Our niche in the these regions health care marketplace is as a non-profit multi-specialty provider of integrated medical, dental, and behavioral health primary care services, mostly for working families. In this role, Peak Vista has endeavored to serve populations that are under or uninsured as well as those on public programs. Partnerships are a significant component of Peak Vista's ability to successfully meet its mission. A full service on-site laboratory and pharmacy provide easy access to testing and medications. | FORM 990, PART III, LINE 2 | 2015 Peak Vista Community Health Centers Accomplishments include: Patient Growth Peak Vista served the largest number of patients in our history in 2015 serving nearly 84,000 patients or an average of 1,256 patient visits per day (in 2006 the average was 600). Opening Access to Care - Peak Vistas newest location, Health Center at South Circle, opened its doors for patients in December 2015. Co-located with SET Family Medical Clinics (SET) on South Circle Drive, the new health center is an ideal location for those living in the south central Colorado Springs area. The new health center is made possible by grant funding awarded by the Department of Health and Human Services and clinic space from Centura. Dental Program Upgrades - Peak Vistas nine Dental Health Centers work daily to improve the oral health of our patients. With a generous grant from the Caring for Colorado Foundation and the Colorado Springs Osteopathic Foundation, Peak Vistas dental services was able to convert to digital x-rays and electronic dental records, enabling providers to have better access to patient charts and facilitate continuity of care. Additionally, the dental teams work with patients to create individual treatment plans to meet their needs, often at a reduced fee. Team-based Care in Health Center at Union - With support from multiple grantors, Peak Vista piloted a fully integrated, team-based care model in the Health Center at Union. By design, the model aims to reduce patient office visits while also elevating the patients overall health. Patient needs are addressed in care teams consisting of the patient, a primary care provider, a behavioral health provider, a clinic-based nurse/care manager, a medical assistant, a resource navigator, a pharmacist, a health educator, a dental hygienist, receptionists and customer service representatives. This pilot affords Peak Vista the ability to test various care team models, processes and space requirements to plan for the future health care needs. Analysis of testing results provides valuable insight into costs, outcomes, patient satisfaction and staff satisfaction. Peak Vista launches the Education Health Initiative (EHI) encompassing Peak Vistas Advanced Practice Registered Nurse fellowship, the Family Medicine Residency program and medical student coordination to focus on developing and training new health care professionals. EHI is an effort to build a local solution to the national health care workforce shortage, with the ultimate goal of improving access to quality care in our community. Advanced Practice Registered Nurse Fellowship - September 2015 saw the commencement of Peak Vistas Advanced Practice Registered Nurse (APRN) Primary Care fellowship, a program designed to seamlessly transition recent nurse practitioner graduates from the academic sphere to a clinical setting. The one-year fellowship combines didactic and clinical learning in a team-oriented, patient-centered clinical environment. The inaugural class of APRN fellows consisted of five nurse practitioners. The fellows are scheduled to graduate in fall 2016. Family Medicine Residency Program - Peak Vistas Family Medicine Residency (FMR) is the only primary care residency of its kind in Peak Vistas service area. The three-year program welcomed eight osteopathic doctors in July 2016. The curriculum, program offerings and emphasis of partnership with community agencies and leaders evoked an overwhelmingly positive response. It was also met with strong community support from local hospitals, health care providers and specialty physicians. Residents will learn through patient interaction, bedside teaching and structured educational sessions. Peak Vista received over 230 applications and interviewed 54 candidates for the inaugural class. |

| FORM 990, PART VI, SECTION B, LINE 12C | Peak Vista Community Health Centers' compliance plan and bylaws contain policies on conflict of interest. In order to ensure that conflicts of interest are disclosed, board members and leadership sign a disclosure statement when first appointed or employed and annually thereafter. Human Resources distribute the disclosure statements to leadership and the CEO's staff ensures that the statement is given to the Board of Directors. Any new board member or leadership staff is given education on the conflict of interest policy. If at any time during the year a potential conflict of interest arises, board members are required to inform those charged with approving the transaction of the conflict, disclose any material facts, and excuse themselves from voting. FORM 990, PART VI, SECTION B, LINE 15 Determination of salaries of highly compensated employees at Peak Vista Community Health Centers are determined by making salary comparisons of similar positions at community health centers and other similar agencies in the state, Federal Region VIII, national, and a survey of Mountain States salaries. From this analysis, a proposal is made to the Chief Financial Officer, Chief Nursing and Operating Officer, Chief Medical and Dental Officer, CEO and the Vice President of Human Resources, and the proposal is accepted by them or adjustments are made as appropriate based on budget constraints. The CEOs salary comparison is completed by external independent experts and reviewed and approved by the Board of Directors. |
