLANS ENABLE THE EXECUTIVE TO EARN BENEFITS DURING EACH YEAR THAT THEY PARTICIPATE. PART I, LINE 7 OTHER NON-FIXED PAYMENTS THE AT RISK COMPENSATION PLAN WAS ESTABLISHED TO ENABLE THE HEALTH CARE SYSTEM AND ITS CARE SITES TO ATTRACT AND ENGAGE QUALIFIED LEADERS AND TO PROVIDE SUCH LEADERS WITH AN ADDITIONAL PERFORMANCE COMPENSATION OPPORTUNITY TO PROMOTE AND FURTHER ITS CHARITABLE MISSION, VISION, STRATEGIC PRIORITIES AND KEY INITIATIVES. THE PLAN OPERATES ON A CALENDAR-YEAR BASIS AND IS FUNDED EACH YEAR BY MEETING THRESHOLD LEVELS OF OPERATING INCOME.
