Briefly describe the organization’s mission: The Department was organized in 1942 and incorporated in 1992. The mission of this organization is to prepare for and respond to emergencies protecting people, property, and the environment through a progressive and responsible approach to planning, education, and hazard prevention with a professionally trained, dedicated, and compassionate workforce of career and volunteer members within the boundaries of Harris County Emergency Services District #25. The Department is under contract by Harris County Emergency Services District #25 ("ESD"), who provides funding for for the Departments operating and capital expenditures. Funding is provided pursuant to an approved annual budget of operating and capital expenditures and monthly reports of amounts advanced by the ESD and amounts actually paid by the Department, and related requests for reimbursement. The Department also receives revenue from amounts billed for responses to fire alarms and non-medical emergency services provided.

	Provide fire suppression and emergency services under contract by and within the boundaries of the Harris County Emergency Services District #25.On average, the Department responds to 100-125 incidents per month.The Department operates two fire stations staffed by both paid and volunteer personnel 24 hours a day, 7 days a week. This assures a quick response time no matter the time of day.

	During 2014, Westfield Fire Department enhanced its firefighter training program by implementing an online training program designed by the command staff. This e-learning platform not only makes training more readily available to firefighters, but is also used to increase communications amongst the agency.

	The Westfield Fire Department began identifying target hazards within its response territory and conducting pre-plans of these hazards. The pre-plans were then entered into the computer system at our communications center as well as our reporting system, so that information regarding the business is readily and easily available to responding crews. By doing this, it increases firefighter and citizen safety.

Form 990, Part I	An audit of 2015 was completed following the filing of the original return. This amended return is filed to reflect the audited balances. This 2015 amended return reflects changes to balances as follows: * Total revenue - a decrease of $6,631. * Total expenses - a decrease of $6,631, including a decrease in compensation related expenses of $500, a decrease in other program expenses of $7,573 and an increase in general and administrative expenses of $1,442.
Form 990, Part VI, Section A, line 6	The Department's Bylaws establish Membership categories as "Active Volunteer Members", "Probationary Members and "Duty Crew".
Form 990, Part VI, Section A, line 7a	The Department's Members elect the Board of Directors from among it's Membership, as follows: Three (3) positions will be elected in January of even numbered years, and will start their terms on the date of election. These three (3) positions will be open to Active Volunteer members. Two (2) positions will be elected in January of odd numbered years, and will start their terms on the date of election. These two (2) positions will be open to paid Duty Crew members.
