Form 990, Part III, Line 4d: Other Program Services Description
OTHER PROGRAM SERVICES 4: SAGE Program: An original grant, in 2009, from the Donald W. Reynolds Foundations launched the Seniors Assisting in Geriatric Education (SAGE) mentoring program. The mission of the SAGE program is to strengthen health professions students medical education in the development of competency in attitude, knowledge and skills in the care of older adults. These students are partnered with a senior mentor/client that receives either home delivered meals through the Meals on Wheels Program, Inc. of Tarrant County or residents of retirement communities like The Stayton. Over the course of a students education, they gain competency with older adults by meeting with them in their homes, two times a semester over a two year period, conducting environmental home safety and nutritional assessments, medical history, physiology of aging and bio-psychosocial interviews. They also conduct medication reconciliation reviews, discuss community resources options, and end of life issues. By end of a four year graduate experience, seniors have mentored students a total of eight (8) times.Since the inception of the program, SAGE has grown yearly to include more medical disciplines on campus. In addition to TCOM students, the program also includes students from the School of Health Professions Physician Assistant Studies and the Department of Physical Therapy. In 2013 SAGE included students from the UNT System College of Pharmacy and in 2014, students from Texas Christian University School of Nursing have joined. Students from TCU Social Work and Dietetics will join in 2015. The growth of the SAGE program is evident not only in the number of students who are participating (1,034 in 2014), but in the number of actively engaged Senior Mentor volunteers (483). While many of those recruited to participate in the SAGE program have also been clients at Meals on Wheels, of the 1,006 total Senior Mentors who have participated since 2009, SAGE continues to engage residents from The Stayton. The numbers of participants now surpasses the number served by Tarrant County Meals on Wheels and now also includes seniors from Senior Citizens Services, local churches, the UNT Health Geriatrics Clinic and recommended participants by local referring physicians. The Health Science Center has also created a subcommittee of our Healthy Aging Council, a group of committed community members who support all programs related to research, education and clinical care of the aging. The SAGE program director has over 20 years of experience in teaching and academic administration in higher education. She has received awards for her leadership and service from professionals associations at the state and national level. In 2013, the SAGE Program received the Mae Cora Peterson Health Aging Award which recognized innovative program or approaches that improve the health and independent of older adults in Tarrant County.

A draft copy of Form 990 was presented to the Executive Committee of the Board before it was filed. The Executive Committee presented the final return to the Board of Directors at its next regular meeting.
