PART I, LN 7 COL(F): ACCOUNTS MAY BE ADJUSTED FOR A VARIETY OF REASONS, INCLUDING ERRORS, DISSATISFACTION WITH SERVICE, AND POTENTIAL LEGAL ACTION. IF ALL COLLECTION EFFORTS FAIL, AN ACCOUNT MAY BE ASSIGNED TO BAD DEBT, AND THE OUTSTANDING BALANCE MAY BE WRITTEN OFF.
PART II, COMMUNITY BUILDING ACTIVITIES: 1. LEADERSHIP DEVELOPMENT: THE COMMUNITY HEALTH & ADOLESCENT MENTORING PROGRAM FOR SUCCESS (CHAMPS) AT CHILDREN'S OFFERS SEVERAL EDUCATIONAL PROGRAMS GEARED TO HELP HIGH SCHOOL STUDENTS OF RACIAL AND ETHNIC MINORITIES EXPLORE THE HEALTHCARE AND BIOMEDICAL RESEARCH PROFESSIONS, SUCH THAT THESE PROFESSIONS EVENTUALLY BECOME MORE REPRESENTATIVE OF CALIFORNIA'S OWN DIVERSE POPULATION. THE LONG-TERM GOAL OF CHAMPS IS TO IMPROVE HEALTHCARE ACCESS AND REDUCE THE HEALTH DISPARITIES PRESENT IN OUR SOCIETY. THE CHAMPS PROGRAM PARTNERS WITH LOCAL HIGH SCHOOLS, HEALTH ACADEMIES, UNIVERSITIES, MEDICAL SCHOOLS, AND RESIDENCY PROGRAMS.2. COMMUNITY HEALTH IMPROVEMENT ADVOCACY/WORKFORCE DEVELOPMENT:RESIDENTS-CHILDREN'S RESIDENCY TRAINING PROGRAM CONTINUES TO BE ONE OF THE PREMIERE TRAINING PROGRAMS IN THE WESTERN U.S., WITH A TOTAL OF 89 RESIDENTS AT THREE LEVELS, AND 4 CHIEF RESIDENTS. AS PART OF THEIR REQUIRED TRAINING, RESIDENTS SPEND SEVERAL MONTHS ON THE COMMUNITY, ADVOCACY, AND PRIMARY CARE (CAP) ROTATION, WHERE FUTURE PEDIATRICIANS LEARN HOW TO ADVOCATE FOR THE RIGHTS, SAFETY, HEALTH, AND EDUCATION OF CHILDREN AND THEIR FAMILIES. DURING THEIR CAP ROTATION, RESIDENTS VISIT MORE THAN 20 COMMUNITY SITES. THEY DO HEALTH EDUCATION AT SOME OF THE SITES, AND A FEW RESIDENTS DEVELOP GRANT-FUNDED PROJECTS IN THEIR SECOND YEAR OF TRAINING. RESIDENTS ALSO EVALUATE PATIENTS' HOMES AS PART OF THE ALAMEDA COUNTY HEALTHY HOMES PROJECT, WHERE THEY EDUCATE FAMILIES ABOUT HOME SAFETY. RESIDENTS AT CHILDREN'S ARE DEDICATED TO SERVING A DISENFRANCHISED POPULATION OF AT-RISK CHILDREN, AND A LARGE PERCENTAGE OF CHILDREN'S RESIDENTS GO ON TO PRACTICE IN LOCAL UNDERSERVED COMMUNITIES. FORTY PERCENT OF RESIDENTS GO INTO FELLOWSHIP TRAINING TO BECOME PEDIATRIC SUBSPECIALISTS, AND MANY STAY OR RETURN TO CHILDREN'S AFTER SUBSPECIALTY TRAINING AN ADDITIONAL 200 RESIDENTS FROM 14 NON-CHILDREN'S PROGRAMS ROTATED THROUGH CHILDREN'S IN 2014. RESIDENTS IN EMERGENCY MEDICINE, GENERAL SURGERY, ORTHOPAEDICS, ANESTHESIOLOGY, NEUROSURGERY, RADIOLOGY, AND OTHER AREAS COME TO CHILDREN'S FOR THE PEDIATRIC EXPERIENCE IN THEIR SPECIALTY.SPECIALTY FELLOWS-CHILDREN'S HAD 25 PEDIATRIC SPECIALTY FELLOWS IN 2014 IN THE AREAS OF CRITICAL CARE MEDICINE, EMERGENCY MEDICINE, HEMATOLOGY/ONCOLOGY, INFECTIOUS DISEASE, AND PULMONOLOGY.MEDICAL STUDENTS-CHILDREN'S PROVIDED MONTH-LONG TRAINING IN 12 PEDIATRIC SPECIALTIES FOR MORE THAN 60 MEDICAL STUDENTS IN 2014 FROM MEDICAL SCHOOLS ACROSS THE COUNTRY. IN 2014, CHILDREN'S PROVIDED PEDIATRIC NURSING TRAINING TO MORE THAN 600 NURSES FROM 15 SCHOOLS OF NURSING THROUGHOUT THE U. S. CHILDREN'S ALSO PROVIDES TRAINING TO PROFESSIONAL INTERNS FOR PSYCHOLOGY, RADIOLOGY AND CHAPLAINCY.3. OTHER:FAMILY HOUSE - THIS LOW-COST "HOME AWAY FROM HOME" HAS 16 BEDROOMS AVAILABLE TO QUALIFYING FAMILIES WHO LIVE 70 MILES OR MORE FROM THE HOSPITAL. ALL RESIDENTS SHARE A COMMON LIVING ROOM, PLAY ROOM, KITCHEN, AND LAUNDRY ROOM. FAMILIES ARE REFERRED BY CHILDREN'S HOSPITAL'S CLINICAL SOCIAL SERVICES DEPARTMENT.CHILD LIFE SERVICES - HERE A CHILD'S LIFE CONTINUES EVEN WHEN HEALTH PROBLEMS GET IN THE WAY. CHILD LIFE SERVICES BRIDGES THE GAPS BETWEEN HOME, HOSPITAL, AND SCHOOL WITH AGE-APPROPRIATE ACTIVITIES DESIGNED TO HELP YOUNG PATIENTS UNDERSTAND AND ADAPT TO THE HOSPITAL EXPERIENCE--AND TO KEEP THEM LOVING LIFE EVERY STEP OF THE WAY.FAMILY RESOURCE AND INFORMATION CENTER - THE FAMILY RESOURCE & INFORMATION CENTER SUPPORTS PATIENTS AND FAMILIES BY PROVIDING RESOURCES TO HELP COPE WITH HOSPITALIZATION, HEALTH AND CONDITION INFORMATION, AND LOCAL- AREA INFORMATION. SUPPORT INCLUDES REFERRAL SUPPORT, ACCESS TO BOOKS AND MAGAZINES, EDUCATIONAL PAMPHLETS, INTERNET ACCESS, A FAX, A COPIER, AND RESPITE--ALL PROVIDED IN A COMFORTABLE AND WELCOMING ENVIRONMENT.CENTER FOR CHILD PROTECTION - THE CENTER FOR CHILD PROTECTION AT UCSF BENIOFF CHILDREN'S HOSPITAL OAKLAND OFFERS A FULL RANGE OF MEDICAL AND MENTAL HEALTH SERVICES TO CHILDREN IMPACTED BY ABUSE AND EXPOSURE TO VIOLENCE. RESEARCH SHOWS THAT WHEN HEALTHCARE PROVIDERS ASK ABOUT DOMESTIC VIOLENCE, MORE VICTIMS DISCLOSE ABUSE AND SEEK HELP, AND A SPECIALLY-TRAINED HEALTHCARE PROVIDER CAN EASE THE WAY FOR A BATTERED CAREGIVER TO RECEIVE SERVICES BY OFFERING ENCOURAGEMENT AND SUPPORT. WE TAKE ALL THESE THINGS INTO CONSIDERATION WHEN WE PROVIDE ROUTINE CARE, AND HAVE BECOME ONE OF NORTHERN CALIFORNIA'S LEADING RESOURCES FOR CHILD PROTECTIVE SERVICES IN A MEDICAL SETTING.INJURY PREVENTION PROGRAM - THE INJURY PREVENTION PROGRAM, THROUGH A TEAM OF HEALTH PROVIDERS AND EDUCATORS, PROVIDES EDUCATION, RESOURCES AND PROGRAMS TO FAMILIES AND PATIENTS WITHIN THE HOSPITAL AND IN OUR COMMUNITY. AS A HOSPITAL WITH A LEVEL-1 TRAUMA CENTER, WE ARE COMMITTED TO DECREASING INJURIES AND PREVENTABLE DEATHS. THROUGH EDUCATION WE STRIVE TO ELIMINATE THE CAUSES OF PREVENTABLE INJURY, INCLUDING UNSAFE BEHAVIORS, CONDITIONS, AND ENVIRONMENTS.
PART III, LINE 4: THE METHODOLOGY USED IN DETERMINING THE AMOUNTS REPORTED AS BAD DEBT EXPENSE ABOVE: THE AMOUNT THAT THE PAYER IS EXPECTED TO PAY AND DOES NOT PAY.THE METHODOLOGY USED FOR ACCOUNTING FOR DISCOUNTS AND PAYMENTS ON PATIENT ACCOUNTS IN DETERMINING BAD DEBT EXPENSE ABOVE:ACCOUNTS MAY BE ADJUSTED FOR A VARIETY OF REASONS, INCLUDING ERRORS, DISSATISFACTION WITH SERVICE, AND POTENTIAL LEGAL ACTION. IF ALL COLLECTION EFFORTS FAIL, AN ACCOUNT MAY BE ASSIGNED TO BAD DEBT, AND THE OUTSTANDING BALANCE MAY BE WRITTEN OFF.MANAGEMENT ANALYZED THE LOCATIONS WITH THE MOST BAD DEBT EXPENSE. AN ESTIMATED 65% OF THOSE PATIENTS WITH BAD DEBT EXPENSE WOULD HAVE BEEN ELIGIBLE HAD THEY COMPLETED THE HOSPITAL'S APPLICATION. MOST OF THE INDIVIDUALS WHO ELECT NOT TO COMPLETE THE PROCESS ARE UNDOCUMENTED AND/OR NON-COMPLIANT AND HAVE RECEIVED THEIR SERVICES, CANNOT PAY AND SIMPLY IGNORE OUR COLLECTION EFFORTS.CHILDREN'S ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR PAYOR SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR DOUBTFUL ACCOUNTS AND PROVISION FOR BAD DEBTS ON A CONSOLIDATED BASIS. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WITH THIRD PARTY INSURANCE COVERAGE, CHLDREN'S ANALYZES CONTRACTUALLY DUE AMOUNTS AND PROVIDES AN ALLOWANCE FOR DOUBTFUL ACCOUNTS AND A PROVISION FOR BAD DEBTS, IF NECESSARY. CHILDREN'S RECORDS A PROVISION FOR BAD DEBTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS PAST EXPERIENCE, WHICH INDICATES THAT MANY PATIENTS ARE UNABLE OR UNWILLING TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE.
PART III, LINE 9B: COLLECTION PRACTICES: DIRECT CHARITY CARE IS THE PROVISION OF HEALTHCARE GOODS AND SERVICES WITHOUT CHARGE OR AT SUBSTANTIALLY REDUCED RATES TO MEMBERS OF THE COMMUNITY WHO DEMONSTRATE AN INABILITY TO PAY FOR NECESSARY HEALTHCARE SERVICES EITHER ENTIRELY OR IN PART. THIS IS DISTINGUISHED FROM BAD DEBT WHICH RESULTS FROM AN UNWILLINGNESS TO PAY AND FROM CONTRACTUAL DISCOUNT WHICH RESULTS FROM PREDETERMINED AGREEMENTS TO ACCEPT SOME REDUCTION FROM BILLED CHARGES. COLLECTION ACTIVITY IS NOT INITIATED ON CHARITY CARE PATIENTS BECAUSE THE CHARGES WERE NEVER EXPECTED TO BE PAID, AND ARE THUS NOT BAD DEBT.
PART VI, LINE 2: NEEDS ASSESSMENT:IDENTIFIED EXISTING NEEDS ASSESSMENTS AND EPIDEMIOLOGICAL REPORTS FOR BOTH ALAMEDA AND CONTRA COSTA COUNTIES; CONDUCTED STRUCTURED ONE-ON-ONE INTERVIEWS WITH AN INFORMED GROUP OF STAKEHOLDERS; USING INFORMATION FROM THE REPORTS AND INTERVIEWS, CREATED A LIST OF OVER 50 PUBLIC HEALTH ISSUES AND SOCIAL OR ECONOMIC CONDITIONS THAT AFFECT THE HEALTH OF CHILDREN IN BOTH COUNTIES; SELECTED TOP 20 DISEASES/CONDITIONS; CREATED GRAPHS ILLUSTRATING THE CONDITIONS AND COMPARED TO CALIFORNIA AND US DATA.
PART VI, LINE 3: PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE:INFORMATION ABOUT FINANCIAL ASSISTANCE AVAILABLE FROM CHILDREN'S HOSPITAL SHALL BE DISSEMINATED THROUGH A VARIETY OF MECHANISMS, INCLUDING POSTING NOTICES IN THE EMERGENCY DEPARTMENT, CLINICS, OTHER PUBLIC PLACES IN THE HOSPITAL, OUTPATIENT SETTINGS, AND INCLUDING INFORMATION ON THE PATIENT'S/GUARANTOR'S BILL. THIS INFORMATION SHALL BE PROVIDED IN ENGLISH AND SPANISH, AND WILL BE TRANSLATED FOR PATIENTS/GUARANTORS WHO SPEAK OTHER LANGUAGES.
PART VI, LINE 4: COMMUNITY INFORMATION:ALAMEDA AND CONTRA COSTA COUNTIES ARE THE PRIMARY SERVICE AREA. THEY ARE AMONG THE TOP TEN LARGEST COUNTIES BY POPULATION IN CALIFORNIA. CHILDREN AND YOUNG ADULTS FROM BIRTH TO 24 YEARS CONSTITUTE ABOUT ONE-THIRD OF THE POPULATION. ALAMEDA COUNTY HAS MORE DIVERSITY, WITH A LARGER PROPORTION OF BLACKS AND NEARLY DOUBLE THE PERCENTAGE OF ASIANS COMPARED TO CONTRA COSTA COUNTY, WHERE WHITES ARE THE MAJORITY.
PART VI, LINE 5: PROMOTION OF COMMUNITY HEALTH:SEVERAL PROGRAMS PROMOTE HEALTH IMPROVEMENT. THE FOLLOWING ARE A FEW EXAMPLES. THE CENTER FOR ASTHMA EDUCATION, MANAGEMENT, AND RESEARCH (CAEMR), BASED IN CHILDREN'S PRIMARY CARE CLINIC, OFFERS EXPERTISE IN THE MANAGEMENT OF ASTHMA IN CHILDREN AND ADOLESCENTS. THE KOHL'S INJURY PREVENTION PROGRAM (IPP), ADMINISTERED BY TRAUMA SERVICES AT CHILDREN'S, AIMS TO REDUCE THE NUMBER OF UNINTENTIONAL INJURIES AND FATALITIES IN CHILDREN YOUNGER THAN 14, PRIMARILY THROUGH EDUCATION AND BY PROVIDING EQUIPMENT TO PROMOTE SAFETY. EARLY INTERVENTION SERVICES (EIS) PROVIDES THERAPEUTIC INTERVENTION, CHILD DEVELOPMENT SERVICES, AND FAMILY SUPPORT SERVICES FOR INFANTS AND YOUNG CHILDREN (AGES 0-6) WITH EMERGING DEVELOPMENTAL, MEDICAL, AND SOCIAL-EMOTIONAL DELAYS. CHILDREN'S COMPREHENSIVE CENTER FOR HEMOGLOBINOPATHIES, ONE OF THE LARGEST IN THE WORLD, TREATS ABOUT 1,000 CHILDREN AND ADULTS WITH SICKLE CELL DISEASE AND THALASSEMIA EACH YEAR, PROVIDES EDUCATION TO FAMILIES AND OTHER MEDICAL PROVIDERS, SERVES AS A LOCAL AND INTERNATIONAL RESOURCE, AND CONDUCTS RESEARCH AND ADVOCACY TO IMPROVE THE SURVIVAL AND QUALITY OF LIFE OF PEOPLE WITH THESE CONDITIONS.
