Tax Indemnification and Gross Up Payments are provided to CEO and EVP for imputed interest calculated on loan from Credit Union for payment of annual Split-Dollar Life Insurance premiums. Health and Social Club membership is provided to Officers.
Schedule J, Part I, Line 4	The CEO participates in a 457(f) and a 457(b) plan. A distribution from the 457(f) plan was paid to the CEO in 2015.
Schedule J, Part II	Column F "Compensation in Column (B) reported as deferred on prior Form 990" for Barry Heape, President/CEO, includes $98,492 in deferrals for FY 2011 and FY 2012, during which time the Credit Union was federally chartered and was not required to file Form 990.

Schedule L, Part II	The Split-dollar agreements for the CEO and EVP are "collateral assignment" agreements. Annual premiums are distributed from a 10 year paid-up Advance Premium Deposit Fund that was fully funded by the Credit Union. The loans to the employees are non-recourse collateral loans.
