Form 990, Part VI, Section A, line 4	The Organization adopted an Amendment and Restatement of the Agreement and Declaration of Trust effective, January 1, 2014, by and between the Trustees, the Maine Bankers Association (MBA) and the participating Members. The following changes were adopted: Only financial institutions authorized to do business in the state of Maine which are full voting members of the MBA are eligible to participate in the Trust; the MBA is named as the Plan Administrator and Plan Sponsor as defined under ERISA; the Trustees will be nominated by the Nominating Committee of the MBA; the Trustees shall be named Fiduciaries with respect to the control and management of the Trust Fund; the Plan Administrator shall be named Fiduciary with respect to the administration of the Trust and the plans operated thereunder.
Form 990, Part VI, Section B, line 11	The Form 990 is always reviewed first by the VP/Treasurer, Marc P. Owen and Senior Vice President, Kathleen Meehan, and then by the President, Christopher W.
