Form 990, Part VI, Line 11 FORM 990 REVIEW PROCESS - The annual filing of Federal Form 990 is reviewed by the finance committe and presented to the board of directors annually.
Form 990, Part VI, Line 12C EXPLANATION OF MONITORING AND ENFORCEMENT OF CONFLICTS - The Code of Ethics and Related Certificate of Acknowledgement is annually distributed to the board of directors and all committee members for review and signature. An executive assistant receives and tracks the signed Certificate.
Form 990, Part VI, Line 15B COMPENSATION REVIEW AND APPROVAL FOR OFFICERS AND KEY EMPLOYEES - At the current time, the Foundation has no employees.
