Patient Care- Rush Oak Park Hospital(ROPH) is a community hospital committed to balancing clinical excellence, a professional and healing environment and community outreach programs to address various healthcare issues. Patient care was provided in over 4,600 inpatient admissions and over 77,000 outpatient visits.

Physician Practices- A group of seven ROPH employed physician practices provide a variety of community healthcare services. Rush Oak Park Physician Group primarily serves residents in Oak Park, IL. Other communities include but are not limited to River Forest, Forest Park, Elmwood Park, Hillside, Berwyn, and North Riverside. In 2015, the physicians provided services in over 81,000 visits. Types of services provided include family medicine,internists,gynecology,geriatric, and neurology. The hospital supports the staff, physicians and operations of the practices. The practices adhere to the mission statement of the hospital.

Successful directives have been implemented to ensure that the ER remains the community's best option for emergency care.

Part V, Section B, Line 5: The assessment includes the collection and analysis of the most up-to-date health, social, economic, housing and other data, as well as qualitative input directly from community leaders, representatives and agencies through focus groups and interviews. Rush Oak Park Hospital conducted in-person interviewees with individual stakeholders and internal staff between September 2012 and January 2013. Participants were chosen based on their ability to identify the primary health concerns of the community, their expertise in public health or their association with the types of program interventions or resources that could address identified concerns. The interviewees consisted of government officials, community organization leaders, church leaders, school leaders, physicians, public health officials and nurses. By virtue of their positions and professional training, these individuals have considerable expertise related to their constituent groups and have provided invaluable insights throughout ROPH's CHNA process. These include those with special knowledge or expertise in public health and leaders or representatives of medical underserved, low income, and minority populations.

Part V, Section B, Line 11: The top three health needs identified and described in the Rush Oak Park Hospital CHNA included the following: access to health care services; chronic disease & risk factors (heart disease, lung disease and diabetes); and mental health and addiction.To address these needs, Rush Oak Park Hospital applies two main strategies:Health Education and PromotionAllocation of Corporate Resources and Building Corporate PartnershipsHealth education and promotion at Rush Oak Park Hospital: ExamplesHealthy Motivations Community Wellness Program (formerly Age-WISE): Through this program ROPH offered free education on topics such as heart and vascular disease, preventive health, thyroid disorders, colorectal cancer and depression. It also provided free Zumba Gold fitness classes to community members and acted as a referral source for those seeking access to health care.Diabetes Health Fair: The annual diabetes health fair provided free health screenings to community participants. Community Outreach Nursing: Health Screenings: Nurses from ROPH provided health education and screenings, including stroke, BMI, cholesterol, prostate, thyroid and A1C screenings. These efforts were often in collaboration with local groups such as the Forest Park Community Center, the Berwyn Public Health District and the Oak Park-River Forest Food Pantry. Health Education at Housing Forward (formerly West Suburban PADS): Nurses at ROPH collaborated with West Suburban PADS (now Housing Forward), an organization dedicated to transitioning people from housing crisis to housing stability, to educate clients on such topics as stroke and stress.Education of health care professionals: Rush Oak Park Hospital acts as a training ground for nursing as well as food and nutrition professionals. In addition, its CEO teaches and advises graduate students in health care management.Allocation of corporate resources and building corporate partnerships at Rush Oak Park Hospital: ExamplesRush Oak Park Hospital donated to organizations dedicated to promoting access to health care such as Catholic Charities, the Human Rights Campaign and the Oak Park-River Forest Infant Welfare Society. Rush Oak Park Hospital, Inc. Part V, Section B, Line 13h: The State of Illinois requires that hospitals make a good faith effort to identify "presumptive" charity care for uninsured patients under 200% of FPL. We complete this process by receiving FPL information from a 3rd party vendor for all uninsured patients.
Rush Oak Park Hospital, Inc. Part V, Section B, Line 22d: The uninsured patient discount for patients at or below 600% FPG for emergent and medically necessary care is set at 68%. ROPH calculates this discount in accordance with the calculation prescribed by Illinois state law, which is based on the hospital's cost to charge ratio. The calculation as of December 2014 yielded a 66% discount. ROPH elected to retain the discount at 68%, which is higher than what is required. Uninsured patients with incomes greater than 600% FPG are granted discounts at a lower level ranging from 50% to 33%.No individual who is determined to be eligible for financial assistance will be charged more for emergency or other medically necessary care than the amount generally billed for individuals who have insurance covering such care. The basis to which any discount is applied is equivalent to the billed charges posted to a patient account minus any prior insurance payments and adjustments from the patient's insurance (if applicable). ROPH determines the amount generally billed (AGB) to individuals who have insurance covering their care by multiplying its charges for any emergency or other medically necessary care it provides by certain percentages using the Internal Revenue Service look-back method as described in Treas. Reg. 1.501(r)-5. The look back method, analyzes a recent 12-month period of allowed claims to determine the actual payment rate that Medicare and private insurers are collectively allowing. The intent is to ensure that the discount provided to financial assistance eligible patients is equal to or greater than the discount provided to patients with insurance.
Part V, Section B, Line 22d Discounts for 100% Charity Care Gross charges or patient liability is discounted at 100% for 100% charity care (300% of the federal poverty level "FPL"). Discounts for Limited Income Gross charges or patient liability are discounted at 70% for patients approved for limited income discount (400% of FPL). Discounts for Self-Pay Patients: ROPH offers an automatic 68% discount for residents of Illinois who do not have a health insurance plan. Non-Illinois residents who do not have health Insurance automatically qualify for a 50% discount. For patients who cannot pay their portion of the bill at the time of service, financial counselors work closely with them to set up monthly installment payment plans with no interest at an amount with which the patient is comfortable. Payment Plans: If requested, Interest-free payment plans are available to patients. Payments can be made over at most, 24 months with a minimum payment of $25.00 each month. The hospital does not assess interest on unpaid balances. State and Federal Programs: This service focuses on providing patients who arrive at the hospital without insurance with the coverage they are entitled to under various federal and state programs. Financial counselors work with patients and alert them if they qualify for one of a handful of programs such as the state Medical Assistance Non-Grant (MANG) program or the Social Security Disability program (SSDI). Because the paperwork required for these programs can be overwhelming, the hospital has specialists who assist patients with the application process.

Form and Line Reference Explanation
Part I, Line 3c: All uninsured patients whose income is at or below 200% of FPG are automatically granted free care under Rush Oak Park's Presumptive Charity Care Program. Rush utilizes data received from a third party vendor that is filed with the major credit bureaus to determine whether the patient's income meets the income threshold. Charges for services rendered to patients meeting this threshold are written off prior in full to a bill being sent to the patient. A patient may also be eligible for presumptive charity care if the patient is eligible for Medicaid for other dates of service or services deemed non-covered by Medicaid; the patient is enrolled in, or eligible for, an assistance program for low income individuals; or the patient is homeless, deceased with no estate, or mentally incapacitated with no one to act on the patient's behalf. All uninsured patients not meeting the requirements of presumptive charity care are automatically granted an uninsured patient discount ranging from 68% to 33%, based on income level. The uninsured patient discount is an Illinois State mandated discount based on Rush's cost to charge ratio which is updated annually. Patients may complete an application process to determine eligibility with Rush's Charity Care and Limited Income Programs. Both programs are income-based and utilize an asset test to determine eligibility. Patients meeting the asset test whose income is 201-300% of FPG qualify for 100% discount to their hospital bill; patients meeting the asset test whose income is 301-400% FPG qualify for a 75% discount to their hospital bill. Discounts under both programs may be applied after primary insurance payment to cover deductibles, coinsurance, and copays.

However, the cost-to-charge ratio for the subsidized health program was calculated specifically on the costs and charges of the program which is the Skilled Nursing Facility Data was derived using the FY15 as-filed Medicare cost report.
Part 1, Line 7 column f Part I, Line 7, Column F -- Total expenses reported on Form 990, Part IX, line 25, column (A) include bad debt expense. However, for the purposes of Schedule H this expense has been removed from the denominator when calculating the percent of total expense considered the net community benefit expense and reported on Part I, Line 7, column (f). The amount of bad debt expense excluded from this percentage calculation is $6,295,928.
Part II, Community Building Activities: Rush Oak Park Hospital is involved in numerous community building activities that promote the health and well-being of the communities it serves. In order to promote the health of the community we serve, our hospital participates in extensive community building activities that are not a part of Part I Charity Care and Other Community Benefits. Here are examples of those activities:Community support American Heart Association Heart Walk: Rush is closely aligned with the American Heart Association's (AHA) mission of building healthier lives free of cardiovascular diseases and stroke. Our partnership is evidenced in our patient care, community education and employee wellness programs, as well as our Heart Walk participation. Rush Heart Walk team promotes heart health and associate camaraderie while raising lifesaving funds. Rush Oak Park staff also provided volunteers to staff the first aid tent at the walk.Support Groups: Rush Oak Park Hospital welcomes support groups to meet on its campus and helps promote these groups via its website and community newsletter. In FY15, Rush collaborated with Women Heart to host a support group and also had groups to support those affected with epilepsy and Parkinson's disease.Cooling/Warming Center: In conjunction with the Village of Oak Park, ROPH offers space in its emergency room for those without adequate air conditioning or heat in instances of extreme temperatures. This effort helps prevent hypothermia and frostbite in extreme cold; and heat stroke and other heat-related illnesses when the thermometers spike. This was particularly important in the winter of 2015, when temperatures dipped below zero on several occasions. Environmental ImprovementsThe Food Scrap and Composting Program: Rush Oak Park Hospital (ROPH) has a program that diverts food waste from landfills, thereby helping diminish greenhouse emissions and promoting better air quality. Planning and implementing the program at ROPH required coordination and communication among a variety of groups. Preliminary research was conducted in partnership with a local non-profit environmental organization called Seven Generations Ahead. This effort enabled hospital staff to create a program tailored for the specific layout and size of the facility. PlanItGreen: This initiative grew out of the Oak Park River Forest Community Foundation's Community works initiative, designed to enhance the vibrancy and quality of life of Oak Park and River Forest. Through PlanItGreen, Seven Generations Ahead is directing the ongoing implementation of the Environmental Sustainability Plan for Oak Park and River Forest. Staff from Rush Oak Park Hospital have partnered with them to help reduce the environmental footprint and advance common sustainability objectives of Oak Park and River Forest through 2020. Leadership Development/Training for Community MembersNurses at Rush Oak Park Hospital participate in professional organizations such the Illinois Organization of Nurse Leaders and Gamma Phi Chapter of Sigma Theta Tau International as committee members to advance the profession of nursing. They also share expertise gained through their experiences in nursing management and bedside care with organizations and students from local universities, such as Dominican University, via presentations and lectures.Workforce DevelopmentCommunity Integrated Transition Education (CITE II) Program: Rush Oak Park Hospital's compassion and commitment to helping those in need includes hosting the Community Integrated Transition Education (CITE II) Program, a young adult transition program that serves students with development disabilities and/or autism who have received a certificate of completion from Oak Park-River Forest High School. Over the years, approximately 300 students have learned not only important job skills at Rush Oak Park Hospital, but they have also gained self-confidence, self-respect and a sense of commitment through their participation in the program. At Rush Oak Park Hospital, students are trained by on-site staff and representatives of ROPH in the areas of environmental services and food and nutrition.Senior workforce development: Rush Oak Park Hospital also serves as a host site for the National Able Network. National Able Network, Inc., is a leading non-profit organization headquartered in downtown Chicago that specializes in providing workforce development programming for individuals, families, and communities. They provide services to businesses in all major sectors and help job seekers of all ages, skills, and income levels achieve economic self-sufficiency through gainful employment.
Part III, Line 2: Rush Oak Park provides a significant amount of uncompensated care to uninsured and underinsured patients, which is reported as provision for bad debts. During FY15, Rush's reported provision for bad debts was a total of $6,295,128 which equates to $1,618,581 at cost based on an overall cost to charge ratio.There can be times when a patient is reluctant to complete a financial assessment to determine their ability to qualify for Charity Care. It is possible that because of these circumstances, there could be a portion of RUMC's bad debt expense that represents patients that do not have the financial ability to pay. We are unable to determine an accurate estimate of this portion of the patient population.
Part III, Line 3 The cost of bad debt expense was calculated In accordance with the Schedule H Form 990 Instructions, derived from Worksheet A, Estimated Bad Debt Expense Uninsured patients are offered a flat discount, as required by the Hospital Uninsured Patient Discount Act of Illinois. These uninsured discounts were not considered In the calculation of the estimated bad debt expense amount, at cost The bad debt expense IS reviewed by management, taking Into consideration historical business and economic conditions, trends In health care coverages and other indicators this assessment is made throughout the year, ensuring the adequacy of the provision by payor category ROPH follows established guidelines for placing certain past due patient balances with collection agencies, In accordance with the hospital's bad debt policy A core component of ROPH's mission is to provide healthcare services to our community including services for which no payment from the patient is anticipated ROPH's financial assistance program is available to all patients. The charity care application IS provided during time of registration or whenever the patient requests it. This is also posted on ROPH's website. Once the application, along with the necessary documentation is submitted, a review will be performed to assess if the patient qualifies for charity care In accordance with the policy. Measures are taken to follow up on incomplete applications by either phone calls or letters. However, there are instances when patients are not willing to submit an application even though they may be eligible. As such, ROPH cannot reasonably estimate what portion of bad debt expense can be attributable to patients eligible under ROPH's charity care policy.
Part III, Line 8 The calculation of the ratio of patient cost to charges was calculated utilizing ROPH's FY 15 As-Filed Medicare cost report and follows the format based on Worksheet 2 of the Instructions to Schedule H Medicare revenues and costs were extracted from the FY15 As-Filed Medicare cost report.
Part III, Line 9b In keeping with ROPH's mission to provide healthcare services to our community, the hospital offers several financial assistance programs to help patients with their hospital bill. If the hospital is aware that a patient qualifies for financial assistance, these accounts will not be referred to a collection agency. It is the policy of ROPH to offer patients payment plan and/or financial assistance when a patient demonstrates financial need. The hospital works with patients to help determine If there are any third party payers which may be available to help the patient meet their obligations The hospital works with patients to determine If they qualify for one of a handful of state and federal programs such as the state Medical Assistance Program or the Social Security Disability program In the discussion of ROPH's provision of charity care to our patient population, there are several factors which must be considered In addition to the charity care number provided for purposes of Schedule H to obtain a full understanding of the breadth of charity provided by the hospital. These factors are outlined as follows. Through utilization of a patient eligibility service, the hospital is extremely proactive in enrolling patients who arrive at the hospital without insurance coverage into various state and federal programs that provide health insurance coverage. The maintenance of this service for our patients has a significant Impact on decreasing the amount of charity care provided In addition to achieving appropriate, available coverage for our patients' medical services, this eligibility service also obtains eligibility for SSI or SSA benefits for applicable patients. Guiding the patient through this often time consuming and arduous process is extremely beneficial to the patient, as once SSI/SSA eligibility is approved, the patient will begin receiving a monthly assistance check that provides a benefit well beyond their health care at ROPH. Due to the process that ROPH and other hospitals must facilitate to prove a patient's eligibility for discounted or free care, the precise amount of charity care often can be Indistinguishable from other categories of uncompensated care. Without the cooperation of the patient in providing appropriate documentation, the hospital cannot correctly distinguish patients who may need the defined charity care policies and appropriately categorize those Individuals as charity care write-offs. Instead, these patient cases are frequently classified as bad debt write-offs due to a lack of support Information. This creates a reported charity amount that is not representative of the true amount of free care provided to low income and indigent patients. Due to the high volume of presumptive charity care cases during FY15, ROPH reversals of discounts to uninsured patients are not included in Schedule H. Accounts do not qualify for bad debt collections if charity care coverage was added to the account. Accounts will not qualify for bad debt If ROPH gives the patient a charity care application.
Part III, Line 4 It is an inherent part of ROPH's mission to provide necessary care free of charge, or at a discount, to individuals without insurance or other means of paying for such care. As the amounts to determined to qualify for charity care are not pursued for collection, they are not reported as net patient service revenue. ROPH provides additional information on the bad debt expense in footnote #2 of the financial statements.
Part VI, Line 2: In addition to the CHNA, community health needs are brought to the attention to Rush Oak Park Hospital through community partners and community projects. For example, local food insecurity was a need identified by a collaborative effort of Oak Park-River Forest Day School, Oak Park-River Forest, High School, Thrive Counseling Center and the Oak Park-River Forest Food Pantry. Other examples of community partners requesting ROPH to address needs: Housing Forward -- anorganization dedicated to transitioning people from housing crisis to housing stability - asked for health education about health needs of the homeless and underserved as well as clean linen services to promote wellness.

Guiding the patient through this often time-consuming and arduous process is extremely beneficial to the patient, as once SSI/SSA eligibility is approved, the patient will begin receiving a monthly assistance check which provided a benefit well beyond their health care at ROPH.To assist the patient in deciding which is the right program for them, ROPH offers the services of Financial Counselors and Billing Customer Service Representatives. These individuals will assist patients in completion of financial application forms, obtaining an estimated cost of anticipated hospital services, providing an explanation and copy of their hospital bill, and notary services. ROPH makes all financial assistance information and policies available on the hospital's website.In order to widely publicize the document, ROPH makes the financial assistance information available in a number of formats. ROPH provides a plain language summary in the Emergency Department as well as the main Registration areas. The plain language summary explains the different types of charity care. In addition, all uninsured patients are provided a financial assistance application when being discharged from the Emergency Department. At our registration sites, it is our policy to have all uninsured patients informed of our financial assistance policy, and we recommend that they speak with a financial counselor. In addition, the policy, application, and plain language summary are available on-line for patients and guarantors to view.
Part VI, Line 4: ROPH specializes in care for the patient population over age 65 in Oak Park (60301, 60302 and 60304) and surrounding communities. These communities include but are not limited to River Forest (60305) and Forest Park (60130). ROPH identified the community it serves by including these surrounding communities and by conducting a patient ZIP code analysis. ROPH's employed physicians' group has offices in Oak Park, Elmwood Park and North Riverside.
Part VI, Line 5: In addition to activities noted in Part V, Section B, Line 11, ROPH provides space free of charge for community support groups (e.g., for those affected by Parkinson's disease, epilepsy and heart disease), those seeking shelter in extreme weather and community meetings. The hospital also is a site for job training young adults with learning disabilities and autism through a collaboration with the Community Integrated Transition Education (CITE II) Program. And the hospital is home to Project Lifestyle Change a free program for those affected by diabetes and pre-diabetes, which is paid for by a grant from the National Diabetes Prevention Program. Clinicians at ROPH teach blood glucose monitoring, exercise and behavior modification as part of this program.

The affiliation between RUMC and ROPH provides patients with access to advanced medical treatments without having to leave their neighborhoods. Part I, Line 3 The CEO's compensation is determined by a related organization.
Part I, Lines 4a-b James Kaese received severence pay of $27,987. RUMC offers a supplemental employee retirement plan to all employees who participate in the executive benefits program and whose compensation exceeds the IRS allocable limit for qualified pension plan. The amounts paid out from this plan were as follows: David A. Ansell,MD -$605,880. The amounts listed in Schedule J,Part II,Column F reflect these distributions less the accrued earnings which were not previously reported. The Rush Executive Retirement Plan(the"Rush ERP or the "Plan")provides supplemental retirement benefits to eligible executives of the Rush University Medical Center(the "Medical Center"). These benefits are in addition to those provided under the Retirement Plan(a tax-qualified defined benefit pension plan), the 403(b) Retirement Savings Plan, and the 457(b) Supplemental Retirement Savings Plan. The Rush ERP is effective January 1,2014, and replaces the Supplemental Executive Retirement Plan(the"SERP"), which was frozen effective December 31, 2013. Any benefits earned under SERP will be preserved. The Rush ERP is a defined contribution plan designed to provide funds to participants that can be used to provide supplemental retirement income. Once vested, the amount contributed plus investment gains (and minus losses) is paid.
Part I, Line 7 Incentive payments are based on a formula. The amounts are calculated after certain performances and operating goals are achieved. The plan provides limited discretionary parameters if needed.
