Part X : FIN48 Footnote EMHS, its hospitals, and certain other affiliates have been determined by the Internal Revenue Service to be tax-exempt charitable organizations as described in Section 501(c)(3) or 501(c)(2) of the Internal Revenue Code ("Code") and, accordingly, are exempt from federal income taxes on related income pursuant to Section 501(a) of the Code. Accordingly, no provision for federal income taxes has been recorded in the accompanying consolidated financial statements for these organizations.Tax-exempt charitable organizations could be required to record an obligation for income taxes as the result of a tax position they have historically taken on various tax exposure items including unrelated business income or tax status. Under guidance issued by the Financial Accounting Standards Board (FASB), assets and liabilities are established for uncertain tax positions taken or positions expected to be taken in income tax returns when such positions are judged to not meet the more-likely-than-not threshold, based upon the technical merits of the position. Estimated interest and penalties, if applicable, related to uncertain tax positions are included as a component of income tax expense. The System has evaluated its tax position taken or expected to be taken on income tax returns and concluded the impact to be not material.Certain of the System's affiliates are taxable entities. Deferred taxes related to these entities are based on the difference between the financial statement and tax basis of assets and liabilities using enacted tax rates in effect in the years the differences are expected to reverse. Part V, Line 5 - Account Input from Persons Who Represent the Community The Maine Shared CHNA research team conducted a statewide survey among stakeholders to identify and prioritize significant health issues in communities across the state. The survey, coordinated with the Maine CDC, engaged public health expertise throughout the process. The survey was administered using a snowball approach where stakeholder agencies agreed to send the surveys to their members and stakeholders for participation. 1,639 people, representing more than 80 organizations and businesses in Maine, completed the survey. Respondents represented health care agencies, public health agencies, law enforcement, municipalities, schools, local businesses, social service agencies, and nongovernmental organizations. In addition to the stakeholder survey, each county conducted a number of community engagement opportunities (forums) to ensure broad interests of the local community were represented; Obtain stakeholder input on identifying significant health needs based on review of shared CHNA data; Solicit stakeholder feedback on prioritizing significant health needs, and; Identify local assets and resources that potentially may address local health priorities. The local forums were conducted in collaboration with the Maine CDC to solicit input from individuals representing populations with health disparities. The following organizations were in attendance at the January 11, 2016 Cumberland County Shared CHNA Community Engagement Forum and provided valuable feedback on the Shared CHNA.Details about organizations: Sample of organizations United Way, ME Health Access Foundation, Southern ME Area Agency on Aging, Preble St. Resource Center, legislator, Somali community members, USM/UNE/SMCC/Tufts medical School, ME Health Mgt Coalition, MaineMedical Center (hospital facility), Spring Harbor Hospital, Mercy Hospital, Bridgton Hospital, HMP, & Portland Public Health. Hospitals and health care providers serve low income patients through Medicare/MaineCare and provide low-cost/no-cost services to medically underserved residents. Many public health programs and services, social service agencies, funding agencies, and non-profit agencies specifically serve racial/ethnic minorities, low-income, and medically underserved residents. Somali community members attended the forum.

Part III, Line 2 - Methodology Used To Estimate Bad Debt Expense The costing methodology used to determine the amount is cost to charge ratio.
Part III, Line 4 - Bad Debt Expense Patient and trade accounts receivable are stated at the amount management expects to collect from outstanding balances. Management provides for probable uncollectible amounts through a charge to earnings and a valuation allowance based on its assessment of the current status of individual accounts. Balances that are still outstanding after management has used reasonable collection efforts are written off through a charge to the valuation allowance and the applicable patient accounts receivable. Credit is extended without collateral.
Part III, Line 8 - Explanation Of Shortfall As Community Benefit Medicare losses should be treated as a community benefit because the losses are incurred in performing an important public service and Maine hospitals experience one of the lowest Medicare reimbursement rates in the country.
Part III, Line 9b - Provisions On Collection Practices For Qualified Patients All account guarantors who express an inability to pay inpatient and outpatient services will be screened for eligibility for charity care using an application and guidelines established by Mercy Hospital. An account may be reconsidered for charity care at any time when new information is available about a patients inability to pay.
Part VI - Needs Assessment See Part V - Community Health Needs Assessment section
Part VI - Patient Education of Eligibility for Assistance Mercys Financial Assistance policy describes how the organization informs and educates patients on Financial Assistance. Public Notice of the availability of assistance through this policy is made through each of the following means: 1. Posting notices in a visible manner in locations where there is a high volume of inpatient or outpatient admitting/registration, such as emergency departments, billing offices, admitting offices, and hospital outpatient service settings. 2. Posting notice of the availability of assistance and contact names and phone numbers on Mercy Hospitals website. 3. Providing uninsured patients a matrix outlining the types of financial assistance available. Upon request, a full text copy of the Financial Assistance policy is made available. Mercy Hospital has developed a professional model for financial counseling for all patients without insurance and has staff onsite to meet individually from 8am to 5pm on Monday through Friday. This includes comprehensive screens for potential resources such as governmental insurance and disability, enrollment on the Health Insurance Marketplace, and commercial insurance. Financial counselors have daily huddles, assigned work lists, and accountabilities to engage and develop an ongoing rapport with uninsured and vulnerable populations. They are available Monday through Friday from 8am-5pm and routinely meet with patients in the community to complete applications. With the advent of the ACA, the financial counselors have all been trained to act as Certified Application Counselors and helped to enroll both Mercy patients as well as to provide staffing for the community at the Portland Public Library.
Part VI - Community Information Mercy Hospital is a community hospital comprised of two acute inpatient locations, six primary care practices, five Express Care facilities, and 14 specialty practices. Mercys service area is the greater Portland metropolitan area, roughly comprised of Maines Cumberland County. The Portland area is experiencing an increase in diversity, with at least six percent of area residents born outside of the United States. Many of these new Mainers are refugees and asylum-seekers, who bring a unique set of circumstances and health needs to the community. Approximately 11 percent of Cumberland county households fall below the federal poverty level.Mercy is a Catholic hospital founded by the Sisters of Mercy in 1918. Its mission is to carry out the healing work of Christ by providing clinically excellent, compassionate health care for all, with special concern for the poor and disadvantaged. It has fulfilled that mission for nearly 100 years, both in the hospital and in the community. Mercys mission-based programs such as McAuley Residence, a comprehensive program with housing for women battling opioid use disorder and their children and Garys House, a hospitality house are tremendous community resources. The Medical Neighborhood project, a formal health collaborative created by Mercy, proactively seeks uninsured and under-insured persons in Greater Portland to complete a comprehensive search of all coverage options and facilitate access to concrete supports such as housing, food, and employment services. Mercys financial counselors routinely meet patients at the local shelters or other social service agencies.
Part VI - Community Building Activities The majority of Mercy Hospitals Board of Directors resides in the primary service area and is not employees or independent contractors. Mercy also has an open medical staff. Mercy Hospital donates food weekly as part of a food rescue program to local food pantries and soup kitchens.

The Shared Community Health Needs Assessment informs initiatives to promote community health across the system as well as within each member hospitals local service area. EMHS tirelessly seeks opportunities to innovate delivery of care by challenging the status-quo and pushing our organization to fearlessly exceed expectations in today's ever-evolving healthcare landscape. When taking a look back at 2016, we celebrate the amazing work being done across EMHS. Here, leaders come in all forms and nowhere is it more evident than in our 2016 Annual Report. Issues such as childhood obesity and the prevalence of mental health issues like anxiety and depression among our youth have garnered the attention of parents and healthcare providers.Fortunately, research has shown that providing our children with healthy activities that teach them the value of exercise, eating well and being part of their communities can make a big difference in their long-term well-being.With that in mind, EMHS organizations offer creative programs and activities designed to support the healthy development of kids, many times in collaboration with other area organizations.A shining example of this approach can be seen in Presque Isle, where The Aroostook Medical Center (TAMC) partnered with the University of Maine at Presque Isle and Healthy Aroostook to organize The REDY Youth Triathlon in September, 2016 as part of TAMC's ongoing Fit and Fun Series.Children, ages 7 through 14, swam, biked, and ran different distances based on their age category. Event organizers named the triathlon after REDY, a well-known mascot of Let's Go 5-2-1-0 (5 or more fruits or vegetables a day, 2 or less hours of recreational screen time, 1 hour of physical activity, and 0 sugary drinks),a nationally recognized youth obesity prevention program that has partners throughout Maine.Jayden Harvell age 8, who according to her mother, Heather, is very athletic, participated in the event, which was her second triathlon. Before Jayden participated, she commented, "Yeah, I'm growing up and I'm getting healthier and smarter, and this helps me get fit. I think it will be fun!"The concept of 5-2-1-0 is also reflected in Inland Hospital's Family Fun Series, now in its fifth year. Inland Hospital, located in Waterville, works with other local organizations to provide a monthly family-oriented event that promotes an active lifestyle. The hospital's community wellness team leader, Ellen Wells, plays an integral role in making the series run smoothly. "We are trying to engage the whole family to make physical activity a way of life. Down the road, that will mean lower adult obesity rates and the related chronic disease that goes along with that."In addition to the Family Fun Series, every Tuesday evening during the summer of 2016, Inland sponsored the Quarry Road Fun Run series. The Fun Run series was very popular, with an average of 15 participants for the nine-week program.Another innovative program that focuses on healthy weight and food choices is the Eastern Maine Medical Center (EMMC) Way to Optimal Weight program (WOW); it focuses on helping children and adolescents who are at higher risk for weight-related health problems. The program recently expanded to Maine Coast Memorial Hospital (MCMH) in Ellsworth, as well as TAMC via televideo technology in Presque Isle.MCMH, in partnership with the Down East Family YMCA, is also using physical activity as a conduit for developing the full spectrum of health-mind, body, and spirit-for young girls through the Girls on the Run (GOTR) curriculum. This is vital, as girls and young women face an overwhelming amount of exposure to images and messages that leave them questioning their personal value."One of the things I love about Girls on the Run is that it's big picture. It's not just about running. Its about teaching the girls life skills that will benefit them for the rest of their lives," explained Kimberly Formby, GOTR coach and Occupational Therapy supervisor at MCMH. "It is my hope that every third through fifth grade girl in our area has the opportunity to participate in this program." Meeting twice a week in small teams, Girls on the Run teaches life skills through dynamic, interactive lessons and running games. The curriculum is taught by certified Girls on the Run coaches and includes three parts: understanding ourselves, valuing relationships and teamwork, and understanding how we connect with and shape the world at large. Running is used to inspire and motivate girls, encourage lifelong health and fitness, and build confidence through accomplishment. Important social, psychological, and physical skills and abilities are developed and reinforced throughout the program.The program is in its second season and has already had 16 girls participate, with plans for another session in the spring of 2017.A program that focuses on a young person's psycho-social-emotional development, in the context of the broader community is Challenge Day, a nonprofit out of California. For 15 years, Acadia Hospital, the only EMHS psychiatric hospital, has coordinated and supported Challenge Day in dozens of Maine high schools, affecting thousands of students and hundreds of volunteer adult facilitators.This past fall, more than 400 high school students experienced Challenge Day at participating schools: Hermon High School, Freeport High School, Maine Central Institute in Pittsfield, and George W.Stearns High School in Millinocket.One Stearns High School student echoed the sentiments of those who wrote reflection papers after their experience, "Challenge Day was an extraordinary experience, unlike any other. The day is a great way to express your true feelings to one another, learn about other people, see what they have gone through, learn empathy and change who you are for the better."In an era where youth report feeling increasingly judged and isolated, Challenge Day provides a proven experiential program that helps transform individuals and entire school cultures. At Hermon High School, which is the only school to host Challenge Day continuously over the past 15 years, the positive effect has been remarkable.Retired Hermon High School health teacher Shelley Gavett, who was instrumental in Hermon's involvement, continues to coordinate the event in a volunteer capacity.
