Missions United, Inc. is a Continuing Care Retirement Community (CCRC) owned by two Montana not-for-profit corporations with equal (50%) membership interests: St. Vincent Healthcare and St. John's Lutheran Ministries, Inc. St. Vincent Healthcare, (St. Vincent) founded in 1889, is one of the largest tertiary hospitals in Montana and is a Restricted Affiliate of the Sisters of Charity of Leavenworth Health Services Corporation, a Kansas not-for-profit corporation. St. John's Lutheran Ministries, Inc. (St. John's) is a Montana not-for-profit corporation formed in 1960. St. John's has grown to become one of the largest providers of senior services in the State of Montana. What started as a 115 unit HUD sponsored retirement facility, has now become a continuum of care providing low income housing, skilled nursing, assisted living, dementia care, independent living, adult daycare services, childcare, rehabilitation and home health services, child adoption services, and numerous other activities to enhance the lives of those we serve. St. John's is a ministry of 22 Lutheran (ELCA) Congregations. Both St. Vincent and St. John's are tax exempt under the exemptions of the Roman Catholic Church in the United States and the Evangelical Lutheran Church of America, respectively. These two organizations came together with a common mission of meeting an unmet need through the creation of Missions United. Missions United, Inc. opened Montana's first state of the art CCRC in Billings on January 5, 1998. Missions United, Inc. is an expression and extension of the church's mission. In public worship and in private settings, the following events happen: God's saving word is proclaimed, the Sacraments are administered, prayers and devotions are offered, pastoral care and counseling is provided, and works of charity and benevolence are offered. Missions United is not just an expression and extension of the church; it is the church at work in this place and in the world.In our 18th year, Missions United provided many community benefits. Included in our effort to promote benefits to our residents and provide service to others in the world were the following:1. Missions United completed our 2nd full year of funding an "attended transportation" program for marginalized elderly in the Billings area. This service is provided by Big Sky Senior Services, a local nonprofit organization that sets its service charges on a sliding scale according to ability to pay and primarily serves lower income elders. The Board approved annual funding of up to $50,000 that will cover the cost of serving those who cannot afford these services. This program served 109 people and the numbers continue to grow. 2. $781,268 was distributed equally to the sponsors to support the charity care and Medicaid services that the sponsors provide to low-income elders and other marginalized elders. Sponsors will be reporting back annually to the Missions United Board of Directors on the use of those funds. 3. Missions United has intentionally not fully occupied its assisted living apartments to keep two respite rooms available to serve area seniors for short periods of time while long-term decisions can be made. These rooms provided care for about 392 days, resulting in a loss of $47,671. Missions United pays monthly bed-hold fees of $36,500 to maintain availability to other levels of care via the St. John's Continuum of Care.4. Staff encourages and facilitates the residents to volunteer in the community via formal programs, sponsorship, and transportation.5. The 2015 Summer Concert series held its 18th season on the Missions United/St. John's campus. We hosted over 9,000 people as we provided five community events at no charge to our residents, friends and neighbors! Additional highlights include: - Distributed $5,600 to local non-profits. - Raised another $6,900 for Lutheran Social Services and Resident Needs Fund: free-will offering. - Provided countless smiles and memories for thousands.6. Provide space to other organizations as needed - groups such as retired teachers, non-denominational worship and bible studies, Billings Public Library, Arrowhead school events, recitals, Sons of Norway and P.E.O.7. Events and educational awareness programs were held for Tumbleweed, a program supporting homeless or displaced teenagers in the Billings community, Missions United and Residents have "adopted" this group and are a regular drop box" location with all donations being delivered weekly. Peace Lutheran Church provides a community food pantry, and is also supported by this drop box" donation site.8. Missions United paid and pledged $5,000/year for 5 years to Leading Age, a National Association for nonprofits, for efforts in developing new technologies to support the nation's growing senior population.9. In 2015, Missions United provided free health assessments, education, and/or guidance to approximately 788 seniors and their families including information about our services beyond that of Missions United. This guidance helps their understanding and preparation for senior living and their understanding of other benefits available to them such as Veterans Aid and Attendance, Medicaid Waiver, Medicare A, B, and D, and/or improving their current situation by identifying opportunities and health needs while still living at home. Once residents, they find additional comfort in the fact that they have a plan for their future when their medical needs change. 10. Invested in a community benefit tracking software that will help better define, track, and store all of our efforts, costs, and people served as a benefit to the larger community. Missions United provided $974,075 of community benefit to 16,760 people in 2015.The goal of the Mission Ridge community is to provide housing, as well as health and financial security for our elders, while simultaneously preserving their privacy and self-sufficiency to the fullest extent possible. The senior living community consists of 120 independent living apartments and 62 assisted living apartments, and in conjunction with the existing long-term care facility (St. John's), provides a full continuum of care to residents. Missions United is providing for the physical, spiritual, social, and emotional wellbeing of its residents by offering housing in an environment designed specifically for the needs of its residents. We regularly have people coming to us whose needs are not being met during transition. Missions United residents are guided and nurtured through those transitions, lessening the hardship and crisis for themselves and their families. Financial security is being provided to the residents of the community by offering quality services to residents at the lowest possible cost. Missions United has adopted a policy, set forth in the Residency Agreement, providing financial assistance to residents who become unable to pay their monthly service fee or other charges. No residents have been, or will ever be, discharged because of an inability to pay. Existing resident deposits are available to assure such financial assistance will be available. The average deposit on a Missions United apartment at December 31, 2015 was $110,555. This is more than 55 percent lower than the 2014 average single family home sale in Yellowstone County of $251,445 (according to realestatemontana.com). Missions United, Inc. contracts for and provides access to a continuum of long-term care services available to elderly residents of middle class financial means.A long-term care benefit is offered to all independent living residents to assist in preserving financial security. The benefit provides $500 per month for three years if transition occurs to assisted living and $750 per month for two years if transition occurs to skilled nursing. The maximum benefit is $18,000, which may be any combination of assisted living or skilled nursing services. During 2015, 7 residents who required either assisted living or skilled nursing care received $45,779 in long-term care benefits. From date of inception (1998), residents have received $1,598,233 in long-term care benefits.Missions United is first and foremost a Christian community. Our mission is God's Mission; the work of healing a broken world, the work of healing the sick, and binding up the broken hearted. This mission is accomplished through serving Christ by creating abundant life opportunities and providing quality services for seniors.

Part X, Line 2: The Company is organized as a Montana nonprofit corporation and has been recognized by the Internal Revenue Service (IRS) as exempt from federal income taxes under Internal Revenue Code Section 501(c)(3). The Company is annually required to file a Return of Organization Exempt from Income Tax (Form 990) with the IRS. The Company has determined it is not subject to unrelated business income tax and has not filed an Exempt Organization Business Income Tax Return (Form 990T) with the IRS. The Company believes it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The Company would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.
