Explanation:

Person responsible for taking over filing responsibilities no longer works with the organization, nor did he communicate he was no longer working with the organization. Everything was left to an inexperienced college student who was not aware of filing responsibility. This college student had to figure out how to do this on his own, but will ensure the organization has a knowledgeable alumni member take responsibility for this role.
