PART V Line 3b the calendar year 2015. The 990-T filing for the 2015 tax year was complete

PART VI Line 6 The credit union is comproised of a single class of approved member/owners,
PART VI Line 6 each of which has equal rights in ownership, governance and voting at the
PART VI Line 6 credit unions annual meeting.
PART VI Line 7a The members of the credit union have the authority to elect the members of
PART VI Line 7a the Board of Directors for a 3-year term on a rotating basis. Candidates

PART VI Line 7a appoints a member to serve on the Supervisory Committee which is vested

PART VI Line 12c listed on an agenda for review. Once reviewed the updated policy is voted o
PART VI Line 12c and approved by the Board of Directors. All policies are available to the
