SABA ASSISTS STUDENTS IN MAKING SUCCESSFUL TRANSITIONS FROM SCHOOL TO CAREERS AND SUPPORTS THE DEVELOPMENT OF A QUALIFIED WORKFORCE IN THE MOHAWK VALLEY. WELL-EDUCATED STRATEGIES ARE USED TO HELP STUDENTS UNDERSTAND THE WORLD OF WORK, DECIDE ON A CAREER, POST-SECONDARY STUDIES AND TRANSITION TO THE WORKPLACE. PARTNERSHIPS BETWEEN BUSINESSES, COMMUNITY AGENCIES, SCHOOL DISTRICTS AND COLLEGES PROVIDE OPPORTUNITIES TO UNDERSTAND THE 21ST CENTURY SKILLS NECESSARY TO BE COLLEGE, CAREER AND CITIZEN READY. D)FSLH PARTNERED WITH THE MOHAWK VALLEY RESOURCE CENTER FOR REFUGEES (MVRCR) TO CREATE EMPLOYMENT OPPORTUNITIES FOR REFUGEES IN SUPPORT SERVICES, INCLUDING ENVIRONMENTAL AND NUTRITION SERVICES. E)MVHS OPENED A NEW INSTRUCTIONAL LAB IN THE MEDICAL IMAGING DEPARTMENT AT THE FAXTON CAMPUS IN FEBRUARY 2015. THE LAB IS USED BY STUDENTS OF THE MOHAWK VALLEY COMMUNITY COLLEGE (MVCC) RADIOLOGICAL TECHNOLOGY PROGRAM AND IS EQUIPPED WITH A CLASSROOM AREA, A FACULTY WORK AREA, AND RADIOLOGY EQUIPMENT ON WHICH STUDENTS CAN PRACTICE BEFORE BEGINNING THE CLINICAL PORTION OF THEIR STUDIES.
PART III, LINE 2: BAD DEBT FOR SCHEDULE H REPORTING IS DETERMINED BY TAKING THE ACTUAL BAD DEBT EXPENSE REPORTED FOR THE YEAR ON THE HOSPITAL AUDITED FINANCIAL STATMENTS, LESS ANY BAD DEBT RECOVERIES RECEIVED DURING THE YEAR FROM THE AUDITED FINANCIAL STATEMENTS. THEN, ADJUSTING THAT AMOUNT TO COST BASED ON THE HOSPITAL'S CURRENT YEAR MEDICARE COST REPORT DERIVED COST TO CHARGE RATIO. THIS AMOUNT UNDERSTATES BAD DEBT SOMEWHAT BECAUSE A LARGE PORTION OF BAD DEBT IS THE RESULT OF UNPAID DEDUCTIBLE AND CONINSURANCE BALANCES. THOSE BALANCES ARE NO LONGER RECORDED AT FULL CHARGES BECAUSE CONTRACTUAL ADJUSTMENTS HAVE ALREADY REDUCED CHARGES TO AN EXPECTED REIMBURSEMENT AMOUNT; HENCE, REDUCING BY A COST-TO-CHARGE RATIO, REDUCES AN ALREADY DISCOUNTED CHARGE. WE REPORT THIS WAY BECAUSE BAD DEBT IS REPORTED IN MUTLIPLE STATE AND FEDERAL REQUIRED REPORTS, RETURNS AND DISCLOSURE STATEMENTS. IN ORDER TO BE CONSISTENT WE ARE REPORTING USING THE COSERVATIVE METHOD WHICH IS REQUIRED FOR HOSPTIAL MEDICARE COST REPORT REPORTING.
PART III, LINE 3: MANY PATIENTS WHO WOULD QUALIFY FOR CHARITY CARE ASSISTANCE ARE UNWILLING TO APPLY FOR IT. THE HOSPITAL BUSINESS OFFICE OFFERS OUR CHARITY CARE PACKAGE TO ALL PATIENTS. IN 2015, THE CHARITY CARE PROGRAM WAS EXPANDED DUE TO THE IMPLEMENTATION OF THE 340B PROGRAM. MANY PATIENTS WILL AGREE TO HAVE THE CHARITY CARE REQUEST FORMS SENT TO THEM BUT NEVER RETURN THEM. MANY PATIENTS SAY THEY CANNOT PAY BUT ARE UNWILLING TO COMPLETE ANY FORMS. THE HOSPITAL BUSINESS OFFICE, COLLECTIONS STAFF, HAVE FORMED A PERCENTAGE ESTIMATE BASED ON THE NUMBER OF CASES OF PATIENTS WHO RECEIVE CHARITY CARE FORMS AND DON'T COMPLETE THEM PLUS THE NUMBER OF PATIENTS WHO STATE THEY CANNOT PAY BUT ARE UNWILLING TO REQUEST ASSISTANCE. THIS RECORDED BAD DEBT EXPENSE WOULD BE RECORDED AS CHARITY CARE IF PATIENT'S WERE WILLING TO COMPLY WITH MINIMAL APPLICATION REQUIREMENTS. THE NATURE OF THE PRESENTATION ON HOSPITAL FIANANCIAL STATEMENTS DOES NOT DETERMINE THE COMMUNITY BENEFIT BUT RATHER THE NATURE OF THE REASON FOR THE EXPENSE. THIS IS CHARITY CARE.

THE PROGRAM, LED BY THE ONEIDA COUNTY HEALTH DEPARTMENT, USED THE MOBILIZING FOR ACTION THROUGH PLANNING AND PARTNERSHIPS FRAMEWORK (MAPP) TO CONDUCT THE CHNA ACTIVITIES. THE ASSESSMENT RESULTS WERE COMBINED WITH POPULATION TO IDENTIFY THE TOP TWO PRIORITY AGENDA INITIATIVES FOR 2014 THROUGH 2017. FSLH WILL CONTINUE TO WORK WITH HERKIMER COUNTY TO MORE FULLY DEVELOP AND ADOPT STRATEGIES AND MEASURES TO SUPPORT THE PRIORITIES. THE CHNA FOR FSLH WILL BE UPDATED QUARTERLY AND REVIEWED ANNUALLY FOR PROGRESS AND UPDATES BY MEMBERS OF THE FSLH HEALTH IMPROVEMENT TEAM AND IN COOPERATION WITH OUR COMMUNITY PARTNERS.

COMPENSATION IS BASED ON PERFORMANCE RELATIVE, IN PART, TO THE ORGANIZATION NET EARNINGS TARGETS, AS WELL AS, VARIOUS OTHER QUALITY RELATED PERFORMANCE TARGETS. AS PREVIOUSLY REPORTED IN SCHEDULE H OF THE 2014 FORM 990 REPORTING (SEE PART VI, LINE 6), FSLH AND ST ELIZABETH MEDICAL CENTER (SEMC) AFFILIATED UNDER THE MOHAWK VALLEY HEALTH SYSTEM (MVHS) ON MARCH 6, 2014. AS A RESULT OF THIS AFFILIATION, DUPLICATE SERVICES ARE BEING CENTRALIZED FOR PATIENT CONVENIENCE AND OVERALL EFFICIENCY. IN 2015, CARDIAC CARE SERVICES FOR CATHETERIZATIONS AND ANGIOPLASTY PROCEDURES, PREVIOUSLY DONE AT FSLH, ARE NOW PERFORMED AT SEMC. IN ADDITION TO THE EFFICIENCIES, THIS ALSO ALLOWS FOR THE FURTHER DEVELOPMENT OF SEMC AS THE CARDIAC CARE CENTER FOR MVHS AND IN OUR COMMUNITY. ADDITIONALLY, MEDICAL GROUPS FOR PRIMARY CARE WHICH WERE SEPARATE FOR SEMC AND FSLH, ARE NOW ALL PART OF THE MVHS MEDICAL GROUP; STILL WITH SEPARATE REPORTING UNDER THEIR RESPECTIVE EIN'S. AN EXCEPTION TO THE GROUPS REMAINING UNDER THEIR ORIGINAL EIN, APPLIES TO TWO COMMUNITY SITES WHICH WERE MERGED INTO ONE EACH FOR THE DUPLICATE LOCATIONS OF NORTH UTICA AND WATERVILLE; THESE NOW REPORT UNDER THE FSLH EIN AS PART OF THE MVHS MEDICAL GROUP. THE OTHER EXCEPTIONS ARE THE SAUQUOIT AND CLINTON MEDICAL OFFICES PREVIOUSLY UNDER SEMC EIN ARE NOW UNDER FSLH EIN. THE MEDICAL CARE PROFESSIONALS REMAIN THE SAME FOR PATIENTS FROM EACH ORGANIZATION AT EACH SITE.
