FORM 990, BOX B AMENDED RETURN ST. JOSEPH HOSPITAL OF EUREKA IS AMENDING FORM 990 TO CORRECT THE AMOUNT OF FINANCIAL ASSISTANCE EXPENSE REPORTED IN SCHEDULE H, PART I, LINE 7A.
FORM 990, PART I, LINE 1 & PART III, LINE 1 ORGANIZATION'S MISSION ST. JOSEPH HOSPITAL OF EUREKA IS COMMITTED TO EXTENDING THE HEALING MINISTRY OF JESUS IN THE TRADITION OF THE SISTERS OF ST. JOSEPH OF ORANGE BY CONTINUALLY IMPROVING THE HEALTH AND QUALITY OF LIFE OF PEOPLE IN THE COMMUNITIES WE SERVE.
FORM 990, PART III, LINE 4A REALIZING OUR MISSION ST. JOSEPH HOSPITAL EUREKA HAS BEEN MEETING THE HEALTH AND QUALITY OF LIFE NEEDS OF THE LOCAL COMMUNITY FOR NEARLY 100 YEARS. SERVING THE COMMUNITIES OF HUMBOLDT COUNTY, PARTS OF DEL NORTE, TRINITY AND MENDOCINO COUNTIES, ST. JOSEPH HOSPITAL EUREKA (SJE) IS AN ACUTE CARE HOSPITAL THAT PROVIDES QUALITY CARE IN THE AREAS OF OPEN HEART SURGERY, INTERVENTIONAL CARDIOLOGY, TOTAL JOINT REPLACEMENTS, COMPLEX EYE SURGERY, PEDIATRIC CARE, AND A FULL RANGE OF WOMEN'S AND CHILDREN'S SERVICES. SJE OFFERS COMPREHENSIVE ONCOLOGY CARE THROUGH AN ACCREDITED CANCER PROGRAM, AS WELL AS EMERGENCY MEDICINE BOARD CERTIFIED PHYSICIANS FOR THE HOSPITAL'S FULL SERVICE EMERGENCY ROOM. THE HOSPITAL PROVIDES THE COMMUNITY WITH BOTH AN OUTPATIENT SURGERY AND AN OUTPATIENT IMAGING CENTER. ALL SERVICES WERE GREATLY ENHANCED ON 11/11/12 WHEN THE NEW PATIENT TOWER WAS OPENED TO PATIENT CARE. THE GENERAL HOSPITAL CAMPUS IS LOCATED A FEW BLOCKS FROM THE ST. JOSEPH HOSPITAL AND OFFERS THE COMMUNITY A MEDICAL REHABILITATION CENTER, OCCUPATIONAL MEDICINE, OUTPATIENT LABORATORY SERVICES, A SLEEP DISORDER CLINIC AND OUTPATIENT REHABILITATION SERVICES. THE CAMPUS ALSO HOUSES MANY OF THE SUPPORT SERVICES NEEDED BY THE MAIN CAMPUS INCLUDING PATIENT REVENUE CYCLE SERVICES, CLINICAL EDUCATION, INFORMATION TECHNOLOGY, THE SJE FOUNDATION, PASO A PASO, AND HEALTHY KIDS HUMBOLDT. WITH OVER 1,000 EMPLOYEES COMMITTED TO REALIZING THE MISSION, SJE IS ONE OF THE LARGEST EMPLOYERS IN THE NORTH COAST REGION. AS A MEMBER OF THE ST. JOSEPH HEALTH SYSTEM, SJE IS COMMITTED TO EXTEND THE HEALING MINISTRY OF JESUS IN THE TRADITION OF THE SISTERS OF ST. JOSEPH OF ORANGE. THE SISTERS OF ST. JOSEPH OF ORANGE TRACE THEIR ROOTS BACK TO 17TH CENTURY FRANCE AND THE UNIQUE VISION OF A JESUIT PRIEST NAMED JEAN-PIERRE MEDAILLE. HE SOUGHT TO ORGANIZE AN ORDER OF RELIGIOUS WOMEN WHO, RATHER THAN REMAINING SAFELY CLOISTERED IN A CONVENT, VENTURED OUT INTO THE COMMUNITY TO SEEK OUT "THE DEAR NEIGHBORS" AND MINISTER TO THEIR NEEDS. THE CONGREGATION MANAGED TO SURVIVE THE TURBULENCE OF THE FRENCH REVOLUTION AND EVENTUALLY EXPANDED, NOT ONLY THROUGHOUT FRANCE, BUT THROUGHOUT THE WORLD. IN 1912 A SMALL GROUP OF SISTERS OF ST. JOSEPH WENT TO EUREKA, CALIFORNIA, AT THE INVITATION OF THE LOCAL BISHOP, TO ESTABLISH A SCHOOL. A FEW YEARS LATER, THE GREAT INFLUENZA EPIDEMIC OF 1918 CAUSED THE SISTERS TO TEMPORARILY SET ASIDE THEIR EDUCATION EFFORTS TO CARE FOR THE ILL. THEY REALIZED IMMEDIATELY THAT THE SMALL COMMUNITY DESPERATELY NEEDED A HOSPITAL. THROUGH BOLD FAITH, FORESIGHT, AND FLEXIBLITY IN 1920, THE SISTERS OPENED THE 28-BED ST. JOSEPH HOSPITAL OF EUREKA, THE FIRST ST. JOSEPH HEALTH SYSTEM MINISTRY. THREE MISSION OUTCOMES STRATEGICALLY GUIDE OUR MINISTRY WORK SJE IS COMMITTED TO THREE SYSTEMWIDE MISSION OUTCOMES: 1) SACRED ENCOUNTERS, 2) PERFECT CARE AND 3) HEALTHIEST COMMUNITIES. EVERY INTERACTION WILL BE EXPERIENCED AS A SACRED ENCOUNTER. THE GOAL OF SACRED ENCOUNTER HAS A DIRECT CONNECTION TO THE OVERALL MISSION. OUR VALUE OF DIGNITY CALLS FOR US TO RESPECT EACH PERSON AS AN INHERENTLY VALUABLE MEMBER OF THE HUMAN COMMUNITY AND AS A UNIQUE EXPRESSION OF LIFE. WE STRIVE TO DO THIS BY KEEPING AT THE FOREFRONT OF OUR MINDS THE UNDERSTANDING OF THE IMPACT WE CAN HAVE ON ONE ANOTHER WITH EVERY ACTION WE TAKE. THE LOW INCOME LATINO POPULATION OF HUMBOLDT COUNTY IS ONE THAT IS GROWING BUT IS STILL LIVING ON THE FRINGES OF SOCIETY. OFTEN STRONG IN FAITH, BUT ISOLATED BECAUSE OF LANGUAGE AND CULTURAL BARRIERS, MONO-LINGUAL FAMILIES SUFFER DUE TO A LACK OF MENTAL HEALTH RESOURCES AVAILABLE IN THEIR PRIMARY LANGUAGE. SJE'S COMMUNITY BENEFIT COMMITTEE VOTED TO HAVE MENTAL HEALTH SERVICES FOR THE LATINO COMMUNITY AS ONE OF TWO AREAS THAT COMMUNITY PARTNERS COULD APPLY FOR FUNDING, THUS PROVIDING THE OPPORTUNITY FOR A GROUP OF MARGINALIZED PEOPLE TO HAVE SOME DIGNITY. ALSO PROVIDING SACRED ENCOUNTERS WITH SPANISH-SPEAKING FAMILIES ARE THE STAFF AT HEALTHY KIDS HUMBOLDT. THEIR PRIMARY ROLE IS TO WORK ONE ON ONE WITH FAMILIES TO ENROLL THEM IN ELIGIBLE SERVICES, SUCH AS HEALTH INSURANCE, BUT THEY OFTEN GO FAR BEYOND JUST ENROLLING FAMILIES. THEY GET TO KNOW THE FAMILIES ON A PERSONAL LEVEL AND ASSIST THEM WITH A VARIETY OF ISSUES; MEETING THE PERSON WHERE THEY ARE AT AND PROVIDING THE SERVICE AND CARE NEEDED AT THE TIME. ALL PATIENTS WILL RECEIVE PERFECT CARE. IT IS OUR ATTENTION TO DETAIL AND THE SMALLEST IMPERFECTIONS OF EACH PATIENT'S EXPERIENCE THAT DRIVES A DEEPER UNDERSTANDING AND ULTIMATELY A SUSTAINABLE APPROACH TO THE ACHIEVEMENT OF PERFECT CARE. THE SUCCESS OF THE CARE TRANSITION PROGRAM AT SJE CONTINUES TO SURPASS ALL TARGETS SET WHEN THIS PROGRAM BEGAN. THE TEAM OF A SOCIAL WORKER AND A NURSE WORK WITH HOSPITAL STAFF TO TRANSITION THE HOMELESS PATIENT FROM THE ACUTE CARE SETTING TO A SAFE ENVIRONMENT FOR HEALING. THE TEAM HAS PARTNERED WITH THE COMMUNITY BENEFIT DEPARTMENT AND THE EUREKA COMMUNITY RESOURCE CENTER, A DEPARTMENT OF ST. JOSEPH HEALTH, REDWOOD MEMORIAL HOSPITAL, TO IDENTIFY THE INDIVIDUALS MOST IN NEED OF THE CARE AND PROVIDE THAT CARE IN ONE OF TWO HOUSING UNITS RENTED FOR THIS PURPOSE. OVER TIME, THE TEAMS HAVE ADDED FOOD, TRANSPORTATION, CLOTHING AND MEDICATION TO THE SUPPORT OFFERED TO THE PATIENT. THE SUPPORT SERVICES ARE MAKING AN IMPACT ON THE QUALITY OF LIFE OF THE PATIENTS SERVED AND ARE MOVING US CLOSER TO ACHIEVING OUR GOAL OF PERFECT CARE. THE COMMUNITIES WE SERVE WILL BE AMONG THE HEALTHIEST IN OUR NATION. WE SEEK TO DEVELOP COMMUNITY HEALTH INITIATIVES THAT IMPACT LONG-TERM HEALTH ACROSS THE ENTIRE COMMUNITY. FOOD INSECURITY IN OUR COUNTY IS ESPECIALLY HIGH FOR SINGLE WOMEN WITH CHILDREN MAKING IT DIFFICULT FOR THEM TO ACHIEVE AND MAINTAIN LONG-TERM HEALTH. IN AN EFFORT TO BETTER ADDRESS THESE POPULATIONS' NEEDS, SJE JOINED A COORDINATED EFFORT LED BY THE HUMBOLDT COUNTY DEPARTMENT OF HEALTH AND HUMAN SERVICES TO INCREASE ACCESS TO CALFRESH FOR ALL THOSE ELIGIBLE FOR THIS SERVICE. HUMBOLDT COUNTY RECEIVED A MULTI-YEAR GRANT TO INCREASE CALFRESH DOLLARS USED BY FAMILIES TO IMPROVE ACCESS TO HEALTHY FOOD FOR THE FAMILY AND TO INCREASE DOLLARS THAT ARE GENERATED BY CALFRESH EXPENDITURES. THE CAMPAIGN HAS BEEN LABELED BY COMMUNITY PARTNER, FOOD FOR PEOPLE, AS 'BRING A MILLION TO HUMBOLDT'. CALIFORNIA STATISTICS SHOW THAT IF ALL PEOPLE IN HUMBOLDT COUNTY THAT ARE ELIGIBLE TO RECEIVE CALFRESH USE THEM, AN ADDITIONAL ONE MILLION DOLLARS IN FOOD REVENUE WILL BE GENERATED. HUMBOLDT COUNTY HAS MANY SMALL FAMILY OWNED FARMS THAT BRING FRESH, NUTRITIOUS FOOD RESOURCES TO THE MULTIPLE COUNTYWIDE FARMER'S MARKETS AND THESE FARMERS ACCEPT CALFRESH DOLLARS. IF THE DOLLARS ARE INCREASED BY ONE MILLION, THE FAMILY FARMS BENEFIT AND THE CIRCLE OF BRINGING HIGH QUALITY, LOCALLY GROWN FOOD TO THE TABLES OF AREA FAMILIES INCREASES AS WELL. SJE PROVIDED CARE FOR THE POOR FUNDING IN FY14 TO NINE (9) LOCAL ORGANIZATIONS TO INCREASE ACCESS TO FOOD RESOURCES. ONE OF THE LARGEST GRANTS WAS GIVEN TO FOOD FOR PEOPLE TO INCREASE ACCESS TO FRUITS AND VEGETABLES FOR LOW INCOME FAMILIES WHO UTILIZE THE FOOD PANTRIES. FOOD FOR PEOPLE PURCHASED THE FRUITS AND VEGETABLES FROM FAMILY FARMS AND HAD SPECIAL EVENTS WHERE THE FRUITS AND VEGETABLES WERE DISPENSED IN A FARMERS MARKET TYPE VENUE. THE EVENTS WERE HELD AT THE MAIN FOOD FOR PEOPLE SITE IN EUREKA AND AT SITES IN THE MORE ISOLATED REGIONS OF THE COUNTY. HUMBOLDT SENIOR RESOURCE CENTER ALSO RECEIVED FUNDING TO SUPPORT THEIR HOME DELIVERED MEAL (HDM) PROGRAM TO HOMEBOUND SENIORS. THE FUNDS PROVIDED ALLOWED THE SENIOR RESOURCE CENTER TO ELIMINATE THEIR WAIT LIST FOR DELIVERED MEALS. AN ADDITIONAL BENEFIT OF THE HDM PROGRAM IS WELLNESS CHECK PROVIDED BY THE DRIVER. THIS IS OFTEN THE ONLY HUMAN INTERACTION IN A DAY FOR ISOLATED SENIORS. PROGRAM SERVICE ACCOMPLISHMENTS CONTINUED FINANCIAL ASSISTANCE PROGRAM, MEDICAID AND OTHER MEANS TESTED PROGRAMS OUR MISSION CALLS US TO PROVIDE QUALITY CARE TO ALL OUR PATIENTS, REGARDLESS OF ABILITY TO PAY. WE BELIEVE THAT NO ONE SHOULD DELAY SEEKING NEEDED MEDICAL CARE BECAUSE THEY LACK HEALTH INSURANCE. THAT IS WHY ST JOSEPH HOSPITAL EUREKA HAS A PATIENT FINANCIAL ASSISTANCE PROGRAM THAT PROVIDES FREE AND/OR DISCOUNTED SERVICES TO ELIGIBLE PATIENTS. IN ADDITION TO THE FINANCIAL ASSISTANCE PROGRAM AND THE UNPAID COST OF MEDICAID AND OTHER MEANS TESTED PROGRAMS, SJE HAS DEVELOPED AND IMPLEMENTED COMMUNITY BENEFIT PROGRAMS TO ADDRESS IDENTIFIED COMMUNITY HEALTH NEEDS. THE FOLLOWING THREE PROGRAMS LISTED BELOW ARE ORGANIZED BY LARGEST TOTAL EXPENSE. PHYSICIAN TRAUMA SERVICES SJE CONTINUES TO ADDRESS THE NEEDS OF THE PATIENTS SEEN IN THE EMERGENCY ROOM WHO LACK PRIMARY MEDICAL PROVIDERS AND A MEDICAL HOME. FOCUSED EFFORTS BY THE CARE TRANSITION TEAM ON ASSISTING PATIENTS POST DISCHARGE AND BY THE EUREKA COMMUNITY RESOURCE CENTER TO ASSIST PREACUTE HOSPITALIZATION CONTINUES TO MAKE AN IMPACT ON THE POPULATION SEEN THROUGH THE EMERGENCY ROOM. THE TWO TEAMS, WORKING TOGETHER, PROVIDE MEDICATIONS, HOUSING, FOOD AND
