Form 990, Part VI, Section A, line 6	There shall be one class of Members. Membership in Coalition for Physician Well-Being, Inc. (Corporation) is open to health care systems, individual hospitals, professional organizations and individuals who share the values of the Corporation and who are interested in supporting the Corporation's goals.
Form 990, Part VI, Section A, line 7a	The Chairman of the Council (Board of Directors) shall be appointed by the members of the filing organization.
Form 990, Part VI, Section A, line 7b	Except for the following rights retained to the Members, the management and control of the affairs, business, and property of the filing organization shall be vested in and exercised solely by the Council. Rights reserved to the Members are as follows: 1) to approve the assessment of annual dues; 2) to approve any amendments to the filing organization's Articles of Incorporation or Bylaws, and 3) to appoint the Chairman of the Council.
