Form 990, Part VI, Section A, Line 6	The credit union has members. Membership is restricted to those individuals that qualify for membership under the credit union bylaws as approved by the members and regulated by the Arizona Department of Financial Institutions.
Form 990, Part VI, Section A, Line 7a	The members elect the board of directors from the general membership. The election is held yearly during the Annual Meeting of the Membership. Members are given notice of the Annual Meeting at least 30 days prior to the meeting. Form 990, Part VI, Section B, Line 11b	The board of directors reviews the completed and filed return at a regular board meeting. Financial information contained within the return is reviewed at monthly board meetings.
Form 990, Part VI, Section B, Line 12c	At the first regular board meeting after the Annual Meeting, the board reviews a policy which states that all board members and employees must disclose any conflict of interest. Individual board members each state that there are no conflicts. New volunteers are required to sign a form disclosing any conflicts of interest during the vetting process for election to the board or appointment to a committee.
Form 990, Part VI, Section B, Line 15	The board of directors reviews the CEO's performance and determines compensation after reviewing industry reports of salary surveys.
Form 990, Part VI, Section C, Line 19	Financial statements and governing documents are available upon request by the membership. An Annual Report is given to each member attending the Annual Meeting. The Annual Report is also available at the credit union office during the year.
