Form 990, Part VI, Section A, line 4	The Credit Union amended it By-laws during 2015 to decrease the number of voting Board of Directors positions from eight to seven members.
Form 990, Part VI, Section A, line 6	Community Choice Credit Union (the Credit Union) is a member-owned financial cooperative operated for the benefit of its members. The membership consists of those employed by a member of the Commerce City Business and Professional Association. Physicians, nurses, medical staff, technicians, employees, volunteers and other persons that work at or are employed by any hospital or hospital owned medical center or clinic, medical plaza, rehabilitation center or clinic, psychiatric center, or hospice center, located in the counties of Adams, Arapahoe, Boulder, Denver, Douglas, and Jefferson, in the state of Colorado are eligible for membership. Employees of firms that maintain an office at the Anthem Building located at 700 Broadway in Denver. Immediate family members of existing Community Choice Credit Union members are eligible for membership. This will include family members related by blood, marriage, adoption, or two or more persons living in the same residence who maintain a single economy union. Anyone employed by one of our Select Employee Groups. Anyone who lives, works, or worships in Montbello & Commerce City is eligible for membership.
Form 990, Part VI, Section A, line 7a	The members of the credit union annually elect the Board of Directors to serve for multiple-year terms by mailing in ballots or voting at an annual meeting.
Form 990, Part VI, Section A, line 7b	Various decisions made by the Board of Directors for a credit union are subject to approval by the members of the credit union as set forth in NCUA rules and regulations and applicable state regulations.
Form 990, Part VI, Section B, line 11	The completed Form 990 is reviewed by the Chief Financial Officer prior to filing. The Chief Financial Officer is authorized to sign and file the return. The Board of Directors does not review.
Form 990, Part VI, Section B, line 12c	The security matrix on the data processing system prohibits employees and officials from initiating transactions on all accounts listed on their conflict of interest disclosure statement, which is updated annually. A written acknowledgment is filled out each year by the officers, directors, and employees which could give rise to potential conflicts within the organization. Any potential issues are discussed with the key officers and those charged with governance for resolution.
