THE EXECUTIVE ASSITANT LEFT TO PURSUE ANOTHER OPPORTUNITY, AND SHORTLY THEREAFTER, THE BOARD OF DIRECTORS RELEASED THE EXECUTIVE DIRECTOR, WHO WAS IN CHARGE OF THE FINANCIAL RECORDS. THE ORGANIZATION SIMULTANEOUSLY WENT THROUGH VARIOUS OFFICER CHANGES DUE TO TERM LIMITS, INCLUDING A NEW BOARD CHAIR AND VICE CHAIR. DURING THIS PERIOD OF CHANGE, AND DUE TO TRANSITION IN EVERY CRUCIAL ROLE IN THE ORGANIZATION, INFORMATION WAS NOT AVAILABLE TO FILE IN A CORRECT AND TIMELY MANNER.
