Return Reference Explanation
General explanation attachment Form 990-EZ was not filed on time due to a change in accountant at the beginning of 2015. The new outside accountant was not made aware that the filing of this tax form was part of the scope of her duties, mainly payroll reports and bank reconciliations. This form was completed as soon as possible after the discovery of non-compliance was realized.
