Form 990, Part VI, Section B, Line 15	The hourly wage of the two staff employees is determined by the Board, currently at $10.00 for second employee and $14.50 for the existing manager. Wendy Leonard was the second employee and Marcia Prater is the manager. The Treasurer/CEO/President receives a monthly salary of $1100.00. Changes in salary are reviewed each year at the annual meeting in February.
Form 990, Part VI, Section C, Line 19	All documents pertaining to the 990 including financial information is available for public viewing in the office located at 157 North Main Street, Suite 31, Edwardsville, Illinois 62025. The financial statements are only on display in the credit union, but form 990 is available upon request. All governing documents are available for members to view and some are on display in the credit union on the front desk.

Explanation:

The manager was hospitalized due to a brown recluse spider bite and major infection in face. Infection was behind eyes as well and eyes were swollen and manager could not see properly. Manager in training could not get it to balance out properly and had to wait for manager to be released from hospital.
