Form 990, Part VI, Section A, Line 6	Every person who opens an account with the Credit Union and deposits $25 is considered a member.
Form 990, Part VI, Section A, Line 7a	Elections for expiring term board members are held annually at the credit union annual membership meeting during the 1st quarter of each year.
Form 990, Part VI, Section B, Line 11b	Members can request a copy of the return in writing based on member access to records policy or can access the return online.
Form 990, Part VI, Section B, Line 12c	Conflict of Interest Policy: Any employee, director or committee member from this credit union who is offered, receives or anticipates receiving anything of value will disclose it to the credit union supervisory committee or designated official for this purpose. The supervisory committee will keep written records of the disclosures. Annual CPA audit: Employees are question about any conflict of interest and any issues that may arise are turned over from the CPA firm to the supervisory committee.
Form 990, Part VI, Section B, Line 15	Peer salary survey data and performance evaluations are used to determine compensation. Compensation is determined and reviewed annually for the CEO by the Board of Directors. Senior Management employees receive annual evaluations by the CEO.
Form 990, Part VI, Section C, Line 19	The credit union has a Member Access to Records Policy. Requests are made in writing and forwarded to the CEO for review prior to releasing information. Financial Statements are posted monthly at each branch location and the public has access to review quarterly call report information online at www.NCUA.gov.
