Form 990, Part VI, Section A, line 3	THE MANAGEMENT COMPANY IS QUANTUM LEADERSHIP INC. THEY PERFORM BY OR UNDER THE DIRECT SUPERVISION OF THE ORGANIZATIONS OFFICERS, DIRECTORS, AND TRUSTEES. THERE ARE NO EMPLOYEES AND NO PAYROLL INVOLVEMENT. THEY ARE INVOLVED IN PLANNING, EXECUTING BUDGETS, FINANCIAL OPERATIONS AND SUPERVISING THE EXEMPT OPERATIONS.
Form 990, Part VI, Section B, line 11	The completed IRS Form 990 is delievered to the Management Company and the Organizations Finance Committee for the first review. All interview questions on preparation are submitted to Management Company. The Finance ommittee presents to the Board of Directors for final approval and the Treasurer signs and mails the IRS form 990. All Board Members and Finance Committee are furnished copies of the Form 990 during review, and a copy of the final return is maintained for public inspection, except for Schedule B.
Form 990, Part VI, Section B, line 12c	The Organization has a written policy for conflict of interest. The organizations directors, trustees, and officers are covered by the policy as executed by the Management Company. Using a questionnaire, the disclosure of nformation is obtained which facilitates in the possible review if a conflict of interest situation is identified, suspected or reported. A direct or indirect benefit financially affecting any director, trustee, or officer as they are executing their fiduciary duties to the organization would be addressed. The organization has no employees.
Form 990, Part VI, Section C, line 19	The organization makes the governing documents, conflict of interest policy, and financial statements available to the public upon written or verbal request within a reasonable amount of time. It is available within two weeks at a designated place and time or within 30 days a mailed copy can be sent to the requestor. This form is also available for three years for inspection as well.
