AS A RESULT OF SUCH PRIOR YEAR ADJUSTMENTS THE 2015 AND 2014 NET PATIENT SERVICE REVENUE INCREASED BY APPROXIMATELY $6,300,000 AND $9,900,000, RESPECTIVELY.THE CURRENT MEDICAID AND MEDICARE PROGRAMS ARE BASED UPON COMPLEX LAWS AND REGULATIONS. NONCOMPLIANCE WITH SUCH LAWS AND REGULATIONS COULD RESULT IN FINES, PENALTIES, AND EXCLUSION FROM SUCH PROGRAMS. THE HOSPITAL IS NOT AWARE OF ANY ALLEGATIONS OF NONCOMPLIANCE THAT COULD HAVE A MATERIAL ADVERSE EFFECT ON THE FINANCIAL STATEMENTS AND BELIEVES THAT IT IS IN COMPLIANCE WITH ALL APPLICABLE LAWS AND REGULATIONS.DESIGNED BY NEW YORK STATE TO SEEK A FEDERAL WAIVER FOR THE EXPENDITURE OF MEDICAID FUNDS, THE DELIVERY SYSTEM REFORM INCENTIVE PROGRAM ("DSRIP") WILL ALLOCATE $6.42 BILLION DOLLARS TO HEALTH CARE PROVIDERS IN NEW YORK STATE TO UNDERTAKE SYSTEM REFORM, CLINICAL PROJECTS, AND POPULATION HEALTH MANAGEMENT. PROVIDERS IDENTIFIED AS SAFETY NET PROVIDERS BASED ON THE PERCENTAGE OF MEDICAID, UNINSURED, AND DUAL ELIGIBLE PATIENTS THEY SERVE ARE ELIGIBLE TO RECEIVE FUNDS UNDER DSRIP. IN ORDER TO PARTICIPATE IN DSRIP, PROVIDERS ARE REQUIRED TO FORM COLLABORATIONS CALLED PERFORMING PROVIDER SYSTEMS ("PPSS") TO EXECUTE DSRIP PROJECTS. THE HOSPITAL MET THE QUALIFICATIONS OF A SAFETY NET PROVIDER AND IS PARTICIPATING IN A COLLABORATIVE PPS. IN DECEMBER 2014, SEVERAL PPSS SUBMITTED APPLICATIONS TO THE NEW YORK STATE DEPARTMENT OF HEALTH ("NYSDOH") FOR PARTICIPATION IN THE STATE'S DSRIP. THE COLLABORATIVE PPS THAT THE HOSPITAL IS PARTICIPATING IN WAS INFORMED THAT ITS APPLICATION RECEIVED ONE OF THE HIGHEST SCORES IN THE STATE ON SEVERAL KEY MEASURES AND WAS RECOMMENDED FOR FUNDING FROM THIS PROGRAM BY THE NYSDOH TO THE CENTERS FOR MEDICARE AND MEDICAID SERVICES. ALTHOUGH THE HOSPITAL BELIEVES THAT IT WILL RECEIVE A SIGNIFICANT AMOUNT OF DRSIP FUNDING OVER THE LIFE OF THE FIVE YEAR PROGRAM, THE ULTIMATE AMOUNT OF FUNDING CANNOT BE DETERMINED AT THIS TIME SINCE IT IS DEPENDENT ON CERTAIN REGULATORY APPROVALS AS WELL AS THE ABILITY OF THE HOSPITAL TO MEET CERTAIN PROGRAM BENCHMARKS AND METRICS OVER THE LIFE OF THE PROGRAM.FOR THE YEARS ENDED DECEMBER 31, 2015 AND 2014, NET INPATIENT REVENUE FOR INDIVIDUAL PAYORS CONSISTED OF THE FOLLOWING: MEDICARE 2015 - 42% 2014 - 40%, MEDICAID 2015 - 18% 2014 - 22%, SELF-PAY 2015 - 3% 2014 - 4%, COMMERCIAL MANAGED CARE & OTHER 2015 - 37% 2014 - 34%.DURING THE YEARS ENDED DECEMBER 31, 2015 AND 2014, NET OUTPATIENT CLINIC SERVICE REVENUE FOR INDIVIDUAL PAYORS CONSISTED OF THE FOLLOWING: MEDICARE 2015 - 5% 2014 - 5%, SELF-PAY 2015 - 13% 2014 - 14%, MEDICAID/MEDICAID HMOS 2015 - 76% 2014 - 75%, COMMERCIAL HMOS 2015 - 5% 2014 - 5%, OTHER 2015 - 1% 2014 - 1%.

PART III, LINE 9B: THE COLLECTION POLICY FOR CHARITY CARE PATIENTS IS THE SAME COLLECTION POLICY THAT IS APPLIED TO THE LARGEST NUMBER OF ITS PATIENTS.
PART VI, LINE 2: FROM AN ONGOING NEEDS ASSESSMENT BASIS, ST. JOSEPH'S IS A PARTICIPANT IN THE HEALTHY YONKERS INITIATIVE WHICH IS A COALITION OF VARIOUS HEALTH CARE PROVIDERS AND SOCIAL SERVICE AGENCIES. THE HEALTHY YONKERS INITIATIVE CONDUCTS PERIODIC FOCUS GROUPS TARGETING BOTH PATIENTS AND THEIR FAMILY MEMBERS TO IDENTIFY UNMET HEALTH CARE NEEDS WITHIN THE COMMUNITY. A RESULT OF A RECENT FOCUS GROUP WAS TO DEVELOP AN INITIATIVE TO EXPAND AND ENHANCE HEALTH CARE SERVICES FOR INDIVIDUALS DIAGNOSED WITH ASTHMA AND/OR NUTRITIONAL DISORDERS.OUTSIDE OF THE ABOVE MENTIONED NEEDS ASSESSMENT, SAINT JOSEPH'S MEDICAL CENTER (SJMC) PERFORMED THE SAINT JOSEPHS MEDICAL CENTER COMMUNITY HEALTH NEEDS ASSESSMENT (SJCHNA) WITH KEY STAKEHOLDERS IN THE COMMUNITY USING A THREE PHASE PROCESS. PHASE I CONSISTED OF THE FOLLOWING:-IDENTIFIED AND CONTACTED THE KEY COMMUNITY STAKEHOLDERS AND KEY INFORMANTS TO PARTICIPATE IN THE PROCESS-DEVELOPED THE KEY INFORMANT PRE-ASSESSMENT SURVEY-HOSTED COMMUNITY MEETINGS WITH KEY STAKEHOLDERS AND CONTACTED THOSE WHO COULD NOT ATTEND FOR ADDITIONAL FEEDBACK-PARTICIPATED IN WESTCHESTER COUNTY HEALTH DEPARTMENT MEETINGSAFTER PHASE I WAS COMPLETED, PHASE II PROVIDED MORE IN-DEPTH INFORMATION REGARDING THE LEADING HEALTH ISSUES AND PRIORITIES IN THE AREA, AS WELL AS THE OPPORTUNITY TO IDENTIFY THE POTENTIAL PARTNERS FOR FUTURE COLLABORATION TO ADDRESS THE HEALTH ISSUES IDENTIFIED IN THE COMMUNITY. THE ACTIVITIES OF PHASE II INCLUDED THE FOLLOWING:-CONDUCTED IN-DEPTH INTERVIEWS WITH THE COMMUNITY KEY INFORMANTS-CONDUCTED ONE-TO-ONE SURVEYING AT THE LIBRARY IN SEPTEMBER AND OCTOBER 2013 AND AT THE WESTCHESTER DIABETES ASSOCIATION WALK IN SEPTEMBER 2013.-PARTICIPATED IN COMMUNITY FORUMS ON AUGUST 14, SEPTEMBER 19, NOVEMBER 14, AND DECEMBER 12, 2013 TO INFORM THE COMMUNITY ORGANIZATIONS ABOUT THE SJCHNA-DEVELOPED AND DISSEMINATED THE SJCHNA RESIDENT SURVEY TOOL PHASE III OF THE HEALTH NEEDS ASSESSMENT WAS TO PRIORITIZE THE IDENTIFIED COMMUNITY HEALTH NEEDS. THE SJCHNA RESULTS WERE REVIEWED BY SJMC WITH ITS KEY INFORMANTS AND COMMUNITY PARTNERS AT A STAKEHOLDER SUMMIT. SJMC REVIEWED THE FEEDBACK FROM THE STAKEHOLDERS SUMMIT ALONG WITH ITS CURRENT SERVICES AND PROGRAMS, RESOURCES AND AREAS OF EXPERTISE, AND OTHER EXISTING COMMUNITY ASSETS TO DETERMINE WHAT PRIORITY NEEDS IT WOULD ADDRESS AND THOSE FOR WHICH IT WOULD PLAY A SUPPORT ROLE IN ADDRESSING. THE PRIORITY NEED IDENTIFIED WAS CHRONIC DISEASE MANAGEMENT FOR DIABETES AND HYPERTENSION.
PART VI, LINE 3: PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE - ST. JOSEPH'S HOSPITAL MAKES AVAILABLE WRITTEN COMMUNICATIONS IN BOTH ENGLISH AND SPANISH EXPLAINING THE HOSPITAL'S POLICY OF PROVIDING CHARITY CARE AND FINANCIAL ASSISTANCE TO ELIGIBLE PATIENTS. NOTICES ARE POSTED THROUGHOUT THE HOSPITAL IN BOTH ENGLISH AND SPANISH STATING THAT CHARITY CARE AND FINANCIAL ASSISTANCE ARE AVAILABLE TO QUALIFYING INDIVIDUALS THAT INCLUDE A TELEPHONE NUMBER FOR PATIENTS TO CONTACT A FINANCIAL COUNSELOR TO DISCUSS ELIGIBILITY. IN ADDITION, HOSPITAL REPRESENTATIVES INTERVIEW PATIENTS WITHOUT INSURANCE TO DETERMINE IF THEY MAY QUALIFY FOR MEDICAID OR OTHER GOVERNMENTAL INSURANCE PROGRAMS AND WILL ASSIST THEM IN COMPLETING REQUIRED APPLICATIONS.
PART VI, LINE 4: COMMUNITY INFORMATION - ST. JOSEPH'S HOSPITAL SERVES A PREDOMINANTLY LOW INCOME, MEDICALLY UNDERSERVED POPULATION WITHIN ITS SERVICE AREA. ST. JOSEPH'S SERVICE AREA POPULATION, WHICH INCLUDES SOUTHWEST YONKERS AND THE NORTH BRONX, IS A MINORITY ONE WITH ALMOST 1/3 OF THE POPULATION BEING AFRICAN AMERICAN AND ANOTHER 1/3 BEING HISPANIC. MANY OF THE PATIENTS DO NOT HAVE A PRIVATE PHYSICIAN AND USE THE HOSPITAL'S EMERGENCY ROOM AND CLINIC FOR THEIR PRIMARY CARE MEDICAL CARE NEEDS.
PART VI, LINE 5: ST. JOSEPH'S HOSPITAL HAS A FAMILY PRACTICE TEACHING PROGRAM THAT INCLUDES APPROXIMATELY 25 RESIDENTS. THE RESIDENTS SPEND THE MAJORITY OF THEIR TIME PROVIDING MEDICAL CARE UNDER THE SUPERVISION OF FACULTY TO CLINIC PATIENTS, MOST OF WHOM EITHER HAVE NO INSURANCE OR ARE MEDICAID PATIENTS. DURING 2015, THE FAMILY HEALTH CENTER, A PRIMARY CARE CLINIC, HAD 28,547 VISITS OF WHICH 84.9% WERE APPLICABLE TO MEDICAID/MEDICAID HMO AND UNINSURED PATIENTS.
