| CORE FORM, PART III; STATEMENT OF PROGRAM SERVICE ACCOMPLISHMENTS | BACKGROUND ============ Aria Health is the largest healthcare provider in Northeast Philadelphia and Lower Bucks County. With three leading-edge community hospitals and a strong network of outpatient centers and primary care physicians, Aria upholds a longstanding tradition of bringing advanced medicine and personal care to the many communities it serves. Aria Health is recognized by the Internal Revenue Service ("IRS") as an Internal Revenue Code 501(c)(3) tax-exempt organization. Pursuant to its charitable purposes, Aria Health provides healthcare services to all individuals in a non-discriminatory manner regardless of race, color, creed, sex, national origin or ability to pay. Moreover, Aria Health operates consistently with the following criteria outlined in IRS revenue ruling 69-545: 1) Provides healthcare services to all individuals regardless of ability to pay, including Charity Care, Self-Pay, Medicare and Medicaid patients; 2) Operates 3 active emergency departments for all persons; which are open 24 hours a day, 7 days a week, 365 days per year, including a trauma emergency department on the Torresdale campus; 3) Maintains an open medical staff, with privileges available to all qualified physicians; 4) Control of Aria Health rests with its board of directors and the board of directors of Aria Health System, Inc. Both boards are comprised of independent civic leaders and other prominent members of the community; and 5) Surplus funds are used to improve the quality of patient care, expand and renovate facilities and advance medical care, programs and activities. The operations of Aria Health as shown through the factors outlined above and other information contained herein, clearly demonstrate that the use and control of Aria Health is for the benefit of the public and that no part of the income or net earnings of the organization inures to the benefit of any private individual nor is any private interest being served other than incidentally. History ======= The story of Aria Health System began in 1902, when a resident of the Frankford section of Philadelphia contracted typhoid fever. The patient's physician, Dr. Joseph Ball, couldnt find a single hospital in the city to admit his seriously ill patient. Not one of the downtown hospitals had an available bed. When Dr. Ball learned other Northeast physicians were also having their patients turned away, he began an effort to establish a community hospital that would provide convenient, high-quality medical care to his neighbors. Named after the neighborhood it would first serve, Frankford Hospital - Frankford (now Aria Health - Frankford Campus) opened its doors on the fourth of July, 1903. During the decades that followed, Frankford expanded upon Dr. Balls founding vision. For over a century, the Hospital introduced many new services to better meet the health needs of its growing communities. For example, as the Hospitals service area grew and flourished, Frankford responded by opening a second site in 1977. Almost immediately, Frankford Hospital Torresdale (now Aria Health - Torresdale Campus) was operating at full capacity, meeting the healthcare needs of families from the Far Northeast and Lower Bucks County. When many families Frankford Hospital had been serving for generations began relocating to Bucks County in the 1980s and 1990s, the Hospital again sought better ways to serve them. In March 1999, Frankford acquired Delaware Valley Medical Center and turned it into its third inpatient site Frankford Hospital Bucks County (now Aria Health - Bucks County). To reflect an expanded, regional vision for health care service delivery in Northeast Philadelphia and Bucks County, Frankford Hospital changed its name to Aria Health in May 2009. Today, Aria Healths focus on its founding philosophy is clearer than ever before. Now, our physicians, nurses and support personnel provide advanced medicine and personal care at three modern inpatient facilities and three outpatient sites all of which are equipped with highly sophisticated technology. In addition to our state-of-the-art intensive care and critical care units, Aria Health-Frankford is planning to expand lower northeast Philadelphia's only Emergency Department by an additional 8,000 square feet to accommodate 60,000 annual visits. Primary Care ------------ Aria Health - Frankford provides convenient primary care on the hospital campus and at the Wakeling Building, adjacent to the hospital. Also on the campus, our Health Center Clinic handles more than 5,000 outpatient cases per year. Preventive Health and Wellness Services --------------------------------------- Aria Health - Frankford is invested the health and well-being of our neighbors. That's why we offer education and wellness programs focused on chronic conditions like Congestive Heart Failure, Chronic Obtrusive Pulmonary Disease and Diabetes. Aria Health - Frankford is also involved with outreach to local businesses and community members by providing on-site AED and CPR Training, participating in numerous health and community fairs throughout the year and by hosting community meetings on our campus. Torresdale ========== Aria Health - Torresdale delivers a wide range of specialty, emergency and primary care in a warm, friendly, service-oriented environment. Minimally Invasive Surgical Services ------------------------------------ Throughout Aria Health's Department of Surgery, specialists take advantage of techniques to reduce the impact of surgery and make the experience less disruptive. Most notably, since August 2010 hundreds of minimally invasive-robotic assisted surgeries have occurred in a variety of specialties including: Thoracic, Gynecologic, Urological and Thyroidectomy surgeries. (2) The Professional Court - Opened in 1999, Aria's Professional Court Medical Building is its newest outpatient center. Administering 7,658 annual procedures, this new site offers a range of healthcare services and outpatient testing for the entire family. In one convenient location we offer primary care, internal medicine, obstetrics and gynecology, ophthalmology and optometry, general radiology, DEXA bone scanning, mammography, ultrasound, Pulmonary Function Testing, Sleep Studies, CAT Scan, MRI and laboratory testing/Phlebotomy Services. (3) Aria Health Center for Rehabilitation and Wellness - This outpatient site currently administers mainly physical medicine & rehabilitation, EMG testing, sleep studies, and wellness center services. Wellness Programs ================= Aria has a long tradition of making disease prevention and health promotion accessible and appealing to its patients, families and community by providing a variety of dedicated programs in fitness and group exercises, nutrition counseling and weight loss, and disease management. Nutrition --------- (1) Individual Nutritional Counseling - Nutritional assessment and counseling provided by registered dietitians for general nutrition and a variety of medical concerns requiring nutritional intervention. (2) Healthy Lifestyle Program - The healthy lifestyle program is a nutrition education program that focuses on helping you to build healthy habits to aid in weight management and to treat and prevent disease. (3) PCOS (Polycystic Ovary Syndrome) Nutrition Education Series - Feel comfortable in this group with others that share this common endocrine problem. The bi-monthly meetings will include educational topics on carbohydrate counting, healthy food choices to help manage cholesterol and blood pressures, weight management, benefits of exercise, and recipe tips. (4) Medgem Indirect Calorimeter Testing - This service requires a simple breath test to accurately measure your individual energy requirements. (5) Nutrition Counseling for Bariatric Surgery - This service is provided for patients thinking about the lap banding weight loss procedure that is now offered at the Oxford Valley Medical Plaza in Langhorne, PA. (6) Heart Healthy & Diabetes Supermarket Tours - These tours will emphasize label reading for fats: saturated and unsaturated, fiber, soy protein, carbohydrate, and other key ingredients to help you manage your blood sugars, cholesterol, and triglyceride levels. In the fall of 1904, a training school for nurses was established in Northeast Philadelphia, just a block away from Aria Health - Frankford Campus, which had been founded in 1903. Under the direction of Anna C. Garrett, who was also Aria Health - Frankford Campus's first superintendent, the Frankford Hospital School of Nursing set forth to provide exceptional education to students interested in pursuing the nursing profession. The Frankford Hospital School of Nursing developed into a distinguished co-educational institution, and over a century-plus provided more than 2,500 students with comprehensive learning experiences in professional nursing. In 2004, Aria Health School of Nursing, under its former name of The Frankford Hospital School of Nursing, celebrated its 100-year anniversary. The decision to move from a diploma program to a BSN-degree granting program was the result of a thoughtful and deliberate process involving an integration team that included nursing and nursing education leaders. Other academic services provided to the residents include a subscription to Challenger, which is an online board review tool with a full curriculum for the life of the residency, and a subscription to up to date. | CORE FORM, PART III; STATEMENT OF PROGRAM SERVICE ACCOMPLISHMENTS | In-house rotations include standard Floor/ Telemetry teaching Teams at Bucks and 2 excellent but vastly different ICU experiences. The Bucks ICU allows for individual growth and autonomy with oversight from 2 different critical care attendings as well as the admitting attending. The Torresdale ICU is a much larger and busier ICU with a wide mix of medical, surgical, and trauma patients. Residents will round with an organized critical care team led by a full-time Intensivist. Both ICU's allow for significant procedural experience. Multiple large university hospitals in the area provide excellent academic and clinical experiences for completing elective and selective requirements, and we place very few restrictions on outside rotations. This allows for valuable "face time" at institutions where you may be pursuing a fellowship. Residents also complete a weekly half-day ambulatory medicine experience in the hospital's urban Internal Medicine Clinic at the Frankford campus. The residents are able to develop their own panel of patients that they manage throughout their residency career. Monthly patient information sessions are held by the residents to educate the Clinic patients about common medical topics, like tobacco cessation and diabetes. We schedule patients so residents will have sufficient time to spend with each patient and be comprehensive in their individual care plan. The traditional call schedule has been replaced with a Night-Float system which allows for better continuity in patient care. The Night-Float system also eliminates missing time away from rotations when residents traditionally would have been post-call. The Aria Health Internal Medicine residency program is committed to providing our residents with the best educational experience found in a community hospital.
