Form 990, Part VI, Line 2 - Related party information among Officers	Ted Lewis, the association's executive director has a legal consulting arrangement with beneficial life insurance company, which is represented on the board by J. David Pearce.
Form 990, Part VI, Line 3 - Delegated Duties to Management Company	Ted Lewis, the association's designated executive director, managed the day to day operations, and kept the books and records of the association.
Form 990, Part VI, Line 6 - Classes of Members or Shareholders	Members of the Association are by statute all the insurers with a relevant certificate of Authority in the State of Utah, and such member insurers elect seven member insurers to the board of directors, subject to the approval of the Utah Insurance Commissioner.
Form 990 Part VI, Line 7a - Election of Members and their Rights	The member insurers of the guaranty association select the board of directors of the guaranty association, subject to the approval of the Utah Insurance Commissioner. The Utah Insurance Commissioner also appoints two public representative members to the board of directors of the association.
Form 990, Part VI, Line 7b - Decisions Subject to Approval of Members	The election of the members of the board is subject to the approval of the Utah Insurance Commissioner. In addition, the plan of operation (including amendments) is subject to the approval of the Utah Insurance Commissioner.
