Form 990, Part VI, Line 11b: Form 990 Review Process	The Form 990 is prepared by an independent CPA Firm and reviewed by the CEO and bookkeeper before it is electronically filed.
Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts	Conflict of interest disclosures (COID) are performed annually. Vendors are disclosed within the COID and new vendors are reviewed by the Board for conflict of interest. Also, loan advances for directors who will have a cumulative loan total of $20,000 or more are reviewed by the Board.
Form 990, Part VI, Line 15b: Compensation Review and Approval Process for Officers and Key Employees	The Board of Directors annually reviews and approves the CEO's compensation by utilizing salary publications and compensation surveys. The CEO annually, and as needed, submits to the Board a compensation schedule for staff and also utilizes publications and surveys.
Form 990, Part VI, Line 19: Other Organization Documents Publicly Available	Financial statements are posted on a wall within the office of the Credit Union. Policies are available to the membership upon written request.
