Form 990, Part VI, Section B, Line 12c	Officers and directors review conflict of interest with the executive director at each meeting of the board of directors to insure compliance with the organizations conflict of interest policy.
Form 990, Part VI, Section B, Line 15a 15b	The compensation committee of the board of directors annually reviews and establishes salary and wage rates for the executive direector and any staff members budgeted for that particular year.
Form 990, Part VI, Section C, Line 19	Upon written request, the executive director of the JCAHBA provides copies of the associations Form 990 and any other documents governed by Section 6104.
Form 990, Part VI, Section B, Line 11a 11b	The independent CPA firm engaged to prepare the annual Form 990 completed a conference call with the executie director on November 9, 2016 to present the Form 990 and any matters which the CPA considered for further review none existed.
