Pt VI, Line 11b	Organization's process to review Form 990. Form 990 is reviewed annually by the executive committee each November. The executive committee has been delegated authority by the Board of Directors to act on behalf of the full board on the 990.
Pt VI, Line 12c	Enforcement of conflicts policy. The written conflict of interest policy was adopted in December, 2008 and revised in 2009. Additionally, a written whistleblower and document retention and destruction policy were adopted in 2009. These forms are signed by all board members and the president.
