	MDwise Marketplace, Inc. offers qualified products on the Federally Facilitated Health Insurance Marketplace, formed with the introduction of the Affordable Care Act, to cover individuals who do not qualify for Indiana Medicaid programs. These plans are subject to individual premiums and cost sharing for residents ages 0-64. Marketplace administers three different levels of plans through the exchange: Gold, Silver, and Bronze. Depending on the individual income levels, individuals may qualify for federal subsidies. The company was responsible for approximately 19,600 members at December 31, 2015.

Form 990, Part VI, Line 7b Decisions requiring approval by members or stockholders	Indiana University Health ("IU Health") shall appoint four members of the board of directors and The Health and Hospital Corporation of Marion County ("HHC") shall appoint three members of the board. The compliance officer of the organization reviews each completed statement and is present at board meetings to enforce compliance with the conflict of interest policy.
