CASE MANAGERS SUPPORT INDIVIDUALS TO OVERCOME OBSTACLES THROUGH ASSESSMENT, WELLNESS PLANNING, EMERGENCY FINANCIAL ASSISTANCE AND REFERRALS TO HEALTHCARE, DENTAL CARE, PSYCHOTHERAPY, TREATMENT FOR ADDICTION AND HOUSING. INDIVIDUALS WHO ENTER MEDICAL TREATMENT, REMAIN ADHERENT ON ANTIRETROVIRAL MEDICATION AND HAVE STABLE LIVING SITUATIONS ARE MORE LIKELY TO SUPPRESS OR CONTROL HIV. THE VIRAL SUPPRESSION OF HIV IS CLINICALLY REFERRED TO AS REACHING AN UNDETECTABLE VIRAL LOAD. RESEARCH HAS SHOWN THAT PERSONS WHO HAVE AN UNDETECTABLE VIRAL LOAD HAVE BETTER HEALTH OUTCOMES AND ARE LESS LIKELY TO INFECT OTHERS. FROM 2012-2015, BCAP CONDUCTED AN EVALUATION OF DATA THAT REVEALED 75% OF PERSONS LIVING WITH HIV ENROLLED IN MEDICAL CASE MANAGEMENT IN THE STUDY'S SAMPLE WERE ABLE TO REACH AN UNDETECTABLE VIRAL LOAD. IN 2015, OF THE 1.2 MILLION PEOPLE LIVING WITH HIV IN THE U.S., ONLY 25% REACHED AN UNDETECTABLE VIRAL LOAD. |

YOUTH IN HIGH SCHOOLS ARE TRAINED AS SEXUAL HEALTH PEER EDUCATORS TO INCREASE AWARENESS ABOUT SEXUAL HEALTH AND MEDICALLY ACCURATE INFORMATION ABOUT HIV AND AIDS TO THEIR PEERS. THE SEXUAL HEALTH AND AIDS PREVENTION EDUCATION (SHAPE) PROGRAM WAS A COLLABORATIVE EFFORT WITH BOULDER VALLEY WOMEN'S HEALTH CENTER. BCAP'S ATLAS PROGRAM FOCUSES ON HIV PREVENTION AMONG MEN WHO HAVE SEX WITH MEN (MSM). VOLUNTEERS IN ATLAS MODEL KNOWING THEIR OWN HIV STATUS AND PROMOTING SAFER SEX PRACTICES. IN 2015, ATLAS VOLUNTEERS WORKED OVER 3,850 HOURS AT PUBLIC EVENTS, COMMUNITY WORKSHOPS AND ONE-ON- ONE SESSIONS TO DISTRIBUTE RISK REDUCTION MATERIALS, ENCOURAGE TESTING AND SHARE INFORMATION. SINCE MAY 2013 BCAP HAS PROVIDED SYRINGE ACCESS SERVICE TO INDIVIDUALS IN BOULDER AND THE SURROUNDING AREA. BCAP DISTRIBUTED OVER 23,000 SYRINGES TO PERSONS WHO INJECT DRUGS IN 2015. OVER 125 INDIVIDUALS WHO INJECT DRUGS CAME TO OUR AGENCY FOR THE "WORKS" PROGRAM, WHICH PROVIDES SYRINGE ACCESS AND HARM REDUCTION EDUCATION AND SUPPLIES, SO THEY DO NOT CONTRACT HIV OR HEPATITIS C AND TO LEARN HOW TO PREVENT OVERDOSE FROM OPIATE DRUGS. |
