Pt VI, Line 11b	The return is reviewed by the board during the yearend financial/audit review.
Other	The return is being amended to correct Line A of page one correcting the reporting period from a calendar year ending on December 31,2014 to a fiscal year that ended September 30, 2015. The organization since inception has been on a fiscal year. The return as reviewed by the committee contained the correct period but the electronically filed return was in error.
Other	Some confusion resulted from the death of the committee's co-chair in 2013. During the period of her illness annual reports were not filed, resulting in the loss of the committee's exemption. The request for reinstatement was granted in June of 2015 and all supporting documentation, as well as all reports filed with the State of Illinois were filed with a September 30 fiscal.
