    Termination of The Joel A. and Ruth F. Leavitt Charitable Foundtion. The Joel A. and Ruth F. Leavitt Charitable Foundation (Foundation) invested in Bernard L. Madoff Investment Securities LLC (BLMIS) in 1999 via the JAL Nominee Partnership. Other than small amounts of cash, the only asset of the Foundation was the BLMIS investment. Joel A. Leavitt, the only contributor to the Foundation, was also invested in BLMIS. On December 11, 2008, BLMIS was determined to be a ponzi scheme and bankrupt. The JAL Nominee Partnership filed a Customer Claim (Claim No. 007003) with the Trustee for the liquidation of BLMIS. The Claim was allowed January 6, 2011 and was to be paid if and as the Trustee recovered funds. The Foundation's portion of the settled claim was $15,849. Joel A. Leavitt was entitled to none of the settled claim. As of March 9, 2015, the Foundation had received $8,649 and had a Claim Receivable of $7,200. In 2015, the other claimants in the JAL Nominee Partnership advanced funds to the Foundation in exchange for its Claim Receivable of $7,200. The Foundation used the $7,200 plus contributed cash of $35 to pay administrative expenses ($2,750), State filing fees ($35), and charitable contributions ($4,450 - disclosed in PartXV). At 12-31-2015 the Foundation had no assets or liabilities and no source of additional funds. The Foundation terminated 12-31-2015.
