Form 990, Part VI, Section B, Question 11B	The sole controlling member's accounting management works with the auditor/tax preparer to provide and review the form 990 data and information. The chief financial officer reviews and approves the draft 990, then presents it to the finance committee for review. Following their review, the finance committee provides a copy of the draft 990 to the governing board prior to filing.
Form 990, Part VI, Section B, Question 12c	SEAM issues the conflict of interest policy to its board members annually. They must sign a statement that they have received the policy, understand it and will comply with it. Employees of the sole controlling member review the conflict of interest policy upon hire and must sign a similar statement at that time.
