Form and Line Reference Explanation
PART V, SECTION B, LINE 5 Southcoast works with a number of community partners to obtain input on community needs, gaps in services and health disparities that exist within Southcoast communities. In 2015, Southcoast worked with the following: 1. Partners for a Healthier Community, a community coalition serving Greater Fall River, which undertook a community-wide needs assessment. Southcoast staff served on a number of needs assessment committees which solicited extensive community input through surveys, focus groups and community discussions. 2. Southcoast worked with the Wareham Coalition to Prevent and End Homelessness to conduct a series of Focus Groups on homelessness in the town of Wareham. These included meetings with homeless residents and agencies serving the homeless. 3. Southcoast solicited input from over 100 community agencies, faith-based organizations and community leaders who are involved in providing behavioral health services. The purpose of this needs assessment was to identify needs and gaps in a range of behavioral health services. This included surveys, key informant interviews and presentations to community groups. 4. Southcoast worked with the Greater New Bedford Allies for Health and Wellness (GNB Allies) Health Equity Committee to research and assess health inequities in the Greater New Bedford region, including the impact of social determinants of health. The result of this research was targeted outreach for programs such as cancer screenings, health insurance enrollment and other health issues.
PART V, SECTION B, LINE 7A AND 10A https://www.southcoast.org/wp-content/uploads/2016/05/2015-southcoast-comm unity-benefits.pdf PART V, SECTION B, LINE 7D Southcoast shared its needs assessment with a regional coalition, Partners for a Healthier Community, which based a more localized, Fall River area needs assessment on Southcoast's broad health needs assessment findings. Southcoast also became part of Health Data Southcoast, housed at the University of Massachusetts - Dartmouth, which houses a range of health, demographic and social data on a web site that can be accessed by the public. Southcoast's regional health needs assessment is often used by a range of community agencies and other partners for grant proposals and joint planning for health and social programs.
PART V, SECTION B, LINE 11 As part of Southcoast's annual Community Benefits process, Southcoast creates a community health improvement plan that is based on Southcoast's needs assessment and is formulated in collaboration with community partners. Southcoast identifies target audiences on an annual basis and also jointly prioritizes and coordinates services that will meet the specific needs of Southcoast communities. In 2015, in collaboration with Southcoast's partners, Southcoast jointly set 49 community health improvement goals and were able to reach 89 percent of these goals. Southcoast reports on it's progress annually and shares this report with the community. Southcoast also establishes a community benefits budget on an annual basis.
PART V, SECTION B, LINE 22D SouthCoast used the financial guidelines established by the Commonwealth of Massachusetts for determining eligibility for financial assistance, including MassHealth and the Health Safety Net. For patients identified between 200-400% of the Federal Poverty Income Guidelines, SouthCoast provided a sliding scale discount. PART II (1) Southcoast engages in a number of community building activities that promote infrastructure improvement in communities and the development of policies and programs that address social determinants of health such as housing, education and workforce development. Southcoast has provided financial contributions to support infrastructure improvements in community parks, that help improve easy access to physical activity. Southcoast has also provided leadership training and support on health advocacy issues such as regulations to limit tobacco access. Southcoast leads, or plays a key role, in a number of local coalitions that promote wellness and collectively work to address pressing health issues identified in Southcoast's needs assessment. Southcoast is the co-leader of a coaltion, Voices for a Healthy Southcoast, that works to promote and support system and environment change that addresses healthy nutrition, improved access to physical activity and a decrease in high rates of smoking.

PART VI, LINE 3 Southcoast posts notifications of insurance eligibility in multiple languages throughout all of its inpatient and outpatient facilities. Southcoast also utilizes outreach brochures, displayed in our facilities and also distributed at a number of health inurance outreach events and also through regulary outreach on our Southcoast Health Van. In 2015, the Southcoast Patient Access Department provided outreach to tens of thousands of residents and enrolled over 7100 residents in health insurance and other subsidies.
PART VI, LINE 4 Southcoast Health system serves a diverse region along the South Coast of Massachusetts, including several urban centers with a number of demographic challenges, and rural and suburban towns. Perhaps the most significant demographic trend in the region is its changing racial makeup; from 2000 to 2010 the minority population increased by 5.6 percentage points region-wide, 8.2 percentage points in Fall River, and 11.0 percentage points in New Bedford. Hispanics account for a significant portion of this change; the number of Hispanics increased by 7.3 percentage points region-wide from 2000 to 2010, and by 4.1 percentage points in Fall River and 6.5 percentage points in New Bedford. Both cities also have pockets of undocumented immigrants, including South and Central Americans in New Bedford, although estimates vary widely as to the size of these groups. Portuguese speakers are being supplanted by Spanish speakers in the regions cities, although some Portuguese-only speakers remain, particularly in Fall River and New Bedford but also in some of the regions suburbs such as Acushnet, Dartmouth, Fairhaven, and Westport. The Southcoast has one of the lowest levels of educational attainment of any region in Massachusetts. The urban areas of Fall River and New Bedford have particularly low rates of educational attainment, although many of the regions suburban towns also have average educational attainment levels that are below the state average. The region and its cities have made gains since 1980 in terms of the percentage of residents who have earned a high school diploma but still lag the state average considerably in terms of the percentage of residents with 4-year college degrees or above. The region's health is affected by the physical conditions of the South Coast. This includes access to amenities that facilitate healthy eating and active living, air quality, and sources of environmental contamination. The analysis shows that Southcoast residents face environmental factors that may be a detriment to their health. For example: -Residents of both Bristol and Plymouth counties have access to fewer grocery stores and supermarkets per capita as compared to Massachusetts as a whole (a combined rate of 17.5 per 100,000 residents, versus 19.8 statewide). -Significant portions of Fall River and New Bedford are classified as food deserts where most residents live over 0.5 miles away from a grocery store or supermarket, while several neighborhoods of Acushnet, Fairhaven and Wareham have no access to such amenities within one mile. -While fresh, healthy, affordable food can be hard for the regions residents to obtain, fast food options are abundant. In Bristol County, there are 65.6 fast food establishments per 100,000 residents and 60.0 per 100,000 in Plymouth County. While these rates are lower than that of the state (71.9), there are nevertheless far more fast food establishments than there are grocery stores and supermarkets. -Southcoast residents also have less access to recreation and fitness facilities than Massachusetts residents as a whole. In Bristol County there are 13.7 such facilities per 100,000 residents versus 15.9 statewide. Plymouth County residents have comparatively better access with 18.0 facilities for every 100,000 residents. -The Southcoast is home to a number of sites that contain and/or generate contaminants that can negatively affect residents health. Fall River is home to 427 contaminated sites, 71 of which present sufficient hazards as to limit activity on and use of these parcels; in New Bedford, 78 out of 583 brownfield sites are so limited. Per capita and median household income levels in the region are below state averages and Fall River and New Bedford have some of the highest poverty rates in the state and account for the majority of the regions poor in absolute numbers. Poverty and income are reflected in wages; there exists a significant wage gap between the Southcoast and the state and this gap continues to widen. The regions changing racial makeup and continued socioeconomic struggles place unique stresses on healthcare delivery, particularly in addressing health disparities based on race, income, and education.
PART VI, LINE 5 Southcoast each year invests millions in direct services and programs that are designed to address pressing health issues in the Southcoast region and improve the communities health. Southcoast works with hundreds of community partners to adopt best practices in community benefits needs assessment and planning and implementation. Southcoasts Community Benefits Strategic Action Plan was first formulated in 1998 as a result of an extensive needs assessment and is updated annually. Southcoast's current plan is based on it's most recent major needs assessment, which was conducted and completed in 2012-2013 and is updated on an annual basis. Southcoast action plan has traditionally focused on three regional priority health issues. -Reduction of the high rate of cardiovascular disease in the Southcoast region, as well as other chronic diseases, such as diabetes and asthma. -Reduction in the incidence of youth risk behaviors such as teen violence, high rates of teen pregnancy and substance abuse. -Improving access to health care. Additional areas of focus, as a result of recent needs assessment data, include: -Expanding cancer screening and education, with a particular focus on reducing health disparities. -Addressing health disparities that exist in the Southcoast region among certain racial, ethnic and demographic groups. -Advocacy and program development that addresses system and environment change, both at Southcoast hospitals and in the community. This advocacy aims to -increase healthy lifestyle options and decrease risk factors, such as a high rate of smoking, lack of access to healthy foods and physical inactivity. Southcoast's efforts focus on vulnerable populations that face considerable barriers to adopting a healthy lifestyle. -Addressing homelessness in Wareham, where the counts of unsheltered homeless approach those in the much larger cities in the Southcoast region. -Addressing behavioral health issues that include substance abuse and mental health. Southcoast serves a large population with behavioral health issues which impact Southcoast Emergency Departments and the regions only inpatient psychiatric unit. Also, Southcoast regional behavioral health system is fragmented and poorly coordinated. -Behavioral health issues that include substance abuse and mental health issues. Southcoast serves a large population with behavioral health issues and poorly coordinated care impacts Southcoast Emergency Departments and the regions only inpatient psychiatric unit located at Southcoast's St. Lukes site. Needs assessment shows Southcoast regional behavioral health system is fragmented and poorly coordinated. Southcoast community benefits activities in 2015 included: -Extensive and collaborative work on smoking cessation and prevention that encompassed regulatory system and environment change along with education and clinical support for smoking cessation. As a result, this year Southcoast witnessed a 10 percent reduction in smoking in Greater New Bedford. This is the first significant reduction in smoking in several decades. -Reaching vulnerable residents with over 21,000 health screenings for more than 3300 people. This included screenings for cardiovascular disease, cancer, diabetes and stroke and targeted our most vulnerable and under-served populations. -Education on teen pregnancy and other youth risk factors that reached over 4000 teens in 12 area schools and an after school drop in center at our RAPPP offices in New Bedford. -Targeted outreach that helped over 9000 residents obtain or keep their health insurance. -Collaboration and programming to address health equity issues in our region by working to increase the capacity and training of community health workers, particularly those serving residents who face cultural, linguistic and economic barriers to health care. In addition to increasing capacity in our communities, we also worked to establish clinical to community linkages for CHWs and explore successful methods of integrating CHWs into clinical care teams. -Coalition building and engagement that helped make meaningful connections across both communities and our region. Programs that were part of our community benefits plan in 2015 include: Health access outreach. Health advocates (intervention for substance abuse.) Cardiac prevention and stroke outreach. Diabetes education and outreach. Smoking cessation and prevention. RAPPP (Responsible Attitudes Toward Pregnancy, Parenting and Prevention) and associated youth outreach programs. Maternal/child health outreach. Southcoast Health Van. Health Equity Project. Voices for a Healthy SouthCoast. Cancer screenings and outreach. Emergency preparedness. Wareham Leadership Council to End and Prevent Homelessness. Southcoast Healthy Housing and Workplace Initiative. The Southcoast Community Benefits Program is under the overall direction of a Community Benefits Committee that meets six times annually to review and advise on activities and expenditures related to community benefits. This committee reports to the Southcoast Hospitals Group Board of Trustees and is chaired by a trustee. The committee includes Southcoast leadership and staff, along with representatives from the various communities served by Southcoast Hospitals. Many of the Southcoast community members have expertise in matters concerning the health and welfare of the community and are active members of local and regional coalitions. This board represents the diversity of our region, with members who are active leaders in minority communities including the Cape Verdean, Hispanic and Portuguese communities. Advisory groups, comprised of community members and hospital staff, plan and carry out activities related to Southcoasts major community benefits initiatives. Also, a number of Southcoast staff participate and provide leadership in local and regional coalitions, including Community Health Network Areas (CHNAs) in Fall River and New Bedford, the Wareham Community Services Collaborative, regional coalitions such as Voices for a Healthy SouthCoast and Mass in Motion, a regional Worksite Wellness Collaborative, Health Access,Health Equity and Youth Empowerment Task Forces in Greater New Bedford and Fall River and substance abuse and mental health coalitions. Community benefits activities by Southcoast staff are organized through an internal Community Benefits Task Force that meets monthly to plan and coordinate programs and activities. This team consists of representatives from departments that regularly engage in outreach in the community, including staff from our Southcoast Health Van, Social Services, Stroke Outreach, Diabetes Management, Behavioral Health Services, Patient Access Services, Cancer Outreach, Smoking Cessation, our Youth Risk Behaviors program and others. Senior management responsibility for the Community Benefits Program rests with Southcoasts Vice President of Marketing and Government Affairs, who also serves as a member of the Community Benefits Committee. The Community Benefits Manager, who reports to the Government Affairs, Strategy and Planning Division, manages Southcoasts day-to-day community benefits activities and leads the internal Community Benefits Task Force. Southcoast also conducts regular updates and presentations on community benefits activities to Southcoast leadership through Directors and Managers/Supervisors meetings at all three hospital sites along with presentations on community benefit activities which are periodically made to all levels of employees. Information is also presented through articles in our internal hospital publications, Coastlines and The Wave, in a community newsletter, Your Health Matters, and in our monthly community electronic newsletter, Southcoast eNews. Information is regularly shared with the community through collaborative meetings and forums.
