| CORE FORM, PART III, STATEMENT OF PROGRAM SERVICE ACCOMPLISHMENTS | St. Luke's Quakertown Hospital ("SL-Quakertown") is a Joint Commission-accredited, not-for-profit, 62-licensed bed acute care hospital located in Quakertown, Bucks County, Pennsylvania. SL-Quakertown provides care primarily to residents of Upper Bucks County, Upper Montgomery County and the southern sections of Lehigh Valley, Lehigh County. SL-Quakertown annually provides care for more than 3,220 observations and admissions, 74,530 outpatient visits and 14,720 emergency department visits. SL-Quakertown is recognized by the Internal Revenue Service ("IRS") as an Internal Revenue code section 501(C)(3) tax-exempt organization. Pursuant to its charitable purposes, SL-Quakertown provides medically necessary healthcare services to all individuals in a non-discriminatory manner regardless of race, color, creed, sex, national origin, religion or ability to pay. Moreover, SL-Quakertown operates consistently with the following criteria outlines in the IRS Revenue Ruling 69-545: 1. St. Luke's Quakertown Hospital provides medically necessary healthcare services to all individuals regardless of ability to pay, including charity care, self-pay, Medicare and Medicaid patients; 2. St. Luke's Quakertown Hospital operates an active emergency room for all persons; which is open 24 hours a day, 7 days a week, 365 days per year; 3. St. Luke's Quakertown Hospital maintains an open medical staff, with privileges available to all qualified physicians; 4. Control of St. Luke's Quakertown Hospital rests with its Board of Directors and the Board of Directors of St. Luke's Health Network, Inc., d.b.a. St. Luke's University Health Network. These boards are comprised of a majority of independent civic leaders and other prominent members of the community, as well as physicians on the Hospital/Network medical staff; and 5. Surplus funds are used to improve the quality of patient care, expand and renovate facilities and advance medical care; programs and activities. The operations of SL-Quakertown, as shown through the factors outlined above and other information contained herein, clearly demonstrate that the use and control of SL-Quakertown is for the benefit of the public and that no part of the income or net earnings of the organization is for the benefit of any private individual nor is any private interest being served other than incidentally. Founded in 1930, SL-Quakertown was acquired by nationally recognized St. Luke's University Health Network ("Network") in 1995. The Hospital provides patients with access to more than 370 physicians across 47 medical specialties. The Hospital achieved an "A" rating for four consecutive years (2011 - 2014) as determined by its Leapfrog Hospital Safety Score. Additionally, in 2015, SL-Quakertown received the QUEST Citation of Merit for High-Value Healthcare at the Annual Premier Breakthroughs Collaborative in National Harbor, MD. This award is given to those organizations that have achieved Top Performance Threshold (TPT) for five of the seven QUEST domains (Mortality, Cost and Efficiency, Inpatient and Outpatient Evidence Based Care, Mortality, safety, Patient & Family Engagement). SL-Quakertown is also Chest Pain Accredited by the Society of Cardiovascular Patient Care, as well as a NICHE (Nurses Improving Care for Health System Elders) designated hospital. Currently, SL-Quakertown is also pursuing Level IV Trauma Certification from the Pennsylvania Trauma Systems Foundation, Primary Stroke Certification from the Joint Commission, and Certification as a Hernia Center of Excellence from the Surgical Review Corporation. The Hospital's specialty services include the following: - Behavioral Health - Cardiology - Dialysis and Hemodialysis - Emergency Services - Endocrinology - Infusion Services - Nephrology - Neurology/Neurosurgery - Oncology/Hematology - Orthopedics, joint and muscle disorders - Otolaryngology/ENT - Pain Management - Pulmonology - Radiology (advanced) - Sleep Disorders - Sports Medicine, Physical and Occupational Therapy, Rehabilitation - Surgery (general and laparoscopic) - Urology - Vascular Services - Women's Health - Wound Management The Network has invested approximately $75 million in technical and facility improvements at SL-Quakertown since 1995, including outpatient centers which offer advanced technology and physician services, further enhancing patients' ability to easily access state-of-the-art healthcare. Recent major improvements include: - a 22,905 sq. ft., $8.5 million St. Luke's Bone & Joint Institute; - a 6,620 sq. ft., $775,000 renovation of St. Luke's Heart & Vascular Center; - a 21,728 Sq. ft., $8.7 million St. Luke's Upper Perkiomen Outpatient center; and - an urgent care center which opened in November 2014. Additional improvements included nearly $1 million in new patient monitoring equipment in FY '13 and in FY '14, installation of new cardiac monitors and new HEV boards, purchase of life support medical simulator, upgrade to barcode scanners and purchase of a new PICC line insertion system The Hospital has significantly expanded medical expertise through the addition of the following services: - Direct access to orthopedic surgeons providing the most advanced diagnosis, treatment and rehabilitation for patients with musculoskeletal disorders and injuries; - Direct access to highly trained cardiac team which includes experienced and skilled cardiologists, cardiac and vascular surgeons, interventional radiologists and electrophysiologists; and - Direct access to oncologists and a wide range of oncology specialists. SL-Quakertown annually reaches more than 10,000 people through its community outreach endeavors. The Hospital offers a variety of free screenings/services for community-run events throughout the year. Community outreach includes, but is not limited to, the following: - utilized numerous media outlets to educate the community about health issues that may impact them; - hosted numerous community events such as Tail on the Trail, Harvest Fest, Monster Mash and 5K Dash and St. Luke's Land and Lake Advanture Event as well as multiple community leader meetings; - participated or sponsored more than 25 community events such the Upper Bucks YMCA 5K, Upper Bucks YMCA Healthy Kids Day; the Upper Perkiomen YMCA Senior Citizen Prom, and Relay for Life's National Night Out event; - hosted Network's dental van traveled to area high/middle schools in Bucks County and onsite at St. Luke's Quakertown Hospital providing dental visits to at-risk, uninsured students; - provided free screenings and diagnostic services to free clinic patients; - supported the Open Link: a multi-service agency offering programs, resources and services designed to meet the basic needs of individuals and families in the Upper Perkiomen Valley; - conducted CPR and first aid courses; - provided educational programs on various topics and participated in various community health fairs; and - provided flu and/or pneumonia immunizations to community residents.
