| CORE FORM, PART III, STATEMENT OF PROGRAM SERVICE ACCOMPLISHMENTS | Carbon-Schuylkill Community Hospital, Inc. ("St. Luke's Miners Memorial Hospital" a.k.a. "SL-Miners") is a Joint Commission-accredited, not-for-profit, 44-licensed bed acute care hospital and 48-bed skilled nursing facility. SL-Miners is located in Coaldale, Pennsylvania, in Schuylkill County near the Carbon County border. SL-Miners primarily provides care to residents of Schuylkill, Carbon and Lower Luzerne counties in Northeastern Pennsylvania. Annually, SL-Miners provides care for more than 2,600 observations and admissions, more than 83,000 outpatient visits and 15,000 emergency department visits. SL-Miners is recognized by the Internal Revenue Service ("IRS") as an Internal Revenue code section 501(C)(3) tax-exempt organization. Pursuant to its charitable purposes, SL-Miners provides medically necessary healthcare services to all individuals in a non-discriminatory manner regardless of race, color, creed, sex, national origin, religion or ability to pay. Moreover, SL-Miners operates consistently with the following criteria outlined in the IRS Revenue Ruling 69-545: 1. SL-Miners provides medically necessary healthcare services to all individuals regardless of ability to pay, including charity care, self-pay, Medicare and Medicaid patients; 2. SL-Miners operates an active emergency department for all persons; which is open 24 hours a day, 7 days a week, 365 days per year; 3. SL-Miners maintains an open medical staff, with privileges available to all qualified physicians; 4. Control of SL-Miners rests with its Board of Directors and the Board of Directors of St. Luke's Health Network, Inc., d.b.a. St. Luke's University Health Network. These boards are comprised of a majority of independent civic leaders and other prominent members of the community, as well as physicians on the Hospital/Network medical staff; and 5. Surplus funds are used to improve the quality of patient care, expand and renovate facilities and advance medical care programs and activities. The operations of SL-Miners, as shown through the factors outlined above and other information contained herein, clearly demonstrate that the use and control of SL-Miners is for the benefit of the public and that no part of the income or net earnings of the organization is for the benefit of any private individual nor is any private interest being served other than incidentally. Founded in 1910, SL-Miners was acquired by nationally recognized St. Luke's University Health Network ("Network") in 2000 and celebrated its 100th year of community service in October 2010. Since joining the Network, SL-Miners admissions have increased by nearly 19 percent. The Hospital provides patients with access to 190 physicians across 25 medical specialties. The Hospital is accredited as a Chest Pain Center by the Society of Cardiovascular Patient Care. Additionally, in 2014, the Hospital ranked first in Pennsylvania and eighth in the U.S., out of 2,728 hospitals, for the highest Value Based Purchasing scores, based on the Hospital's clinical quality outcomes and patient satisfaction scores. SL-Miners was named a 2014 Premier QUEST winner for achieving top decile performance in all measurements (mortality, cost of care, patient satisfaction evidence-based care, safety and readmission); and earned a 2014 Achievement Award from the Hospital and Healthsystem Association of Pennsylvania. SL-Miners was named a 2015 Premier Quest Award winner for High-Value Healthcare - as a recipient of the Citation of Merit, which recognizes hospitals that achieve top performance in five out of seven measures. The Hospital was granted Level IV Trauma Center accreditation status by the Pennsylvania Trauma Systems Foundations, effective November 1, 2013, which status recently was renewed for an additional three-year period. This is the first time that a new trauma program received a three-year resurvey term which is attributed to the Pennsylvania trauma System Foundation's confidence in the SL-Miners program. The Hospital's specialty services include the following: - Cardiology - Certified Diabetes Educator - Emergency Services - Endocrinology - Gastroenterology - Geriatric Medicine - Infectious Disease - Long-term Care Services - Nephrology - Neurology - Oncology/Hematology - Orthopedics, joint and muscle disorders - Otolaryngology/ENT - Psychiatry - Pulmonology - Radiology (advanced) - Short-term Rehabilitation - Sleep Disorders - Sports Medicine, Physical, Speech and Occupational Therapy, Rehabilitation - Surgery (general and laparoscopic) - Urology - Vascular - Women's Health - Wound Management The Network has invested approximately $32.1 million in technical and facility improvements within SL-Miners since 2000, including outpatient centers which offer advanced technology and physician services, further enhancing patients' ability to easily access state-of-the-art healthcare. Capital projects include the following: - a state-of-the-art emergency department in 2002, new MRI, sleep disorder center, cardiovascular conditioning center, new access road from Rt. 209 directly to the Hospital; - a newly renovated $1.6 million six-bed ICU in 2010; - an extensive $2.6 million renovation of the Hospital's ground floor, including the lobby, registration and laboratory was completed in 2012, and a new St. Luke's Heart & Vascular Center was added; - a new $1.6 million outpatient facility was established in Tamaqua on Route 309 in 2013. Services offered include: orthopedics, sports medicine, women's health, neurology, imaging and laboratory; - Additional facility improvements in 2013 included elevator replacement, updated telemetry equipment and roof replacement; - a new blood draw site in Albrightsville, PA; completed structural upgrades to the main Hospital building, purchased operating room equipment, including an arthroscopy table for use in a muscle sparing hip replacement procedure which enables patients to recover more quickly; and a laser for use in non-invasive urology procedures in 2014; - SL-Miners purchased three physical therapy clinics to improve availability of services in the Miners coverage area in August 2014; - a new Medical Office Building adjacent to the hospital to provide additional physician specialty services and improve ease of access to our patients in 2015; and - Construction has commenced on a new inpatient floor. It will feature private rooms and open in April 2016. SL-Miners is located in Coaldale, a medically underserved area. The Hospital has significantly expanded medical expertise through the addition of the following services: - Direct access to orthopedic surgeons providing the most advanced diagnosis, treatment and rehabilitation for patients with musculoskeletal disorders and injuries; - Direct access to highly trained cardiac team which includes experienced and skilled cardiologists, cardiac and vascular surgeons, interventional radiologists and electrophysiologists; - Direct access to oncologists and a wide range of oncology specialists; and - Direct access to hospitalists. SL-Miners operates three federally designated rural health clinics in Hometown, McAdoo and Nesquehoning, serving 13,265 patient visits in FY '15. The centers treat patients of all ages, offering exceptional quality care close to home. In FY'15, mental health assessment and therapeutic services were added at the centers. No patient is denied care, regardless of their ability to pay for their care. All insurances, including Medicaid, are accepted. In FY'15, SL-Miners began providing 24/7 TelePsychiatry and TeleNeurology services which delivers immediate benefit to both patients and clinical staff by providing quick access for psychiatric and neurological consultations for patients. The Hospital also established a clinical rotation in collaboration with four schools to provide physician assistant students the opportunity to do their clinical study in a rural community setting. St. Luke's Miners Rehabilitation and Nursing Center is a fully accredited, 48-bed hospital-based, skilled nursing facility located on the fifth floor of the Hospital. Individualized short-time rehabilitation and long-term care services are provided. |

The mission of SL-Miners is an unwavering commitment to excellence as we care for the sick and injured; educate physicians, nurses and other healthcare providers; and improve access to care in the communities we serve, regardless of a patient's ability to pay for their care. Community Outreach ------------------ In keeping with its commitment to the communities it serves. SL-Miners annually reaches more than 3,200 people through its community outreach endeavors. The Hospital offers a variety of free screenings/services for community-run events throughout the year. Community outreach includes, but is not limited to, the following: - Utilized numerous media outlets to educate the community about health issues that may impact them; - Provided Live Your Life wellness program - more than 400 participants. The focus of the program is to promote healthy eating habits and increase physical activity while reducing the prevalence of diabetes and other chronic diseases; - Provided "Get Your Tail on the Trail" physical activity program for children and adults - more than 270 participants; - Provided smoking cessation classes, diabetes prevention and education classes; - Administered blood pressure screenings and 175 flu and pneumonia vaccines at no charge; - Offered Reading Rocks! Program, an after-school reading program that pairs elementary students identified by teachers as needing additional assistance with members of the hospital staff for additional reading practice; - Provided vision van services to 148 students and health van services to 40 students in local school districts; - Conducted Healthy Living Initiative in partnership with the Kellyn Foundation - healthy living, nutrition, and garden programs for 147 third grade students; - Provided summer lunch program in conjunction with two local churches, serving children and adults; - Conducted four health fairs and 21 Community Health talks on a variety of topics including: diabetes, drug and alcohol awareness, exercise and heart disease, community safety, the benefits of screening for colorectal cancer, weight management and type 2 diabetes, smoking cessation; - Sponsored MS/Parkinson's and nutrition and weight management support groups; and - Matter of Balance Program. The program consisted of the mock car crash with victims being extracted from the vehicle by emergency responders and transported via ambulance, booking of charges and magistrate activity. A presentation by St. Luke's Miners Hospital trauma team followed to reinforce the dangers of distracted driving; - Participated in "Carbon County Safety Day," an annual event attended by more than 1,200 members of the community.
